The ionic contribution of proteoglycans to mechanical stiffness of the meniscus.

Load transmission is an important function of the meniscus. In articular cartilage, proteoglycans help maintain hydration via negatively charged moieties. We aimed to investigate the influence of electrostatic effects on stiffness of meniscal tissue. Circular discs from bovine menisci of 8 mm diameter and 5 mm thickness were placed within a confined compression chamber. The apparatus was bathed in distilled water, 0.14 M PBS or 3 M PBS before being subjected to 5% ramp compressive strain and held for 300s. FEBio software was used to fit resultant relaxation curves to a non-linear poroviscoelas

Temporal Characteristics of Sodium Fluorescein in the Tear Meniscus.

PURPOSE: To observe the emission intensity profile of sodium fluorescein in the human tear film as a function of time and concentration. METHODS: Twenty-two participants with no dry eye signs or symptoms were randomly allocated to receive 1 μL of either a 2 or 10% concentration of fluorescein to one eye. Images of the inferior tear meniscus were captured at regular intervals over 30 minutes and the process repeated for the other eye with the alternate concentration. Fluorescence intensity was quantified on the basis of the grayscale pixel values in the tear meniscus images. The fluorescein-de

Arthroscopic treatment of symptomatic discoid meniscus in children.

PURPOSE: To study discoid meniscus in children who underwent arthroscopic surgery at our center. SUBJECTS AND METHODS: Thirty-four patients (39 knees) younger than 15 years with discoid lateral meniscus (mean age, 12.9 years; 13 males, 21 females) who underwent arthroscopic surgeries since 2007 were studied. The surgical procedure consisted of, in principle, saucerization with additional suture repair in the case of meniscal instability at the peripheral rim of the residual meniscus. The mechanisms of symptom onset, symptoms, complications and postoperative outcome in these cases were examine

Comparison of Medial and Lateral Meniscus Root Tears.

The meniscus root plays an essential role in maintaining the circumferential hoop tension and preventing meniscal displacement. Studies on meniscus root tears have investigated the relationship of osteoarthritis and an anterior cruciate ligament tear. However, few studies have directly compared the medial and lateral root tears. To assess the prevalence of meniscal extrusion and its relationship with clinical features in medial and lateral meniscus root tears, we performed a retrospective review of the magnetic resonance imaging (MRI) results of 42 knee patients who had meniscus posterior horn

Regeneration of a discoid meniscus after arthroscopic saucerization.

We report the case of a 5-year-old girl who presented to the clinic with recurrent knee pain after meniscal repair and saucerization at age 3. The finding of a regenerated discoid meniscus was confirmed arthroscopically and radiographically. To our knowledge, this is the first report in the literature to confirm regeneration of a discoid meniscus after saucerization. The case has significant implications for management of pediatric discoid meniscus tears and patient counseling, and is encouraging in terms of the potential for repair and regeneration of meniscal tissue, particularly in the very

Suturing complete radial tears of the lateral meniscus.

Lateral meniscus (LM) with complete radial tear (CRT) is common in patients with acute anterior cruciate ligament (ACL) injury. We have been approximating unstable LM CRT in an all-inside suture fashion. We report 3 cases of complete healing of such a torn meniscus on second-look arthroscopy. We recommend approximation of the displaced CRTs of the LM whenever possible because we expect improved healing with this approach. Surgically, the arthroscope is placed into the anteromedial portal and the suture hook into the anterolateral portal with the knee in figure-of-4 position. A suture hook load

Arthroscopic visualization of the posterior horn of the medial meniscus.

We present a method of visualizing the posterior horn of the medial meniscus through the anterolateral portal that has not been previously described in the literature. It allows easy visualization and instrumentation of the posterior horn, an area that commonly has pathology that can be difficult to identify and treat. The technique involves allowing the knee to flex to 60 degrees to 90 degrees over the side of the bed and applying a varus force to the tibia, opening up the posteromedial part of the joint. It does not require any valgus force and therefore minimizes the risk of injury to the m

Current and future directions for meniscus repair and replacement.

Torn meniscal cartilages and the consequences of missing meniscus tissue represent the major indications for operative arthroscopy of the knee. After recognizing the importance of the meniscus, clinicians made the shift away from complete meniscectomy to partial meniscectomy aided by the development of arthroscopic instrumentation. Despite extensive basic science data showing the importance of a complete meniscus for normal force transference, meniscus repair occurs in less than 10% of all meniscal tears found at arthroscopy. This paper will discuss the reasons for the common clinical approach

A surgical algorithm for treatment of cystic degeneration of the Meniscus.

