Objective evaluations of transverse tarsometatarsal (TMT) hypermobility/instability are lacking. This study aims to radiographically explore the relationship between transverse TMT instability and metatarsus adductus (MA) in hallux valgus (HV). This study retrospectively analyzed 207 feet with varying degrees of HV, employing the distance between the first and second metatarsals (M1-2 distance) to assess transverse TMT instability of the first ray. Participants were categorized into MA and non-MA groups. It was found that the M1-2 distance significantly increased with the hallux valgus angle (

Autologous bone grafts are considered the gold standard grafting material for the treatment of nonunion, but in very large bone defects, traditional autograft alone is insufficient to induce repair. Recombinant human bone morphogenetic protein 2 (rhBMP-2) can stimulate bone regeneration and enhance the healing efficacy of bone grafts. The delivery of rhBMP-2 may even enable engineered synthetic scaffolds to be used in place of autologous bone grafts for the treatment of critical size defects, eliminating risks associated with autologous tissue harvest. We here demonstrate that an osteoinductiv

BACKGROUND: Hallux rigidus and metatarsus primus elevatus (MPE) are associated, but their causal relationship remains unknown. Several surgical approaches for treating hallux rigidus are available. We evaluated morphological characteristics of hallux rigidus with different grades to determine the optimal surgical approach. The amount of degenerative change in the metatarsophalangeal joint on the preoperative roentgenograms was graded on a scale of 1-3. We analyzed the morphology of hallux rigidus using X-ray image mapping developed by our team.
METHODS: This study involved weight-bearing, dor

BACKGROUND: Metatarsus adductus is a relatively common congenital foot deformity often unrecognized at birth. There is adduction of the metatarsals, supination of the subtalar joint, and plantar flexion of the first ray.
OBJECTIVE: The objective of this study was to utilize the most commonly used methods for assessing metatarsus adductus deformity in children and to identify the measurements with the least intra- and inter-observer variability.
MATERIALS AND METHODS: All consecutive patients between 2 and 9 years of age with metatarsus adductus by roentgenographic evaluation on weight-bearin

AIM: To assess inter-reader reliability of metatarsus adductus (MA) using the traditional method and Engel's angle (EA) on radiography and magnetic resonance imaging (MRI) and assess correlations with hallux valgus (HV).
METHODS AND MATERIALS: Ninety consecutive patients with radiographs and MRI of the foot were included. Two readers measured HV angle (HVA), traditional metatarsus adductus angle (MAA), and EA on radiographs and HVA and EA on MRI. Three- and two-way mixed model analyses were used for reader agreements. Ninety-five percent bootstrap confidence intervals were calculated. The lin

Metatarsus adductus is usually a morphologic feature of the clubfoot or occurs as an isolated defect. Such deformation causes shortening of the medial foot column and lengthening of the lateral. The purpose of the study is a retrospective evaluation of long-term therapeutic effects in a group of patients, who underwent closing wedge cuboid osteotomy and opening wedge medial cuneiform osteotomy. Surgery was performed in 19 persons, procedure was applied to 26 feet, of which 3 presented congenital metatarsus adductus and 23 presented recurrent clubfoot. During ostoetomy we performed additional c

INTRODUCTION: Aim of this retrospective study was to analyse the long term results after basal closing wedge osteotomy for correction of metatarsus primus varus and hallux valgus in the younger patient.
PATIENT AND METHODS: 49 patients (70 feet) were operated according to a basal closing wedge osteotomy from 1974 to 1985 at our institution. Age was under 40 years in all patients at the time of surgery. 34 patients (50 feet) were evaluated in respect to their clinical and 26 patients (37 feet) to their radiological outcome. The average age was 26 years (14-39 years). The follow-up was 12 to 22

The authors have reviewed 252 children with congenital metatarsus varus as distinct from club foot with an adequate follow up. They describe fully conservative treatment of the condition and conclude that true correction was obtained in about 80 p. 100 of cases by the age of 3 years and that another 16 p. 100 healed between the ages of 3 and 5 years. There are only about 4 p. 100 of cases which are complex and do not correct. The authors emphasize the fact that among 96 p. 100 of corrections the conservative treatment had no influence whatsoever. When the foot does not correct by the age of 5

