Reactions to, and Coping With, the Sequelae of Medical Errors That Lead to Disfigurement and Loss of Function.

The Psychiatric Consultation Service at Massachusetts General Hospital sees medical and surgical inpatients with comorbid psychiatric symptoms and conditions. During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. These discussions have given rise to rounds reports that will prove useful for clinicians practicing at the interface of medicine and psychiatry. Prim Care Companion CNS Disord 2026;28(5):26f0421

Communication and medical errors: a bibliometric analysis of research trends and knowledge structures.

BACKGROUND: Communication is increasingly positioned in patient safety research as a cross-cutting domain influencing multiple care processes. However, its structural organization within the scientific literature remains insufficiently integrated. OBJECTIVE: This study aims to conduct a large-scale bibliometric analysis of the scientific field linking communication and medical errors and to propose a conceptual communication governance framework integrating the identified communication domains into a coherent structure relevant to healthcare, including nursing practice. METHODS: We conducted

The problem with the existing reporting standards for adverse event and medical error research.

The Enhancing the Quality and Transparency of Health Research (EQUATOR) Network indexes over 600 reporting guidelines designed to improve the reproducibility of manuscripts across medical fields and study designs. Although several such reporting guidelines touch on adverse events that may occur in the context of a study, there is a large body of research whose primary focus is on adverse events, near-misses and medical errors that do not currently have a dedicated reporting guideline to help set reporting standards and facilitate comparisons across studies. As part of the process prescribed by

A preventable medical error: Successful management of iatrogenic Zenker's diverticulum perforation.

INTRODUCTION AND SIGNIFICANCE: Zenker's diverticulum is a rare condition characterized by a false diverticulum, as a true diverticulum involves herniation of all wall layers outward. Dysphagia, difficulty in swallowing, is the most common symptom. Diagnosis is primarily made through X-ray studies using contrast material during swallowing. Treatment options include open surgery or endoscopy. CASE PRESENTATION: We present the case of an adult male who experienced progressive dysphagia for approximately two years. A diagnosis of Zenker's diverticulum was confirmed by radiography following unsucc

Experimental evidence for structured information-sharing networks reducing medical errors.

Errors in clinical decision-making are disturbingly common. Recent studies have found that 10 to 15% of all clinical decisions regarding diagnoses and treatment are inaccurate. Here, we experimentally study the ability of structured information-sharing networks among clinicians to improve clinicians' diagnostic accuracy and treatment decisions. We use a pool of 2,941 practicing clinicians recruited from around the United States to conduct 84 independent group-level trials, ranging across seven different clinical vignettes for topics known to exhibit high rates of diagnostic or treatment error

Types of therapeutic errors in the management of osteoporosis made by physicians and medical students.

BACKGROUND: Clinical reasoning is of high importance in clinical practice and thus in medical education research. Regarding the clinical reasoning process, the focus has primarily been on diagnostic reasoning and diagnostic errors, but little research has been done on the subsequent management reasoning process, although the therapeutic decision-making process is at least equally important. The aim of this study was to investigate the frequency of therapeutic decision errors and the cognitive factors leading to these errors in the context of osteoporosis, as it is known to be frequently associ

Assessment of Physician Sleep and Wellness, Burnout, and Clinically Significant Medical Errors.

IMPORTANCE: Sleep-related impairment in physicians is an occupational hazard associated with long and sometimes unpredictable work hours and may contribute to burnout and self-reported clinically significant medical error. OBJECTIVE: To assess the associations between sleep-related impairment and occupational wellness indicators in physicians practicing at academic-affiliated medical centers and the association of sleep-related impairment with self-reported clinically significant medical errors, before and after adjusting for burnout. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional s

Could no-fault compensation for medical errors improve care and reduce costs?

The costs of litigation are large and increasing, to a level that places a drain on precious health-care resources and affects the way medicine is practised. This article examines whether a change to a no-fault legal system would lead to reduced costs and improved patient care.

Medication error in an extremely low birth weight infant: paracetamol overdose.

Errors are part of human nature and are usually present in our actions. Medical errors occur quite often and can be serious. Medication errors are among the most frequent, especially in newborn infants because of the multiple steps that occur during the process of prescribing and administering drugs and because most drugs are not licensed for being used in newborn infants (off-label). The aim of this report is to describe a medication error in prescribing paracetamol for closing a patent ductus arteriosus in a preterm infant and to analyze its causes. A preterm female infant born at 27 weeks o

The Effect of Standard Concentration Infusions on Medication Errors in Neonatal and Pediatric Healthcare Settings: A Systematic Review.