Cystic degeneration of the meniscus is not uncommon. Recent reports suggest that most (or all) meniscal cysts are associated with a meniscal tear, and that the treatment of choice is arthroscopic partial meniscectomy with cyst decompression. This report describes a case of a symptomatic lateral meniscal cyst that was suspected clinically and confirmed by magnetic resonance imaging, but was not associated with a meniscal tear at arthroscopy. Open cyst extirpation and peripheral meniscal repair were performed to maximize preservation of meniscal tissue. A clinical algorithm is presented for mana

Technical considerations in the management of complex meniscus tears.

Meniscal repair continues to be an important procedure in patients with meniscal tears. The orthopedic literature has recommended excision of complex tears or tears that are located in the central avascular region. For the past 13 years, the authors have repaired all meniscal tears that can be reduced anatomically and that have healthy tissue that will support the multiple sutures required for a stable repair, regardless of tear pattern or location. Correct patient selection criteria continue to be important. In all cases, the risks of complications and retear must be considered carefully and

Cysts of the medial meniscus. Arthroscopic diagnosis and management.

We report the clinical and arthroscopic findings in 20 cases of medial meniscal cyst with a mean follow-up of 20 months. These were studied prospectively from a series of 7435 knee arthroscopies in which there were 1246 stable non-arthritic knees with medial meniscal tears. The diagnosis on referral was incorrect in seven, and incomplete in seven. There was coexistent meniscal injury in 17 (85%), but in the other three no tear was visible at arthroscopy. Ten knees had additional intra-articular abnormalities. Treatment of the cyst was by open resection in 12 and arthroscopic evaluation at meni

[Comparative study of arthroscopic and open meniscus surgery].

In this paper we compared two groups of 80 patients each with meniscal tears. One group was treated by Arthrotomie and partial meniscectomie and the other group by an arthroscopic procedure also with partial meniscectomie. All patients were seen at a follow up between six and 18 months postoperatively. According to the Gaudernak scale we found in both groups 98% of very good and good results. The duration of hospitalisation and sickleave was reduced by 50% after the arthroscopic operation, which showed a significant difference.

Blood Flow Restriction Training for Meniscus Repair Surgery.

BACKGROUND: Blood flow restriction (BFR) is a training tool that involves wearing a tourniquet or occlusive device during exercise. Data support that low-load training with BFR may produce muscle hypertrophy similar to standard high-load training. Because of the weight-bearing and range of motion (ROM) restrictions after meniscal repair, patients encounter substantial atrophy of lower extremity musculature. We perform BFR for these patients to limit atrophy postoperatively with the goal of facilitating their return to prior function and sports. INDICATIONS: We incorporate BFR in the postopera

Phenotypic characterization of regional human meniscus progenitor cells.

Stimulating meniscus regeneration using meniscal progenitor cells has been suggested as a promising new strategy. However, there is a lack of studies which decisively identify and characterize progenitor cell populations in human meniscus tissues. In this study, donor-matched progenitor cells were isolated via selective fibronectin adhesion from the avascular and vascular regions of the meniscus and chondroprogenitors from articular cartilage (n = 5). The mixed populations of cells from these regions were obtained by standard isolation techniques for comparison. The colony formation efficacy o

Medial meniscus posterior root tear: a comprehensive review.

Damage to the medial meniscus root, for example by a complete radial tear, destroys the ability of the knee to withstand hoop strain, resulting in contact pressure increases and kinematic alterations. For these reasons, several techniques have been developed to repair the medial meniscus posterior root tear (MMPRT), many of which have shown complete healing of the repaired MMPRT. However, efforts to standardize or optimize the treatment for MMPRT are much needed. When planning a surgical intervention for an MMPRT, strict surgical indications regarding the effect of pullout strength on the refi

Vascular safety during arthroscopic all-inside meniscus suture.

PURPOSE: The purpose of the study is to assess the orientation and distance of the popliteal artery (PA) from both the anteromedial and anterolateral portals. METHODS: The records of 97 patients (100 knees) who underwent knee arthroscopy were reviewed. The shortest distance from the posterior tibial cortex to the PA on the lines from both the medial and lateral borders of the patellar tendon to the PA was evaluated by magnetic resonance imaging at full knee extension. The figure-of-four position was compared between patients with intact and deficient anterior cruciate ligaments (ACLs). The sh

Radial displacement of lateral meniscus after partial meniscectomy.

Radial displacement or extrusion of the meniscus has been reported as a complication after meniscal transplantation and is sometimes observed in knees with advanced osteoarthritis. In this report, the case of a patient with radial displacement of the lateral meniscus after partial meniscectomy is presented. He had an incomplete discoid lateral meniscus with an anterior horn tear. The inner portion of the lateral meniscus was excised to leave a width of 8 to 10 mm. However, follow-up magnetic resonance imaging showed radial displacement of the mid-body of the lateral meniscus and a compatible f
