BACKGROUND: The relationship between nasal functions and middle ear surgery is still under debate. Nasal obstruction is considered as a negative prognostic factor in middle ear surgery. This is based on the theory that it may cause Eustachian tube dysfunction (ETD) by leading to reduced ventilation of the middle ear, as found in several patients with nasal septal deviation, chronic rhinitis and nasal polyps.
OBJECTIVES: To assess how the subjective feeling of nasal function, evaluated by a preoperative questionnaire, may be predictive of surgical outcome and/or risk of failure in middle ear s

The most common technique in sound restoration of the middle ear is prosthetic surgery. Hypoventilation of the middle ear may cause adhesive otitis or atelectasis resulting in a higher risk of prosthetic extrusion rate and recurrence of the underlying cholesteatoma. We report long-term results using an island of tragal cartilage as an autologous PORP in selected patients with poor middle ear ventilation. Retrospective chart reviews were performed for procedures involving 52 patients between year 2000 and 2009. All patients that underwent surgery using tragal cartilage interposed between the su

A combined tympanoplasty method, the mobile-bridge tympanoplasty, has been applied at the Clinic for Ear, Nose and Throat Diseases in Novi Sad since 1998. Mobile-bridge tympanoplasty is performed at our Clinic by applying Feldmann's procedure utilizing a microsurgical oscillating saw. It is a combination of closed and open techniques for surgical treatment of middle ear diseases. In addition to this technique, a method for reconstruction of the posterior bone wall of the external auditory canal was introduced. This procedure is applied in cases of damage or impairment of the bony ear canal wal

BACKGROUND: Eustachian tube dysfunction (ETD) is the inability of the Eustachian tube (ET) to adequately perform at least one of its functions: to protect the middle ear from sources of disease, to ventilate the middle ear, and to help drain secretions away from the middle ear. There are a number of treatment options for ETD, but there is little consensus about management.
OBJECTIVES: To determine the clinical effectiveness of interventions for adult ETD and to identify gaps in the evidence to inform future research.
DATA SOURCES: Twelve databases were searched up to October 2012 for publish

BACKGROUND: The prevalence of secretory otitis media is very high among the nonoperated cleft palate patients.
METHODS: Ninety-one cleft palate operations were performed on late presented cases in Sudan, Africa. The surgeries were performed according to the Veau-Wardill-Kilner push-back technique. A laptop connected pen-type endoscopic camera was used to evaluate the tympanic membrane and address middle ear disease in all patients simultaneously. The procedure was performed after removing wax from the external auditory canal. A small incision was made with a myringotomy knife through the laye

OBJECTIVES: To use multifrequency multicomponent tympanometry (MF-MCT) for evaluating the middle ear mechano-acoustics of children with a history of ventilation tube (VT) insertion and to determine the effects of tube insertion sequelae on middle ear function.
METHODS: A prospective case-control study in a tertiary referral center involving 26 children (51 ears) aged 3-15 years. The children had undergone VT insertion surgery at least one-year previously and had no other known ear diseases. In addition, 13 age-matched healthy controls (26 ears) with no prior history of ear disease were includ

INTRODUCTION: Adenotonsillar and middle ear diseases result in some of the most frequently performed operations in the pediatric population worldwide. The pathogen reservoir hypothesis (PRH) suggests that the adenoids act as a reservoir of bacteria which play a potential pathogenic role in otitis media. Evidence supporting this hypothesis is limited. This study sought to comprehensively determine and compare associations between the adenotonsillar and middle ear bacterial microbiota within individual patients via next-generation sequencing and microbial network analyses.
METHODS: Bacterial 16

The Eustachian tube (ET) it is a complex structure connecting the middle ear cavity with the nasopharynx. Its function is to maintain middle ear ventilation to facilitate transmission of sound from the tympanic membrane to the cochlea. It is located medial to the parapharyngeal space. The ET and tympanomastoid cavities form an anatomic and functional unit that cannot easily be divided, and it plays a fundamental role in the etiology of different clinical entities. However, the underlying pathophysiological mechanisms are not yet fully understood, and further research is needed to address the g

Tympanosclerosis is a pathological condition which results in deposits of dense hyaline tissue or even bone in the lamina propria of the middle ear mucous membrane. It is induced by chronic inflammation in the middle ear, and is likely a Type III immune complex disease. Clinical tympanosclerosis results in decreased sound conduction by fixing ossicles or the tympanic membrane, or by obstruction to ventilation of the middle ear or mastoid. Management is dictated by the location and extent of the tympanosclerotic involvement. Ossicular problems are met by removal of the tympanosclerosis and reco

INTRODUCTION: The aim of the study was to investigate whether Peptest, an immunoassay used to detect pepsin, could be used to diagnose extraesophageal reflux (EER) in children with chronic otitis media with effusion (OME). The results obtained using this fast, simple and non-expensive method were compared with the results of previous studies.
METHODS: Children 1-7 years old who had been diagnosed with OME and who were undergoing myringotomy with insertion of a ventilation tube were included in the prospective study. Middle ear fluid obtained during myringotomy was analyzed with Peptest to det

Eustachian tube dysfunction is a frequent pathology of middle-ear ventilation, leading to many more or less disabling otologic symptoms. Severity varies from simple otitis media with effusion to aggressive chronic otitis or cholesteatoma. First-line treatment is medical, but surgery can also be proposed. It consists in balloon dilation of the Eustachian tube, in order to increase ventilation and improve symptoms. It is performed in second-line in patients with Eustachian tube dysfunction confirmed by tubomanometry, after failure of local drug treatment. It can be performed under general or loc

A cohort of 946 children who were screened for otitis media with effusion (OME) by serial tympanometry from the age of 2 to 4 years was followed up by otomicroscopy at 7.5 to 8 years of age. At school age, signs of active middle ear disease were present in 2.2% of ears that had been free from OME at preschool age, whereas 12.7% of ears that had suffered from persistent OME and 17.8% of ears that had been treated by a ventilation tube showed signs of active middle ear disease. Otopathologic sequelae (atrophy, tympanosclerosis, atelectasis, and attic retraction) were present in 13.7%, 39.8%, and

Secretory otitis media is defined as a fluid in the middle ear without signs or symptoms of infection. As the aetiology and pathogenesis of the disease are unknown, and as it affects children aged from 3 to 12 years, treatment procedures proposed for management of secretory otitis media, are not uniform. Some authors [1, 4, 6] consider that functional or mechanical obstructions of the Eustachian tube could provoke secretory otitis. The purpose of the treatment is to remove exudate from the middle ear and appropriately ventilate it for a longer period. That could instantly normalize the hearing

OBJECTIVE: Adopted children with cleft lip and/or cleft palate form a diverse group of patients. Due to increased age at palatal repair, adopted children have a higher risk of velopharyngeal insuffiency and poor speech outcome. Delayed palate repair may also lead to longer lasting Eustachian tube dysfunction. Decreased function of the Eustachian tube causes otitis media with effusion and recurrent acute otitis media, which can lead to other middle ear problems and hearing loss.
METHODS: One-hundred-and-thirty-two adopted children treated by the Cleft palate team in Wilhelmina Children's Hospi
