BACKGROUND: Patients in intensive care units frequently receive multiple intravenous medications, increasing the risk of medication errors, including incompatible Y-site drug co-administration. Drug incompatibility can lead to serious clinical consequences, yet evidence supporting the safety of many commonly co-administered drug combinations is limited. Clinical decision support systems may help reduce medication errors and improve patient care, but challenges remain in their design and adaptation to complex and dynamic clinical settings such as the intensive care unit. The Swedish Y-site compa

OBJECTIVE: The Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey evaluates whether patients understand the purpose and potential side effects of newly prescribed medications. These measures affect patient experience metrics and may influence Centers for Medicare & Medicaid Services (CMS) reimbursement. Prior studies have suggested pharmacist-led counseling may improve medication-related HCAHPS outcomes.
METHODS: This pre-post quality improvement initiative was conducted across 19 inpatient units at an 897-bed community hospital. Four units with historically lowe

Compliance checking for clinical pathways (CPs) is getting increasing attention in health-care organizations due to stricter requirements for cost control and treatment excellence. Many compliance measures have been proposed for treatment behavior inspection in CPs. However, most of them look at aggregated data seen from an external perspective, e.g. length of stay, cost, infection rate, etc., which may provide only a posterior impression of the overall conformance with the established CPs such that in-depth and in near real time checking on the compliance of the essential/critical treatment b

BACKGROUND: Patients are at significant risk of harm from unintended medication errors, which remain prevalent and often preventable in healthcare systems, especially in low- and middle-income countries. Medication reconciliation (MedRec) plays a critical role in minimizing these errors by ensuring an accurate and complete medication list throughout patient transitions in the healthcare system. AIM: This study aimed to assess the current status of the MedRec process in Egypt by exploring the knowledge, attitudes, and practices of physicians and pharmacists as key stakeholders in its implementa

As part of its plan to build a regional integrated healthcare delivery network, Genesys Health System, Flint, MI, has been developing a computerized patient record (CPR). The CPR will give users throughout the system immediate access to diagnostic studies, treatment information, discharge summaries, operative reports, x-rays, and other patient information. Before considering technical aspects of the system, healthcare executives considering implementation of a CPR should examine business and clinical issues to determine what they want to accomplish with the CPR. The Genesys information system

BACKGROUND: Point-of-care ultrasound (POCUS) use continues to expand across multiple clinical subspecialties, and the need for standardization of training and quality assurance has become increasingly important. Despite the need for training, there are currently no widely accepted multispecialty criteria to define an acceptable quality POCUS image for common POCUS applications used by clinicians. Without such criteria, discrepancies in rating POCUS image quality occur, leading to inconsistencies in training and quality assurance, which can ultimately compromise patient care and safety.

In recent years, thanks to the development of miniaturized ultrasound devices, comparable to personal computers, tablets and even to smart phones, we have seen an increasing use of bedside ultrasound in internal medicine departments as a novel kind of ultrasound stethoscope. The clinical ultrasound-assisted approach has proved to be particularly useful in assessing patients with nodules of the neck, dyspnoea, abdominal pain, and with limb edema. In several cases, it has allowed a simple, rapid and precise diagnosis. Since 2005, the Italian Society of Internal Medicine and its Ultrasound Study

The results of a national survey of pharmaceutical services in federal and nonfederal community hospitals conducted by ASHP during summer 1994 are reported and compared with the findings of earlier ASHP surveys. A simple random sample of community hospitals (federal and nonfederal) was selected from hospitals registered by the American Hospital Association. A questionnaire was mailed to each director of pharmacy. The adjusted gross sample size was 896. The net response rate was 44% (393 usable replies). The mean number of hours that respondents' pharmacies were open per week for inpatient serv
