Court rulings on medication errors in Germany: focus on heparin, acetylsalicylic acid, and oxytocin.

Medication errors remain a challenge in the pharmacological treatment of patients. To improve the safety of drug therapy, 197 publicly accessible court rulings from Germany were systemically analyzed in various dimensions according to the PRISMA 2020 statement. Particular attention was paid to the three active ingredients that were mentioned most frequently in connection with allegations of medication errors in court. According to the results of our keyword search, current as of April 2024, these are heparin, acetylsalicylic acid, and oxytocin. Cases involving MOR agonists and benzodiazepines

A Standardized Formulary to Reduce Pediatric Medication Dosing Errors: A Mixed Methods Study.

Objective: We hypothesized that implementation of a Medical Control Guideline (MCG) with a standardized formulary (fixed medication concentrations) and pre-calculated medication dosages in a large emergency medical services (EMS) system would reduce pediatric dosing errors. To assess the effectiveness of the standardized formulary to reduce errors, we chose to evaluate midazolam administration for seizures, because it is the most frequently dosed medication by EMS for children, and seizures are a time-sensitive condition. The objective of this study was to compare: 1) frequency of midazolam do

Epidemiology of and Risk Factors for Harmful Anti-Infective Medication Errors in a Pediatric Hospital.

BACKGROUND: Literature is limited on pediatric anti-infective medication errors. There is a pressing need for additional research, as studies suggest high rates of overall pediatric medication errors and known harmful side effect profiles for anti-infective medications with narrow dosing ranges. This study aimed to identify risk factors related to harmful anti-infective medication errors in pediatric patients. METHODS: A retrospective chart review of all voluntary error reports involving anti-infective medication errors and pediatric patients (0 to < 22 years old) reported June 2014-December

Finite sample size errors in the context of multiple error sources in quantitative medical imaging: An evaluation for breast magnetic resonance diffusion-weighted imaging.

BACKGROUND: Selecting appropriate sample sizes in magnetic resonance imaging studies is a complex process that requires to balance statistical rigor with the practical challenges of measuring a large patient population. In this Institutional Review Board approved study, we evaluate the dominant error types ("finite N" errors versus precision errors) for apparent diffusion coefficient (ADC)-based lesion characterization in diffusion-weighted magnetic resonance imaging (DWI) of the female breast in a local dataset and compare our results with current literature. METHODS: First, in a literature

The science of medical decision making: neurosurgery, errors, and personal cognitive strategies for improving quality of care.

During the last 2 decades, there has been a shift in the U.S. health care system towards improving the quality of health care provided by enhancing patient safety and reducing medical errors. Unfortunately, surgical complications, patient harm events, and malpractice claims remain common in the field of neurosurgery. Many of these events are potentially avoidable. There are an increasing number of publications in the medical literature in which authors address cognitive errors in diagnosis and treatment and strategies for reducing such errors, but these are for the most part absent in the neur

Methotrexate toxicity due to a medication dispensing error compounded by an interaction with proton pump inhibitors.

Methotrexate at a low weekly dose is used for immunomodulation in autoimmune conditions such as rheumatoid arthritis and psoriasis. With an elimination half-life of a few hours, the drug is primarily cleared through the renal route. Elimination of the drug is delayed in the presence of polypharmacy or deranged renal function. Here, we present a case of a 55-year-old lady who was on chronic once-weekly methotrexate therapy for rheumatoid arthritis. A recent change in the branded generic formulation of methotrexate by her pharmacist led to confusion with another prescription medication she was r
