Energy vessel sealing systems versus mechanical ligature of the inferior mesenteric artery in laparoscopic sigmoidectomy.

PURPOSE: With the development of new devices, our ligation technique of the inferior mesenteric artery changed from mechanical ligature (ML) to energized vessel sealing systems (EVSS) ligature. The aim of this study was to determine if EVSS could be considered as safe and effective as the more convention ML of the inferior mesenteric vessels division during elective laparoscopic left colectomy. METHODS: Between 2001 and 2014, 200 consecutive patients (111 males) of mean age 54.1 years were operated laparoscopically for a symptomatic sigmoid diverticulitis. Vascular interruptions were performe

Robotic Low Ligation of the Inferior Mesenteric Artery for Rectal Cancer Using the Firefly Technique.

PURPOSE: By integrating intraoperative near infrared fluorescence imaging into a robotic system, surgeons can identify the vascular anatomy in real-time with the technical advantages of robotics that is useful for meticulous lymphovascular dissection. Herein, we report our initial experience of robotic low ligation of the inferior mesenteric artery (IMA) with real-time identification of the vascular system for rectal cancer using the Firefly technique. MATERIALS AND METHODS: The study group included 11 patients who underwent a robotic total mesorectal excision with preservation of the left co

Postpartum Uterine Bleeding from the Inferior Mesenteric Artery: Case Report and Review of the Literature.

Postpartum hemorrhage (PPH) is the leading cause of maternal mortality worldwide. The common causes of primary PPH include uterine atony, abnormal placentation, retained placenta, and birth canal injury. The inferior mesenteric artery (IMA) arises from the aorta and is the main artery to supply the descending and sigmoid colon, as well as the rectum. We present a case of immediate, severe uterine bleeding after a normal vaginal delivery; angiography determined that hemorrhage originated from the IMA, and there were no anastomoses between the IMA and other pelvic arteries. The patient was succe

Total bowel ischemia after carbon dioxide angiography in a patient with inferior mesenteric artery occlusion.

A 70-year-old man with a history of hypertension and chronic renal failure experienced claudication in his left leg. He was treated by endovascular stenting in his left leg. We used CO2 angiography to prevent contrast-induced nephropathy. After the procedure, livedo vasculitis and lactic acidosis developed. We diagnosed the patient with CO2-induced ischemic colitis, after which we changed his position and administered conservative therapy. Contrast-enhanced computed tomography showed total bowel ischemia and total occlusion at the ostium of the inferior mesenteric artery. This rare complicatio

[Esophageal replacement by interposition of pedicled ascending colon flap supplied by the inferior mesenteric artery].

OBJECTIVE: Colon interposition for esophageal replacement is indicated in patients with benign esophageal disease, in patients who require an esophago gastrectomy for a potential cure and in patients in whom the stomach is no longer available for replacement because of preceding surgery. METHODS: In 30 patients we performed colon interposition grafts for esophageal replacement using a modified technique. This technique includes ligation of the middle and right colic artery, thereby creating an interposition graft of the whole ascending colon which receives blood exclusively from the left coli

Distribution of lymph node metastasis and level of inferior mesenteric artery ligation in colorectal cancer.

To investigate the distribution of lymph node metastasis along the inferior mesenteric artery (IMA) and clarify whether high ligation of the IMA is important or not, we examined the surgical results of 172 patients with cancer of the sigmoid colon and rectum. Histologically, lymph node metastasis was absent in 108 (63%) patients and present in 64 (37%) patients. The distribution was adjacent to the wall of the rectum (35.5%) and sigmoid colon (10.5%), along the IMA (7.7%) and sigmoid colic artery (6.3%), and at the root of the IMA (0.7%, 1 of 135 patients). The presence of lymph node metastasi

High tie of the inferior mesenteric artery in distal colorectal resections--a safe vascular procedure.

