A problem-based approach in musculoskeletal ultrasonography: central metatarsalgia.

Ultrasonography (US) is a useful diagnostic method that can be easily applied to identify the cause of metatarsalgia. The superficial location of structures in the foot, dynamic capability of US, and the ability to perform direct real-time evaluations of the pain site are also strong advantages of US as a modality for examining the foot. Moreover, knowing the possible pain sources to investigate when a patient has a specific site of pain will enhance the diagnostic quality of US, and will help radiologists to perform US efficiently and effectively. The purpose of this article is to review the

Metatarsal osteotomy for primary metatarsalgia: radiographic and pedobarographic study.

The effect of surgical procedures for primary metatarsalgia on the load distribution and bony architecture of the forefoot is poorly understood. A prospective study was performed on 45 feet with this diagnosis treated by dorsal wedge osteotomy and compared with 29 symptom-free contralateral feet. Each foot was evaluated preoperatively and postoperatively with quantitative radiographic analysis, a pedobarographic study, and by physical examination. Following osteotomy there was a 4.5-mm increase in average height from ground and a 7.0 psi decrease in pressure. The symptom-free control group dem

[Investigation and treatment of metatarsalgia due to avascular osteonecrosis of a sesamoid].

The Renander disease or avascular osteonecrosis of one (or two) sesamoid of the first metatarsal is a rare and poorly studied pathological entity. It is part of the differential diagnosis of metatarsalgia, a more common complaint in clinical practice. We report the case of a 14-year-old patient presenting with pain on the plantar area of the left first metatarsal, aggravated while fencing. Clinical examination and MRI suggested avascular osteonecrosis of the medial sesamoid, for which conservative treatment resolved the complaints. The presentation of this clinical case gives the opportunity t

Radiological approach to metatarsalgia in current practice: an educational review.

Metatarsalgia, characterized by forefoot pain, is frequent and is primarily due to foot static disorders. Initial evaluation with weight-bearing radiographs is essential, allowing precise analysis of the architecture of the foot. Ultrasound is useful for soft tissue and tendon examination and provides the best clinical correlation. Computed Tomography provides detailed bone assessment and is helpful for pre-operative planning. Magnetic Resonance Imaging is the gold standard modality, offering superior soft tissue contrast. The common causes of metatarsalgia include hallux pathologies (hallux v

The Effect of Metatarsal Padding on Pain and Functional Ability in Metatarsalgia.

BACKGROUND AND AIMS: Many kinds of insoles and pads are commonly used as a conservative treatment of metatarsalgia. However, earlier studies of insole treatment provide contradictory results, and the natural progression of metatarsalgia is still unknown. The aims of this study were to (1) determine whether simple custom-made metatarsal pad insoles reduce pain and improve functional ability, (2) find out patients' satisfaction with padding treatment, and (3) investigate predisposing factors for metatarsalgia. MATERIAL AND METHODS: All metatarsalgia patients provided with metatarsal pad insoles

Metatarsalgia, lesser toe deformities, and associated disorders of the forefoot.

Forefoot pain in the adult often alters mobility and has a negative impact on quality of life. Metatarsalgia describes pain localized to the forefoot. Forefoot pain may be caused by conditions of the lesser toes themselves (eg, hammertoes, mallet toes, claw toes). The pathophysiology of lesser toe deformities is complex and is affected by the function of intrinsic and extrinsic muscle units. In addition to lesser toe and metatarsal abnormality, forefoot pain can be attributed to interdigital neuritis, disorders of the plantar skin, and gastrocsoleus contracture. Treatment of these conditions m

The use of collagen injections in the treatment of metatarsalgia: a case report.

Metatarsalgia is an increasingly common condition seen in the outpatient setting. A number of conservative and surgical management options are available for this condition. We present an interesting case that was unsuccessfully treated with'dermal filler.' To our knowledge, we have not found any published material in peer-reviewed journals documenting the use of collagen injections in the treatment of metatarsalgia. The authors feel that the lesson learned from this case should be highlighted for other foot and ankle surgeons.

Forefoot complaints--the Morton's metatarsalgia. The role of MR imaging.

An illustrated case with Morton's metatarsalgia is presented. MR imaging was helpful for determining the presence, location and magnitude of intermetatarsal neuroma. Moreover, it had a large influence on the differential diagnostic thinking because many disorders may produce forefoot complaints mimicking Morton's metatarsalgia.

Optimum position of metatarsal pad in metatarsalgia for pressure relief.

OBJECTIVES: To determine the optimum position of a metatarsal (MT) pad to treat metatarsalgia. DESIGN: We used a sensing mat with 16 x 16 sensors, 4.4 x 4.4 mm each, to measure plantar pressure in ten metatarsalgia patients walking with an MT pad in various positions. Peak pressures of the MT head and MT pad were analyzed. RESULTS: Walking without MT pads, the peak pressure of the MT head was 678 +/- 227 and 687 +/- 228 kPa in one proximal and one distal row of three sensors. Placement of the MT pad resulting in peak pressure two sensors proximal to the peak pressure of the MT head did not s

[Sagittal load-bearing tomography of the forefoot and static metatarsalgia].

We describe the use of weight-bearing foot tomograms. The outline of the food is drawn so as the axes of the metatarsal beyond the toes on the AP plain radiogram and the tomograms are done successively on each metatarsal. This morphological, but also functional, analysis enables exact diagnosis of excessive loading on the metatarsal head. This procedure is used in case of metatarsalgia with normal metatarsal length and in late post-traumatic sequellae of the forefoot. Surgical correction can then be precisely performed.

Surgical Treatment of Metatarsalgia and Severe Instability of Lesser Metatarsophalangeal Joints.

The main object of this prospective cohort study was to compare surgical treatment options for primary metatarsalgia and the severe instability of lesser metatarsophalangeal joints. The outcomes of triple Weil osteotomy combined with direct plantar plate repair and triple Weil osteotomy, performed with proximal interphalangeal joint arthrodesis, are analyzed and compared. One hundred thirteen patients (117 feet) were enrolled in the study. They were split into 2 groups. In the first group, undergoing Weil osteotomy, combined with the plantar plate repair, good results, including complete pain

[Short-term results of Weils osteotomy in patients with propulsive metatarsalgia].

INTRODUCTION: Metatarsalgia is the most common reason for consultation in orthopedic pathology of the foot. The key to its treatment lies in the correct determination of the symptoms, which are biological, morphological and mechanical alterations responsible for pain. Weil´s osteotomy is commonly used for multiple forefoot pathologies, including metatarsalgias. MATERIAL AND METHODS: We included patients with first-line failure syndrome and associated mechanical metatarsalgia-producing deformities. The minimum follow-up was six months. All individuals were interviewed with a standardized quest