BACKGROUND: Elevated first metatarsal, Metatarsus primus elevatus (MPE), has been a topic of controversy. Recent studies have supported a significantly elevated first metatarsal in hallux rigidus on weight-bearing radiographs (WBR). However, conventional radiographs have limitations for accurate measurement. Our objective was to comparatively assess MPE and other variables which can affect the spatial relationship of the forefoot in the HR group compared to controls using weight-bearing CT (WBCT).
METHODS: In this single-center, retrospective, case-control study, 25 patients (30 feet) with sy

Correction of a bunion deformity with the use of proximal first ray procedures has historically included additional distal soft tissue and bone work about the first metatarsophalangeal joint. We report the case of a young adult female who underwent isolated first tarsometatarsal fusion for correction of a bunion deformity. Moreover, we describe the published data related to this approach. This technique could have the potential to improve patient outcomes by avoiding postoperative arthrofibrosis of the first metatarsophalangeal joint.

BACKGROUND: Congenital hallux varus (CHV) is a rare form of hallux varus deformity, characterized by medial deviation of the first toe at the metatarsophalangeal joint. It may be primary or secondary and presents clinically with pain and asymmetry with footwear.
CASE SUMMARY: We documented a case of a 6-year-old girl with bilateral CHV, accompanied by adduction of the toes in the left foot. Clinical diagnosis was made by physical examination and X-ray imaging based on Bleck's classification. Conservative treatment did not show any noticeable improvement, so the child underwent corrective surg

BACKGROUND: Typical foot deformity patterns of patients with rheumatoid arthritis (RA) include hallux valgus, claw toes, splay foot, flat foot, and hindfoot valgus deformities. However, some patients present deformities that are different from a typical pattern, such as metatarsus primus elevatus, described as dorsal elevation of the first metatarsal in relation to the lesser metatarsals. We speculated that metatarsus primus elevatus might be associated with calcaneal inclination and hindfoot varus alignment; however, studies on the association of hindfoot alignment with metatarsus primus elev

Metatarsus adductus is a relatively common congenital foot deformity that is often unrecognized at birth. Thus, the adult foot with metatarsus adductus is prone to pathologic entities that have been theorized to result from lateral column overload. We present a descriptive study comparing plantar foot pressure distribution during gait in subjects with and without metatarsus adductus. A total of 65 subjects were recruited for the study: 28 subjects with and 37 subjects without metatarsus adductus. An EMED(®) pedobarograph was used to collect the data. The analysis of the peak pressure and press

The purpose of this study was to determine the incidence of the intermetatarsal facet on the lateral aspect of the base of the first metatarsal and its role in the medial angulation of the proximal metatarsal base as it relates to metatarsus primus varus. A total of 77 human first metatarsals were examined for the presence of the intermetatarsal facet. The intermetatarsal facet was seen on the lateral aspect of the first metatarsal base, superior to the tuberous attachment of the peroneus longus tendon. The obliquity of the first metatarsal base was measured on metatarsals with and without the

OBJECTIVE: To document a distinctive pattern of stress fractures in the lateral metatarsal bones of patients with metatarsus adductus foot deformity.
DESIGN AND PATIENTS: Conventional radiographs and available medical records were reviewed in 11 patients (6 women, 5 men; ages 25-61 years) with stress fractures of the lateral (fourth or fifth) metatarsal bones and metatarsus adductus. Evaluation included the number and location of fracture(s), forefoot adduction angle, and qualitative assessment of bone mineral density. Conditions that might predispose patients to metatarsal fractures, includi

Chronic inflammatory disease of bones and joints (e.g., rheumatoid arthritis, gout, etc.), but also acute bone injury and healing, or degenerative resorptive processes inducing osteoporosis, are associated with structural remodeling that ultimately have impact on function. For instance, bone stability is predominantly orchestrated by the structural arrangement of extracellular matrix fibrillar networks, i.e., collagen-I, -IV, elastin, and other proteins. These components may undergo distinct network density and orientation alterations that may be causative for decreased toughness, resilience a
