A 'Natural' thyroid storm!

WHAT IS KNOWN AND OBJECTIVE: Over the counter supplements are often taken for granted during medication reconciliation in the emergency department. Supplements are not regulated by FDA, and some can be potentially dangerous. CASE SUMMARY: We report a case of thyrotoxicosis secondary to over the counter bovine thyroid supplements. Our patient presented with atrial fibrillation with rapid ventricular response refractory to calcium channel blockers. Had we not known about the supplement, the course of treatment would have been different with potential adverse outcome. WHAT IS NEW AND CONCLUSION

The incidence of medication errors on discharge prescriptions following pharmacist discharge prescription planning (PDPP) in an orthopaedic surgery unit: a parallel group-controlled study.

BACKGROUND: Pharmacist Discharge Prescription Planning (PDPP) supports safe and effective transition of patients from hospital to home or other care settings. It involves a pharmacist reviewing and reconciling the patient's medications and planning an accurate discharge prescription. Orthopaedic patients are particularly susceptible to medication-related errors at discharge due to several factors including polypharmacy and frequent prescribing of high-risk medications. Previous PDPP studies have not focused on orthopaedic patients exclusively. AIM/OBJECTIVE: To assess the impact of PDPP on med

Medication-related problems in a nephrology outpatient clinic: the role of polypharmacy and computerized prescribing system performance in a resource-limited setting.

BACKGROUND: Chronic kidney disease (CKD) patients in resource-limited settings face high risks of medication-related problems due to polypharmacy and suboptimal clinical decision support systems. Despite the implementation of Computerized Physician Order Entry systems, errors occur and their patterns and system gaps remain understudied in outpatient nephrology clinics. METHODS: This cross-sectional study analyzed 552 CKD patients attending a tertiary nephrology clinic in Egypt (January-July 2024). We conducted standardized medication reviews using National Coordinating Council for Medication

Interventions to optimise prescribing for older people in care homes.

BACKGROUND: There is a substantial body of evidence that prescribing for care home residents is suboptimal and requires improvement. Consequently, there is a need to identify effective interventions to optimise prescribing and resident outcomes in this context. This is an update of a previously published review (Alldred 2013). OBJECTIVES: The objective of the review was to determine the effect of interventions to optimise overall prescribing for older people living in care homes. SEARCH METHODS: For this update, we searched the Cochrane Central Register of Controlled Trials (CENTRAL) (includ

New and Persistent Sedative Prescriptions Among Older Adults Following a Critical Illness: A Population-Based Cohort Study.

BACKGROUND: ICU survivors often have complex care needs and can experience insufficient medication reconciliation and polypharmacy. It is unknown which ICU survivors are at risk of new sedative use posthospitalization. RESEARCH QUESTION: For sedative-naive, older adult ICU survivors, how common is receipt of new and persistent sedative prescriptions, and what factors are associated with receipt? STUDY DESIGN AND METHODS: This population-based cohort study included ICU survivors aged ≥ 66 years who had not filled sedative prescriptions within ≤ 6 months before hospitalization (sedative-naive)

Predicting risk of adverse drug reactions in older adults.

Adverse drug reactions (ADRs) are common in older adults, with falls, orthostatic hypotension, delirium, renal failure, gastrointestinal and intracranial bleeding being amongst the most common clinical manifestations. ADR risk increases with age-related changes in pharmacokinetics and pharmacodynamics, increasing burden of comorbidity, polypharmacy, inappropriate prescribing and suboptimal monitoring of drugs. ADRs are a preventable cause of harm to patients and an unnecessary waste of healthcare resources. Several ADR risk tools exist but none has sufficient predictive value for clinical prac

Syndrome of Inappropriate Antidiuretic Hormone (SIADH) Induced by Long-Term Use of Citalopram and Short-Term Use of Naproxen.

BACKGROUND Use of selective serotonin reuptake inhibitors (SSRIs) has been reported to be associated with the syndrome of inappropriate antidiuretic hormone (SIADH), although it is uncommon. Nonsteroidal anti-inflammatory drugs (NSAIDs), as a sole agent, are an even rarer cause of SIADH. Despite being documented in the literature, the understanding of the mechanism of both agents is limited. Here, we report a case of a patient taking both of these medications, a dangerous combination that led to the development of SIADH. CASE REPORT An 88-year-old woman with a history of asymptomatic chronic h

What Do Patients with Long-Term Conditions Disclose About Medication Adherence During and After Hospital Discharge? A Qualitative Study at an Internal Medicines Ward in Norway.

PURPOSE: To explore how patients' life circumstances, perceptions, lived experiences and behaviors may influence medication adherence in the social context surrounding hospital discharge. PATIENTS AND METHODS: Ten patients aged 48-89 years were included at a hospital ward close to the presumed day of discharge, observed during healthcare personnel (HCP) encounters and interviewed 1-2 weeks after discharge. Medication adherence was reconstructed through medication reconciliation and by comparing observations of hospital encounters and discharge information with patients' post-discharge account

Identification of undocumented over-the-counter medications in an academic nephrology clinic.

OBJECTIVES: To explore how accurately over-the-counter (OTC) medications were documented in an academic nephrology clinic and the benefits of using a novel short questionnaire as part of medication reconciliation (MR). METHODS: We developed a 3-item tailored questionnaire with questions about use of aspirin, nonsteroidal anti-inflammatory drugs (NSAIDs), and proton pump inhibitors (PPIs), which clinical leadership identified as medications of interest. Over the course of 20 days, medical assistants administered the questionnaire to clinic patients immediately after the standard MR. We summari

Chronic Constipation in Adults.

