Betrayal trauma and adult mental health: The role of mentalizing and dissociation.

BACKGROUND: Trauma experienced across the lifespan has been linked to a wide range of adverse mental health outcomes. However, the psychological mechanisms connecting betrayal trauma to later psychopathology remain insufficiently understood. OBJECTIVE: This cross-sectional study investigated associations between betrayal trauma during childhood and adulthood and adult psychopathology-specifically depressive symptoms and level of personality functioning within a dimensional model of personality disorders. We tested a path model examining direct and indirect associations between betrayal trauma

Engagement of mental effort in response to mental fatigue: A psychophysiological analysis.

Acute mental fatigue, characterized by a transient decline in cognitive efficiency during or following prolonged cognitive tasks, can be managed through adaptive effort deployment. In response to mental fatigue, individuals can employ two main behavioral patterns: engaging a compensatory effort to limit performance decrements, or disengaging effort, leading to performance deterioration. This study investigated the behavioral pattern used by participants in mental fatigue conditions. Fifty participants underwent a sequential-task protocol with counterbalanced sessions who took place in two sepa

Predictors of Mental Health Recovery in Homeless Adults with Mental Illness.

For people with mental illness, experiences of homelessness can complicate mental health recovery processes. This study used longitudinal data from a randomized controlled trial of housing first (HF) to examine predictors of recovery among homeless people with mental illness. Findings showed that health and community predictors were most strongly associated with mental health recovery. Receipt of HF did not have any effect on changes in recovery scores at follow-up. Overall, the findings suggest that interventions aimed at preventing chronic homelessness, strengthening social networks and comm

Simple mental addition in children with and without mild mental retardation.

The speeded performance on simple mental addition problems of 6- and 7-year-old children with and without mild mental retardation is modeled from a person perspective and an item perspective. On the person side, it was found that a single cognitive dimension spanned the performance differences between the two ability groups. However, a discontinuity, or "jump," was observed in the performance of the normal ability group on the easier items. On the item side, the addition problems were almost perfectly ordered in difficulty according to their problem size. Differences in difficulty were explain

Behavioral synchronization of two individuals during cooperative interaction: the role of the mentalization ability as measured by the reading the mind in the eyes test.

BACKGROUND: Interpersonally synchronized behaviors play a fundamental role in social interactions. An important prereqvisite for behavioral synchronization is the mentalization ability of the individuals involved in the interaction. While the Reading the Mind in the Eyes Test (RMET) is widely applied to measure mentalization, most studies of RMET used single-subject designs, which lack ecological validity and have limited generalizability for real, on-going, reciprocal social interactions. Using a two-subject design, our objectives were to examine (i) the predictive validity of RMET for the sy

Impact of clinical severity on treatment response in a randomized controlled trial comparing day hospital and intensive outpatient mentalization-based treatment for borderline personality disorder.

This study examined the impact of clinical severity on treatment outcome in two programs that differ markedly in treatment intensity: day hospital mentalization-based treatment (MBT-DH) and intensive outpatient mentalization-based treatment (MBT-IOP) for borderline personality disorder (BPD). A multicenter randomized controlled trial was conducted. Participants include the full intention-to-treat sample of the original trial of N = 114 randomized BPD patients (MBT-DH n = 70, MBT-IOP n = 44), who were assessed at baseline and subsequently every 6 up to 36 months after start of treatment. Mentalization of complex emotions in borderline personality disorder: The impact of parenting and exposure to trauma on the performance in a novel cartoon-based task.

Borderline personality disorder (BPD) is characterized by a range of interpersonal difficulties, which are, in part, related to adverse experiences during childhood. Unresponsive parenting and traumatization may cause functional impairment of mentalization, i.e. the ability to reflect upon own and others' mental states. However, the relationship of poor parenting, trauma and mentalization in BPD has not exhaustively been studied. Thirty patients diagnosed with BPD and 30 matched control subjects were asked to sequence a novel cartoon-based mentalization task involving complex emotions such as

Day Hospital Mentalization-Based Treatment (MBT-DH) versus treatment as usual in the treatment of severe borderline personality disorder: protocol of a randomized controlled trial.

BACKGROUND: Severe borderline personality disorder is associated with a very high psychosocial and economic burden. Current treatment guidelines suggest that several manualized treatments, including day hospital Mentalization-Based Treatment (MBT-DH), are effective in these patients. However, only two randomized controlled trials have compared manualized MBT-DH with treatment as usual. Given the relative paucity of data supporting the efficacy and cost-effectiveness of MBT-DH, the possible influence of researcher allegiance in one of the trials, and potential problems with the generalization o

The Australasian Approach to the Definition of Mental Disorder in a Mental Health Act.

How should the mental element be defined in the legal standards governing a person's 'sectioning' or placement under the Mental Health Act (MHA)? This article considers how this mental element is defined in many MHAs in Australasia: via a statutory list of disorders of mental function said to 'characterise' the necessary state of mind. This article assesses the assumptions behind the adoption of this approach. It discusses the views of several English law reform committees that have explored how the mental element should be defined. It examines the philosophy of psychiatry, expounded clearly b

Promoting Mental Health and Preventing Mental Illness in General Practice.