Background and Objectives: Neonates and children are at high risk of medication errors (ME) with intravenous (IV) infusion therapies, particularly when strategies such as the "rule of six" with individualized, weight-based concentrations are used. Standard concentrations (SCs) have been proposed to reduce variability, improve safety, and facilitate the use of electronic prescribing and smart infusion technologies. The impact of SCs for continuous IV infusions on ME rates in neonatal and pediatric settings was systematically evaluated. Methods: A systematic review was conducted in MEDLINE, Emba

Prevalence of medication administration errors and its determinants among nurses in Ethiopia: a systematic review and meta-analysis.

BACKGROUND: Despite nurses being the backbone of patient care, medication administration errors (MAEs) remain a serious risk to patient safety in low-income countries, including Ethiopia. However, the previous review was outdated, included fewer than 10 studies, focused solely on tertiary hospitals, and did not pool determinants. As a result, this meta-analysis aimed to determine the pooled prevalence of MAEs and their determinants among nurses in Ethiopia, addressing gaps in the study setting, time, and outcomes. METHODS: The study protocol was registered in PROSPERO. Observational studies c

Off-label drug use and the risk of medication errors in critically ill neonates: A conceptual pilot study.

BACKGROUND: Off-label drug (OLD) use is common in neonates. OBJECTIVE: There is a dearth of information associating the OLD use and the risk of medication errors in critically ill neonates. Hence, the present study was carried out. METHODS: Drug prescriptions in neonates admitted to the intensive care unit of a tertiary care hospital between September 2018 and June 2019 were evaluated. Details on their demographics, reason for admission in intensive care unit, drug-related information and serum creatinine were extracted. United States Food and Drug Administration approved drug labels were co

Patient-reported outcomes and medication errors during oral antitumour therapy in paediatrics: Results from the youngAMBORA care programme.

AIM: Increasing use of oral antitumour therapeutics (OAT) in paediatrics is associated with numerous advantages, but also with error-prone aspects. Based on the randomized AMBORA trial including adults treated with various OAT, we developed a tailored pharmacological/pharmaceutical care programme for children and their caregivers during OAT treatment (youngAMBORA). We now aimed to analyse the impact of the youngAMBORA care programme on medication safety by assessing (1) patient-reported outcomes (PROs) and (2) medication errors. METHODS: Depending on the intended OAT treatment duration, the y

Medication Errors and Blood Pressure Control Among Patients Managed for Hypertension in Public Ambulatory Care Clinics in Botswana.

Background The prevalence of hypertension in low- and middle-income countries is rapidly increasing, with most cases undiagnosed and many poorly controlled among those diagnosed. Medication reconciliation studies from high-income countries have demonstrated a high occurrence of antihypertensive medication errors and a strong association between medication errors and inadequate blood pressure control, but data from low- and middle-income countries are lacking. Methods and Results We conducted a cross-sectional study from April to October 2018 of adult patients on pharmacologic management for kn

A Case of Inappropriate Self-Medication Compounded with Prescribing vis-à-vis Dispensing Errors and the Hazardous Consequence.

INTRODUCTION: Self-medication behavior appears to be a commonplace; and when it is ignorant it may prove dangerous. On the other hand, dispensing errors and consequent adverse outcomes, though not too uncommon, are seldom reported. We report here a case of methotrexateinduced acute vesico-bullous eruptions in a patient of psoriasis who indulged in self-medication and was wrongfully dispensed higher doses of methotrexate. CASE DESCRIPTION: A 50-year-old man was diagnosed with psoriasis two years back and advised tablet methotrexate 20 mg once weekly and folic acid supplementation. He experienc

A comprehensive study of prescribing, administering and drug handling medication errors in ten wards of a university hospital after implementation of electronic prescribing, clinical pharmacists or medication reconciliation.

Background and aim: Medication errors lead to preventable risks. Preventing strategies such as e-prescribing, clinical pharmacists and medication reconciliation have been implemented in recent years. However, information on long-term medication error rates in routine procedures is missing. Investigations: We aimed to identify predefined medication errors in ten wards of a university hospital where e-prescribing, clinical pharmacists and medication reconciliation have been partially implemented. Patient files were reviewed and routine processes were monitored for drug prescription errors (missi
