Molecular mechanism of water and glycerol transport through hydrophobic selectivity filter in the aquaporin homolog of Trypanosoma brucei.

The protozoan parasite Trypanosoma brucei is implicated in deadly African sleeping sickness. Experimental studies show that T. brucei codes for three aquaporin homologs (TbAQP1 to TbAQP3). TbAQP2 has been established as the high affinity drug transporter of drugs pentamidine and melarsoprol. Mutation in TbAQP2 or its loss results in pentamidine-melarsoprol cross-resistance. TbAQP2 is also shown to transport water, glycerol and other solutes to respond to osmoregulation in the infected hosts or glycerol metabolism. Experimentally determined structures of TbAQP2 show that it adopts the same aqua

Trypanosomiasis in a rural hospital in Tanzania. A retrospective study of its management and the results of treatment.

A retrospective analysis was done of the management and the results of treatment in 158 trypanosomiasis patients seen in a rural hospital in Tanzania during 1985. The distribution of cases in the population and during the year reflected an endemic situation. 109 patients (68.9%) were in the meningoencephalitic (ME) stage and 49 (31.1%) in the hematolymphatic (HL) stage of the disease. In total 19 patients (12.0%) died, 17 with ME and 2 with HL trypanosomiasis. Encephalopathy was seen in 19 (17.9%) of 106 patients treated with melarsoprol, and 10 of them died. In 15 (24.4%) of 64 patients who w

[Human African trypanosomiasis, contributions of experimental models].

Melarsoprol has remained the chosen drug for the late-stage treatment of human African trypanosomiasis (HAT) due both to Trypanosoma brucei (T.b.) gambiense and T.b. rhodesiense; however, arsenical encephalopathies, which are often fatal, occur in 5-10% of the treated cases. To date, two major problems have not been solved. The first one is the precise diagnosis of early involvement of the central nervous system (CNS) which determines the therapeutics to be administered. The second one is linked to the lack of data on in vivo efficacy of products which are effective in vitro against trypanosom

Luminescent multiplex viability assay for Trypanosoma brucei gambiense.

BACKGROUND: New compounds for the treatment of human African trypanosomiasis (HAT) are urgently required. Trypanosoma brucei (T.b.) gambiense is the leading cause of HAT, yet T.b. gambiense is often not the prime target organism in drug discovery. This may be attributed to the difficulties in handling this subspecies and the lack of an efficient viability assay to monitor drug efficacy. METHODS: In this study, a T.b. gambiense strain, recently isolated in the D.R. Congo, was made bioluminescent by transfection with Renilla luciferase (RLuc) without altering its in vitro and in vivo growth cha

Transporters in African trypanosomes: role in drug action and resistance.

Sleeping sickness is an increasing problem in many parts of sub-Saharan Africa. The problems are compounded by the lack of new medication, and the increasing resistance against traditional drugs such as melarsoprol, berenil and isometamidium. Over the last few years, much progress has been made in understanding how drug action, and the development of resistance, is related to the mechanisms by which the parasite ingests the drugs. In some cases novel transporters have been identified. In other cases, transporters do not appear to be involved in drug uptake, and selectivity must lie with other

Trypanocidal activity of dicationic compounds related to pentamidine.

Eight dicationic compounds related to pentamidine were studied for trypanocidal activity in seven trypanosome isolates. In vitro studies revealed that diamidines are more potent than diimidazolines. For example, 2 (a diamidine) and 4 (a diimidazoline) inhibited the growth of KETRI 243 with IC50 values of 2.3 and 900 nM, respectively. Introduction of polar groups into the linker decreased the effectiveness of the compounds against drug-resistant trypanosomes. In compounds with a 2-butene linker between the cationic groups, trans-isomers were more potent than cis-isomers. The cis- and trans-bute

Rhodesian trypanosomiasis in a splenectomized patient.

We report the first apparent case of a splenectomized individual who developed severe trypanosomiasis with central nervous system involvement. The patient was a 41-year-old man who participated in an east African safari. Upon his return to the United States, the patient presented with an infection with Trypanosoma brucei rhodesiense that was treated successfully with suramin and melarsoprol. The onset of symptoms, laboratory studies, and disease progression did not differ from previously reported cases in the literature. The role of the spleen in trypanosomiasis is not well understood and the

[The return of sleeping sickness in an epidemic form: international action for drugs].

In Central Africa, the number of people suffering and dying from sleeping sickness is the same as it was in 1920, whereas the disease was under control in the 1950s. In Zaire (Congo), Gambiense sleeping sickness was virtually under control by 1960. Due to war, chaos, population movements, lack of resources, and collapse of public health, the situation is now as it was 80 years ago and some years ago it was even more bleak as the supply of drugs seemed to halt. Due especially to the action of Médecins sans Frontières, the availability of the four essential drugs pentamidine, suramine, melarsopr

High-performance liquid chromatographic method for the separation and quantitative estimation of anti-parasitic melaminophenyl arsenical compounds.