Division of the inferior mesenteric artery flush with the aorta (high tie) allows a tension-free anastomosis in distal colorectal resections but may also diminish the blood supply. Tissue oxygen tension was measured proximal to the resection margin before and after either low or high division of the inferior mesenteric artery in 62 patients undergoing elective colorectal resections. Oxygenation was maintained or improved when the transverse (median change after vs before resection for low tie +9 mmHg (P < 0.05), high tie +8 mmHg (P = 0.3)) and descending colon (low tie +7 mmHg (p < 0.01), high

A randomized clinical trial comparing the initial vascular approach to the inferior mesenteric vein versus the inferior mesenteric artery in laparoscopic surgery of rectal cancer and sigmoid colon cancer.

BACKGROUND: The inferior mesenteric artery approach with a selective lateral splenic flexure mobilization is the most widely used initial step in laparoscopic rectal and left colon surgery. Surgery started through the inferior mesenteric vein (IMV) with systematic medial mobilization (MM) has some theoretical advantages that have never been analyzed in a clinical trial. The aim of this study was to compare the two techniques with regards to surgical technique variables (conversion, surgical time, bleeding, morbidity, and mortality) and pathological outcomes. METHODS: A single-blinded, randomi

Computer-assisted rescue of the inferior mesenteric artery in a child with a giant ganglioneuroblastoma: A case report.

BACKGROUND: Ganglioneuroblastoma (GNB) is a peripheral neuroblastoma (NB) with malignant degree between highly malignant NB and benign ganglioma (GN). Pathology is the gold standard of diagnosis. Although GNB is not uncommon in children, biopsy alone may lead to an inaccurate diagnosis, especially for giant tumors. However, surgical resection may be associated with significant complications. Here, we report a case of computer-assisted surgical resection of a giant GNB in a child and successful rescue of the inferior mesenteric artery. CASE SUMMARY: A 4-year-old girl was admitted to our depart

Ideal level of ligation of the inferior mesenteric artery. An old debate for a current surgical approach.

AIM: The ideal level of ligation of the inferior mesenteric artery (IMA) during resection for colorectal cancer is still controversial. The aim of this study was to demonstrate the real advantages and, above all, the adequacy of oncological staging after a low ligation of the IMA with additional LN retrieval in patients undergoing surgery for colorectal cancer. MATERIALS AND METHODS: Between January 2013 and December 2020, 157 patients who underwent curative resection of a primary colorectal tumor were retrospectively included: 64 patients underwent high ligation of the IMA and 93 patients un

Influence of prophylactic apical node dissection of the inferior mesenteric artery on prognosis of colorectal cancer.

BACKGROUND/AIMS: Middle ligation (ML) of the inferior mesenteric artery (IMA) maintains adequate blood supply to an anastomosis and has no risk of autonomic nerve injury. If apical node dissection of the IMA improves the prognosis, ML and prophylactic dissection of the apical node without division of the IMA above the colic artery may also result in an additional prognostic improvement in patients with sigmoid colon or rectal cancer. METHODOLOGY: Four hundred and one patients with either Dukes' B or Dukes' C colorectal cancer were clinicopathologically examined. In order to evaluate the influ

Does the level of inferior mesenteric artery ligation affect outcomes in rectal cancer: a meta-analysis.

BACKGROUND: The optimal level of inferior mesenteric artery ligation during rectal cancer surgery remains uncertain. High ligation may improve vascular length and apical nodal clearance, whereas low ligation preserves the left colic artery and may support colonic perfusion. METHODS: PubMed, Embase, Cochrane CENTRAL, and Web of Science were searched for comparative studies published from January 01, 2020 through March 31, 2026. Randomized and nonrandomized studies involving adults undergoing curative rectal cancer resection were eligible. Two reviewers independently selected studies and extrac

Feasibility of Central Lymphadenectomy During Low Ligation of Inferior Mesenteric Artery in Laparoscopic Colorectal Cancer Surgery.

INTRODUCTION: The optimal level of inferior mesenteric artery (IMA) ligation during colorectal cancer (CRC) surgery remains controversial. Low ligation with preservation of the left colic artery (LCA) may maintain bowel perfusion, but concerns persist regarding adequacy of central lymph node dissection. AIM: To evaluate the technical feasibility and short-term safety of central lymphadenectomy at the IMA root during low ligation with preservation of the LCA in laparoscopic CRC surgery. METHODS: This prospective observational study included 30 patients with nonmetastatic sigmoid colon or rect
