Study of arginine metabolism in medication overuse chronic migraine: possible defect in NO synthesis.

BACKGROUND AND AIM: The pathogenesis of the pain that occurs in episodic migraine attack is due to the activation of the trigeminal system's first neuron receptors located on vessel wall. The release from the endothelium of nitric oxide, a product of arginine metabolism, causes vasodilation and stretching of the vascular trigeminal system and promotes pain. It is unknown whether this same metabolic event is involved in the pain accompanying chronic migraine. To understand the possible role of arginine in the pathogenesis of chronic migraine patients, we evaluated the metabolism of arginine in

Volume Gain of Brainstem on Medication-Overuse Headache Using Voxel-Based Morphometry.

BACKGROUND: Histopathology identified the anatomical and molecular abnormalities of brainstem nuclei in migraine patients. However, the exact whole brainstem structural changes in vivo have not yet been identified in medication-overuse headache (MOH) transformed from migraine. The aim of this study was to investigate the regional volume changes over the whole brainstem in the MOH patients using voxel-based morphometry (VBM) in vivo. METHODS: High-resolution three-dimensional structural images were obtained using a 3.0-Tesla magnetic resonance system from 36 MOH patients and 32 normal controls

Chronic migraine with and without medication overuse: experience in a hospital series of 434 patients.

INTRODUCTION: Chronic migraine (CM) is a complication of episodic migraine, favored by risk factors as medication overuse (MO). We intend to compare demographic and clinic characteristics of a series of CM patients, with and without MO. METHODS: The study included patients with CM (2006 revised criteria) attended in a headache outpatient office located in a tertiary hospital between January 2008 and May 2012. We recorded demographic characteristics, age at migraine onset, time from onset, previous use of symptomatic or preventive therapy, and headache impact measured with six-item headache im

Increased susceptibility to cortical spreading depression in an animal model of medication-overuse headache.

OBJECTIVE: The objective of this article is to evaluate electrically evoked thresholds for cortical spreading depression (CSD) and stress-induced activation of trigeminal afferents in a rat model of medication-overuse headache (MOH). METHODS: Sumatriptan or saline was delivered subcutaneously by osmotic minipump for six days to Sprague-Dawley rats. Two weeks after pump removal, animals were anesthetized and recording/stimulating electrodes implanted. The animals were pretreated with vehicle or topiramate followed by graded electrical stimulation within the visual cortex. CSD events were ident

Habituation to pain in "medication overuse headache": a CO2 laser-evoked potential study.

OBJECTIVE: Our aim was to investigate CO(2) laser-evoked potential (LEP) habituation to experimental pain in a group of patients affected by medication-overuse headache, with a history of episodic migraine becoming chronic, before and after treatment, consisting in acute medication withdrawal and a preventive treatment cycle. BACKGROUND: One of the main features of LEPs in migraineurs is a lower habituation to repetitive noxious stimuli during the interictal phase. METHODS: LEPs were recorded to stimulation of both the right hand and the right perioral region in 14 patients and in 14 healthy

Medication overuse headache: rates and predictors for relapse in a 4-year prospective study.

We present a prospective 4-year follow-up study of 96 patients with medication overuse headache following withdrawal. Complete datasets were available from 85 patients (89%) 6 months, from 79 patients (82%) 1 year and from 75 patients (78%) 4 years after withdrawal. Twenty-six patients (31%) relapsed within the first 6 months after withdrawal. The number of relapses increased to 32 (41%) 1 year and to 34 (45%) 4 years after withdrawal. The 4-year relapse rate was lower in migraine than in tension-type headache (32% vs. 91%, P<or=0.001) and combination of migraine and tension-type headache (32%

Whole blood DNA methylation signature of epigenetic aging in medication overuse headache.

BACKGROUND: Migraine is the most frequent primary disorder of medication overuse headache (MOH). As MOH progresses, patients tend to increase medication doses, which paradoxically results in more frequent headaches and reduced efficacy of analgesics. Epigenetic age, which can be evaluated by DNA methylation (DNAm)-based models, has been associated with disease risk, morbidity, and mortality in several health conditions. The principal aim of this work was to study epigenetic aging-related mechanisms in MOH. METHODS: This study is part of Epimode project in which MOH (n = 23), episodic migraineu

Two-Year Outcomes and Interictal Burden After Treatment for Medication Overuse Headache.

Background/Objective: Medication overuse headache (MOH) is a disabling secondary headache disorder that arises from an underlying primary headache, most commonly migraine. Although treatment reduces headache frequency and medication overuse, the interictal burden-the impact experienced between headache attacks-remains poorly characterized over the long term. This study evaluated interictal burden and clinical outcomes two years after MOH diagnosis. Methods: This study was derived from a prospective multicenter cohort of patients with MOH, using data from a single center. Of 149 patients enroll

Network meta-analysis comparing efficacy of different strategies on medication-overuse headache.

BACKGROUND: Medication-overuse headache (MOH) is the most common secondary headache disorder, resulting from or leading to the frequent use of acute headache medications. Despite the availability of various treatment strategies, the optimal approach remains uncertain. OBJECTIVE: This network meta-analysis (NMA) aimed to evaluate the comparative efficacy of different strategies for managing MOH, focusing on reducing monthly headache days. METHODS: We systematically reviewed randomized controlled trials (RCTs) comparing withdrawal strategies, including bridging therapies, the use of concurrent

Critical reflections on medication overuse headache in patients with migraine: An unsolved riddle in nociception.

Migraine chronification very frequently exposes patients to the inevitable risk of excessive symptomatic intake that, in turn, prompts development of medication overuse headache (MOH). The latter further compromises headache severity establishing a vicious cycle of symptomatic intake and relapsing head pain that critically worsens the overall clinical status of patients. A great deal of attention has been focused on MOH pathogenesis, and thanks to preclinical and clinical studies knowledge about this disorder is now remarkably advanced. Still, some open questions remain regarding issues relate

Medication overuse headache from antimigraine therapy: clinical features, pathogenesis and management.

Medication overuse headache (MOH) is being recognised more often in headache, neurology and primary care clinics, but is still frequently overlooked. The most significant factor in the development of MOH is the lack of widespread awareness and understanding on the part of clinicians and patients. While the diagnosis of MOH may be suspected clinically, it can only be confirmed in retrospect. Diagnosis may take > or =3 months because of the need for prolonged observation after cessation of medication. Diagnosis must be based on observation of patterns of headaches and medication use, remembering