This paper calls for the routine integration of mental health promotion and prevention into UK General Practice in order to reduce the burden of mental and physical disorders and the ensuing pressure on General Practice. The proposals & the resulting document (https://ethicscharity.files.wordpress.com/2015/09/rcgp_keymsg_150925_v5.pdf) arise from an expert 'Think Tank' convened by the London Journal of Primary Care, Educational Trust for Health Improvement through Cognitive Strategies (ETHICS Foundation) and the Royal College of General Practitioners. Adult neurogenesis, mental health, and mental illness: hope or hype?

Psychiatric and neurologic disorders take an enormous toll on society. Alleviating the devastating symptoms and consequences of neuropsychiatric disorders such as addiction, depression, epilepsy, and schizophrenia is a main force driving clinical and basic researchers alike. By elucidating these disease neuromechanisms, researchers hope to better define treatments and preventive therapies. Research suggests that regulation of adult hippocampal neurogenesis represents a promising approach to treating and perhaps preventing mental illness. Here we appraise the role of adult hippocampal neurogene

The mental health of children exposed to maternal mental illness and homelessness.

In recent years a number of reports have documented an increase in the number of homeless families in the US. Using a sample of 195 mothers who were veterans of the US armed forces we assessed the association of maternal homelessness and clinical status, with measures of children's mental health, school enrollment and attendance. Although maternal homelessness had no significant association with children's reported emotional problems it had a profound effect on school enrollment and attendance. Mothers' mental health status, history of incarceration and cumulative history of trauma, as well as

Cigarette smoking among patients with mental retardation and mental illness.

Among 136 adults with mental illness and mental retardation who were consecutively treated at a developmental disabilities clinic, 25 reported that they currently smoked cigarettes. Among those with mild and borderline retardation, smoking rates were 30 and 37 percent, respectively. Smokers were significantly more likely than nonsmokers to drink alcohol, use other drugs, and be sexually active. Multiple regression analysis found that a mild or borderline level of retardation and a diagnosis of schizophrenia were significant predictors of smoking. Mentally retarded persons with mental illness a

The effects of mentalization and sensitivity of postpartum depression mothers after the circle of security-intensive intervention on the social-emotional development of three-year-old children.

OBJECTIVE: Postpartum depression (PPD) significantly impacts mothers and their children, making relationship-building interventions necessary for prevention. The Circle of Security-Intensive (COS-I) is a specialized approach designed to enhance parental sensitivity and mentalization, with the objective of promoting the child's social-emotional development. This study is based on data from a previously conducted randomized controlled trial and aims to evaluate the effectiveness of COS-I in mothers with postpartum depression (PPD) exhibiting reflective functioning (RF), pre-mentalization (PM), a

Can addiction counselors be trained to deliver Mothering from the Inside Out, a mentalization-based parenting therapy, with fidelity? Results from a community-based randomized efficacy trial.

This study evaluated methods for training community-based clinicians to deliver a mentalization-based parenting intervention in an addiction treatment setting. Mothering from the Inside Out (MIO) targets psychological deficits associated with early stages of addiction recovery by fostering improvement in parental reflective functioning, the capacity to make sense of strong emotions in oneself and the child. Fifteen addiction counselors were randomized to training in MIO versus a Parent Education comparison, and completed eight training sessions and a clinically-supervised 12-session training c

Effects of Short-Term Versus Long-Term Mentalization-Based Therapy on Psychiatric Symptoms in Outpatients With Borderline Personality Disorder: Exploratory Results From a Randomized Clinical Trial.

We conducted a randomized clinical trial assessing the beneficial and harmful effects of short-term versus long-term mentalization-based therapy (MBT) for outpatients with borderline personality disorder (BPD) and found no evidence of a difference on BPD symptom severity, functional impairment, quality of life, global functioning, and severe self-harm. In this paper, we assessed the effects of short-term versus long-term MBT on the exploratory outcome, psychiatric symptoms, assessed with the Symptom Checklist-90-Revised (SCL-90-R). Mentalization-based treatment versus bona fide treatment for patients with borderline personality disorder in Germany (MAGNET): study protocol of a prospective, multi-centre randomized controlled trial.

BACKGROUND: Borderline Personality Disorder (BPD) is a serious mental disorder. Mentalization-based treatment (MBT) is an evidence-based treatment for individuals with BPD. Specifically, MBT has previously been highlighted for its effectiveness regarding the reduction of suicidal and non-suicidal self-injury (NSSI). Yet, randomized-controlled trials (RCT) on MBT in outpatient settings compared with bona fide treatment (BFT) are still scarce and none has been conducted in Germany. The primary objective of this RCT is to investigate whether outpatient MBT is more effective in the reduction of cr