A sensitive high-performance liquid chromatographic method has been developed for the separation and quantitative estimation of melaminophenyl arsenical drugs used in the treatment of human and veterinary trypanosomiases. Five clinically relevant compounds (melarsoprol, melarsonyl potassium, cymelarsan, melarsen oxide, and sodium melarsen) can be separated with an octadecylsilane reverse-phase column and detected down to 10 pmol per injection. The chromatographic system utilizes a propanol gradient in water, with lithium camphor sulphonate as ion modifier, and detection by ultraviolet absorban

Efficacy of combinations of difluoromethylornithine and bleomycin in a mouse model of central nervous system African trypanosomiasis.

DL-alpha-Difluoromethylornithine, a polyamine biosynthesis inhibitor, and bleomycin, a currently used antineoplastic agent, have each previously been shown to be curative for acute short-term infections of mice with Trypanosoma brucei brucei, an African trypanosome closely related to those that cause the human disease African sleeping sickness. These agents were tested singly and in combination in a previously described mouse model of sleeping sickness with demonstrable brain involvement. The original model is extended by using two additional strains of outbred mice and by demonstrating that m

[Cutaneous lesions in human African trypanosomiasis].

We report the case histories of 12 patients infected by Trypanosoma gambiense seen between 1967 and 1979. The course of the disease follows two stages, with the lymphatic and hematologic stage preceding invasion of the central nervous system. The following cutaneous lesions were recorded: 5 patients had trypanomas or chancres which are inflammatory nodules on uncovered areas; 5 patients showed trypanosomal skin rashes with erythematous, infiltrated, transient and recurring trypanids; 5 patients had soft transient edema, affecting mainly the face; 3 patients experienced pruritus, which was usua

An evaluation of prednisolone as a routine adjunct to the treatment of T. rhodesiense.

The purpose of this exercise was to evaluate the efficacy of a cortico-steroid in protecting the patient suffering from Rhodesian sleeping sickness from the complications of melarsoprol treatment. It is quite clearly shown that prednisolone was not effective in offering protection against the most serious treatment problem, encephalopathy. The apparent total success of the steroid in protecting the first group from the less severe treatment complications and in minimising early death need to be interpreted with some scepticism due to the smallness of the series. Nevertheless, it does help to s

Current chemotherapy of human African trypanosomiasis.

Human African trypanosomiasis is a fatal disease caused by Trypanosoma brucei gambiense and Trypanosoma brucei rhodesiense that has re-emerged in recent years. However, very little progress has been made in the development of new drugs against this disease. Most drugs still in use were developed one or more decades ago, and are generally toxic and of limited effectiveness. The most recently introduced compound, eflornithine, is only useful against sleeping sickness caused by T. b. gambiense, and is prohibitively expensive for the African developing countries. We present here an overview of tod

A unique transporter of S-adenosylmethionine in African trypanosomes.

African trypanosomes are pathogens of humans and livestock in equatorial Africa. There is a great deal of resistance to present front line drugs for treating African trypanosomiasis such as melarsoprol (Arsobal) and pentamidine. In the search for new and novel drugs against this disease, we have found a unique transporter of S-adenosylmethionine (AdoMet), a metabolite used in transmethylation reactions and polyamine synthesis. This transporter is distinct from those for methionine and adenosine, since AdoMet uptake was not inhibited by trypanocidal drugs, which compete with adenosine for trans

Evidence of blood cerebrospinal fluid barrier permeability impairment in Trypanosoma rhodesiense infection in vervet monkeys.

Blood-cerebrospinal fluid (CSF) barrier damage in 11 vervet monkeys was estimated before infection and during the early and late phases of Trypanosoma rhodesiense disease, using the method given by Tibbling, Link and Ohman (1977). Of the 11, only one monkey showed signs of barrier impairment that ranged from a slight (12.6) to total barrier impairment (285); the latter occurring just before the height of clinical encephalitis. The barrier reverted to normal after melarsoprol treatment at 1.8 mg/kg X 4. Between the two extremes of barrier impairment, there were periods of remission during which

The clinical manifestations of Rhodesian trypanosomiasis: an account of cases contracted in the Okavango swamps of Botswana.

Over 100 years ago, David Livingstone reported the presence of tsetse flies in the Okavango swamps in northern Botswana. They have persisted in the region and recently have been responsible for many cases of Rhodesian sleeping sickness caused by Trypanosoma rhodesiense in visitors to the area. The clinical manifestations in illustrative cases of this disease are described. One patient who refused treatment died five months after being infected. One patient died of encephalopathy complicating treatment with Melarsoprol (Mel B) and one died in a hemorrhagic state associated with a heavy parasite
