Changing Faces, Changing Futures: The CLAPP Humanitarian Cleft Program Model for Cleft Care in Low-Resource Settings.

Cleft lip and palate (CLP) represents the most common congenital anomaly worldwide, with the majority of untreated cases concentrated in low and middle-income countries (LMICs). The Cleft Lip and Palate Association of Pakistan (CLAPP) has developed a locally driven, programmatic model to deliver comprehensive cleft care in resource-constrained settings. This study describes the operational structure, clinical protocols, and outcomes of the CLAPP humanitarian program, providing a replicable framework for similar health care systems. Since 2004, CLAPP has conducted 205 surgical programs across P

Paratyphoid blamed on Ulster: a nursing odyssey.

The aim of the Modicum mission from the United States was to determine the fate of the Western World, the Second Front and the Manhattan Project plans for development of atomic weapons. The Modicum mission was appointed in March 1942 by Franklin Delano Roosevelt as President and Commander-in-Chief of the US forces. The journey via the Anglican Cathedral in Bermuda, to Gander, to London, to Ulster was eventful. There was a clay-pigeon shooting contest in Gander. Generals Marshall, Eisenhower, Clark and Averell Harriman were outshot by their pilot. In Ulster, an escorting US sergeant killed a Lo

Knowledge and power: the asymmetry of interests of Colombian and Rockefeller doctors in the construction of the concept of "jungle yellow fever," 1907-19381.

This study examines the asymmetries among the different interests of officials and medical doctors who worked for the Rockefeller Foundation and their Colombian counterparts in the development and consolidation of the concept of "jungle yellow fever," as distinguished from the known urban form of yellow fever. We explore the research responses to a variety of disease outbreaks in Colombia in the context of the Rockefeller campaigns against yellow fever, from the time of Roberto Franco's initial description of "yellow fever of the forests" in 1907 until the consolidation of the concept of "jung

Ethical challenges faced by French military doctors deployed in the Sahel (Operation Barkhane): a qualitative study.

BACKGROUND: French military doctors are currently deployed in the Sahel to support the armed forces of Operation Barkhane, in medical or surgical units. As well as supporting French soldiers, their other missions are diverse and complex: medical assistance to civilians and persons under control (PUC), advice to commanding officers. These tasks can create ethical dilemmas when decisions are forced upon doctors that may be in conflict with medical values or fundamental principles. Little is known about the specific dilemmas experienced by French military doctors in overseas operations. We theref

Measuring and Comparing the Cost-Effectiveness of Surgical Care Delivery in Low-Resource Settings: Cleft Lip and Palate as a Model.

Cleft lip and palate (CLP) care is the longest sustained global effort in humanitarian surgical care. However, the relative cost-effectiveness of surgical delivery approaches remains largely unknown. We assessed the cost-effectiveness of two strategies of CLP surgical care delivery in low resource settings: medical mission and comprehensive care center. We evaluated the medical records and costs for 17 India-based medical missions and a Comprehensive Cleft Care Center in Guwahati, India, from Operation Smile, a humanitarian nongovernmental organization. Age, sex, diagnosis, and procedures were

4.5 % incidence of accidental hypothermia in a German HEMS service - A retrospective 14-year analysis.

OBJECTIVE: Helicopter emergency medical services (HEMS) treat patients suffering from a multitude of conditions. Accidental hypothermia, defined as an unintentional drop in core temperature below 35 °C, may occur regardless of age, season, or ambient temperature. The underlying reasons can be cold exposure, impaired thermoregulation, decreased heat production, or increased heat loss. The incidence of accidental hypothermia in HEMS is unclear. METHODS: This retrospective analysis included patients with a documented temperature < 35 °C attended by a helicopter of the German ADAC air rescue serv

Ethical issues in vesico-vaginal fistula care and research.

The plight of women in poor nonindustrialized countries who have incurred catastrophic childbirth injuries, such as vesico-vaginal and recto-vaginal fistulas, from prolonged obstructed labor is receiving increased attention from the world medical community. While the good intentions that have prompted this greater concern are not in doubt, intentions by themselves are insufficient guarantees of ethical conduct in programs developed to repair these injuries. Clinical proposals put forward to deal with the problem of fistula must undergo critical analysis to insure that basic ethical requirement

Delivering orthopaedics in Burundi: a model for humanitarian surgery in resource-limited settings.

Burundi remains one of the most socioeconomically challenged countries globally, facing profound limitations in healthcare infrastructure, workforce, and access. In this context, the Italian Medical Foundation for Central Africa (FIMAC) has conducted humanitarian orthopaedic missions for over two decades in Bubanza, addressing critical musculoskeletal conditions in both paediatric and adult populations. This essay provides a comprehensive overview of the operational, clinical, and ethical dimensions of these missions. Commonly treated pathologies include chronic osteomyelitis, neglected fractu

[Foreign firms and dissemination of western medicine in China before the Opium War].

From the beginning of the 19th century to the Opium War, by taking the advantages of free entering and departing China, and to protect themselves, the foreign firms introduced vaccination technique into China. This is the beginning of the introduction of western medicine to China. After 1807, following the arrival of protestant missionaries, foreign firms became the stronghold for the missionaries to conceal their missionary status so as to propagate the principles of Christianity. With the aid of the business firms employee's legal identity, the missionaries started their activities of delive

A Health Systems Perspective on the Mission Model for Cleft Lip and Palate Surgery: A Matter of Sustainability or Responsibility?

One in 700 children around the world are born with cleft lip and/or palate (CL/P). Although reconstructive surgery is widely available in high-income settings, over 2 billion people in low- and middle-income countries lack access to essential surgical care. The mission model has been demonstrated to be highly effective in responding to the global surgical workforce crisis, but has been questioned in regard to its sustainability, value, and overall impact. Through effective health systems integration, the mission model presents abundant opportunities for streamlined delivery and horizontal impa

Extracorporeal membrane oxygenator rescue and airborne transportation of patients with influenza A (H1N1) acute respiratory distress syndrome in a Mediterranean underserved area.

Adult respiratory distress syndrome (ARDS) secondary to H1N1 viral infection has been a worldwide medical and organizational challenge. We report our experience with extracorporeal membrane oxygenator (ECMO) rescue and transportation of patients with H1N1 ARDS within an insular and rural Mediterranean area of seven million inhabitants. A 24/7 on-call ECMO team was organized including one anesthesiologist, one cardiac surgeon, and one perfusionist. To limit missions' time to and from peripheral hospitals, airborne transportation with helicopter was the first choice. From November 2009 to Januar