Chronic constipation has significant quality-of-life implications. Modifiable risk factors include insufficient physical activity, depression, decreased caloric intake, and aggravating medication use. Chronic constipation is classified as primary (normal transit, slow transit, defecatory disorders, or a combination) or secondary (due to medications, chronic diseases, or anatomic abnormalities). Evaluation begins with a detailed history, medication reconciliation, and physical examination. Routine use of laboratory studies or imaging, including colonoscopy, is not recommended in the absence of

Pharmacists can help prevent lipoid pneumonia: Two case reports.

OBJECTIVES: To report 2 cases of lipoid pneumonia. SUMMARY: Lipoid pneumonia is an inflammatory process in the lower airways due to the presence of lipid molecules in the alveoli. Exogenous lipoid pneumonia is due to the inhalation or aspiration of fat-containing substances. Historically, mineral oil is the most common medication cause but there have also been several reports of lipoid pneumonia associated with petroleum jelly, medicated vapor rub, and lip glosses. Two case reports are presented to illustrate the importance of identifying risk factors for lipoid pneumonia. RESULTS: Use of th

Pharmacist Identification of Medication Therapy Problems Involving Cognition Among Older Adults Followed by a Home-Based Care Team.

BACKGROUND: Dementia, depression, and delirium alone or in combination (3Ds) can threaten independence among older adults, and polypharmacy may further accelerate decline. Clinical pharmacists can play an important role on multidisciplinary home-based care teams by identifying medication therapy problems (MTPs) involving cognition. Within a larger ongoing clinical trial, this paper describes cognition-related MTPs and pharmacist recommendations among older adults with 3Ds followed by a home-based care team. METHODS: We conducted a retrospective analysis of medication data among Medicare Advan

Metformin-Induced Lactic Acidosis (MILA): Review of current diagnostic paradigm.

A new diagnostic paradigm has been proposed to better categorize causes of Metformin-Associated Lactic Acidosis (MALA). The diagnostic criteria defines a link between Metformin and lactic acidosis if lactate is >5mmol/L, Ph<7.35 and Metformin assay >5mg/L. Metformin assays are not readily available in emergency departments including nationwide Veteran's Affairs Hospitals; thereby making this proposed classification tool difficult to use in today's clinical practice. We describe a case report of a 45-year-old male, who took twice the amount of Metformin prescribed and presented with Metformin-i

[Geriatric ophthalmologic co-management : A case report of interdisciplinary collaboration].

This case report details the assessment and interdisciplinary collaboration in the management of an 81-year-old patient presenting with acute visual impairment, dizziness, general weakness, gait disturbances and fear of falling. A holistic geriatric evaluation revealed orthostatic dysregulation and an underlying multifactorial gait disorder exacerbated by visual impairment. Ophthalmological findings included left central retinal artery branch occlusion and cataracts. A comprehensive geriatric assessment showed frailty, impaired mobility and decreased functional abilities. Subsequent patient-ce

Providing systematic detailed information on medication upon hospital discharge as an important step towards improved transitional care.

WHAT IS KNOWN AND OBJECTIVE: Good communication between hospital and primary care physicians (PCPs) is important for the continuity of patient care in the transition phase following hospital treatment. Drug-related problems and medication errors may occur at that interface. Discharge letters often lack a structured medication report at the end and therefore may not provide the reasons for medication changes, resulting in low adherence rates. The objectives were to develop a structured medication report as part of the discharge letter, to evaluate the impact of the medication report in ischaemi

From authoring to editing: a competency framework for trainee use of ambient AI clinical documentation.

Ambient AI has spread through clinical settings faster than curricula can adapt, shifting trainee documentation from authoring to editing notes. Authoring historically honed clinical reasoning for trainees, yet medical education is not prepared to develop the editing task now replacing it. We define this task as a teachable competency with four domains - error detection, clinical reconciliation, independent generation, and trust calibration - and describe how simulation can teach them.

Implementation of a Pharmacy-Led Psychotropic Stewardship Pilot Program at an Academic Medical Center.

BACKGROUND: Psychotropic stewardship programs (PSPs) have been proposed to optimize psychiatric medication use and improve medication safety; however, data describing their implementation and impact in general medical inpatient settings remain limited. This quality improvement project evaluated a pharmacist-led PSP pilot at an academic medical center. METHODS: Adult inpatients were identified using predefined criteria including use of high-risk psychotropic agents (carbamazepine, clozapine, lithium, lamotrigine, valproic acid [VPA]), scheduled benzodiazepines in patients 65 years and older, a

Exploring the role of pharmacists in outpatient dialysis centers: a qualitative study of nephrologist views.

BACKGROUND: Implementation of pharmacy services in dialysis centers seems to be limited and requires acceptance from nephrologists. The aim of this study was to explore the opinions of Australian and Portuguese nephrologists toward a potential future provision of clinical pharmacy services in outpatient dialysis centers. METHODS: A qualitative study using semistructured interviews was conducted with a purposeful sample of 7 Australian and 14 Portuguese nephrologists. The audiotaped interviews were transcribed verbatim and thematically analyzed. RESULTS: Three themes emerged from the analysis
