BACKGROUND: Mirizzi syndrome is a rare complication of prolonged cholelithiasis, characterized by narrowing of the common hepatic duct due to mechanical compression and/or inflammation due to biliary calculus impacted in the infundibula of the gallbladder or in the cystic duct.
OBJECTIVES: To describe a series of eight consecutive patients with Mirizzi syndrome, at a single institution, submitted to surgical treatment and to comment on their aspects with emphasis on the diagnosis and treatment.
METHODS: Four women and four men, with a mean age of 61.6 years (42 to 82 years), presenting Miriz

Mirizzi syndrome is a rare complication of gallstone disease characterized by extrinsic compression of the common hepatic duct and is associated with an increased risk of gallbladder carcinoma. We report the case of an 83-year-old female who presented with altered mental status, cholestatic hepatic enzyme elevation, and obstructive jaundice. Computed tomography imaging demonstrated biliary ductal dilation with a proximal common bile duct stricture concerning for malignancy. Intraoperative findings during laparoscopic cholecystectomy confirmed Type I Mirizzi syndrome with suppurative cholecysti

BACKGROUND Mirizzi syndrome (MS) is relatively a rare condition; incidence rates may increment with age. It is characterized as an obstruction of the common hepatic duct (CHD) auxiliary to outward compression of an infected stone in the cystic duct. Carbohydrate antigen (CA) 19-9 is a tumor marker that is usually related to upper-gastrointestinal malignancies. However, a few case reports have shown high levels of CA19-9 in the absence of malignancy. In this case, we report a case of a patient with MS, elevated CA19-9, and radiological findings suggesting gallbladder cancer, which shows the cha

OBJECTIVE: To determine the diagnostic accuracy of combined magnetic resonance cholangiopancreatography (MRCP) and computed tomography (CT) for preoperative diagnosis of Mirizzi syndrome.
MATERIALS AND METHODS: Fifty-two patients with surgically proven Mirizzi syndrome (n = 13) and cholecystitis without evidence for Mirizzi syndrome (n = 39) underwent both MRCP using single-shot turbo spin echo and 3-dimensional turbo spin echo sequences and CT. Two blinded observers independently and retrospectively reviewed the combination of MRCP and CT images and CT images alone. Diagnostic accuracy for a

Hemobilia is an uncommon diagnosis and is often not suspected in the absence of recent hepatobiliary intervention or trauma. Hemobilia in the setting of cystic artery pseudoaneurysm secondary to type I Mirizzi syndrome is a rare occurrence. We report the case of a 61-year-old male who presented with epigastric pain and vomiting. Blood tests demonstrated hyperbilirubinemia with elevated inflammatory markers. Magnetic resonance cholangiopancreatography revealed type I Mirizzi syndrome in the presence of a 21 mm cystic duct stone. During endoscopic retrograde cholangiopancreatography, hemobilia w

Mirizzi syndrome is a rare chronic cholecystitis complication. However, the current consensus on managing this condition remains controversial, especially through laparoscopic surgery. This report describes the feasibility of treating type I Mirizzi syndrome with laparoscopic subtotal cholecystectomy and electrohydraulic lithotripsic gallstone removal. A 53-year-old woman presented with dark urine and right upper quadrant pain for 1 month. On examination, she was jaundiced. Blood tests showed highly elevated liver and biliary enzyme levels. Abdominal ultrasound showed a slightly dilated common

Mirizzi syndrome, known as extrinsic bile compression syndrome (EBCS), is difficult to diagnose without surgery. To the best of our knowledge, our case shows that air in the biliary duct may indicate Type V Mirizzi syndrome. A 48-year-old Chinese male, complaining of anorexia with weight loss and jaundice, showed decreased bilirubin for two months after being diagnosed with gallbladder stones. Upon re-examination of the CT, evidence of air in the biliary duct was discovered. Type V EBCS was finally diagnosed during surgery. The patient underwent cholecystectomy, cholecystocolic fistula excisio

INTRODUCTION: A 50-year-old Japanese man presented with obstructive jaundice. We performed endoscopic retrograde biliary drainage before biliary decompression. CT showed a thickened gallbladder wall with low-density areas and a 35-mm gallstone; the stone was impacted in the gallbladder neck and cystic duct. The patient was therefore diagnosed with Mirizzi syndrome (type II or III) and scheduled for laparoscopic treatment. We performed subtotal cholecystectomy and intraoperative choledochoscopy because we recognized a fistula between the gallbladder and common bile duct preoperatively.

The results of treating 29 patients with Mirizzi syndrome are analyzed: 17 patients underwent traditional surgery, 11 patients--laparoscopic procedures, and endoscopic sphincterotomy with nasobiliary drainage was performed in a type I patient. Laparoscopic operations were performed in patients with types I and II syndrome, patients with types III and IV had only open surgery. In 2 cases operations were complicated by iatrogenic injury to the hepaticocholedochus, in 4 cases--by residual bile duct stones. Pronounced inflammatory alterations and scarring of the hepaticocholedochus, III and IV typ

In 1948, Mirizzi described a syndrome characterized as the obstruction of the common hepatic duct by a stone located in the gallbladder's neck or in the cystic duct. We present a rare case of Mirizzi syndrome resulting from a fistula involving the cystic duct, the neck of the gallbladder, and the right hepatic duct. This finding was possible because the patient had a rare biliary anatomic variation: the insertion of the cystic duct straight in the right hepatic duct. The diagnosis was suggested by abdominal ultrasonography and confirmed by endoscopic retrograde cholangiopancreatography. The su

We report a case of an extrahepatic bile duct metastasis from a gallbladder cancer that mimicked Mirizzi's syndrome on cholangiography. A 67-yr-old woman was admitted to our hospital with a diagnosis of acute calculous cholecystitis. As obstructive jaundice developed after the admission, percutaneous transhepatic biliary drainage was performed to ameliorate the jaundice and to evaluate the biliary system. Tube cholangiography revealed bile duct obstruction at the hepatic hilus, and extrinsic compression of the lateral aspect of the common hepatic duct, with nonvisualization of the gallbladder.

Mirizzi syndrome (MS) is a syndrome that causes chronic destructive and fibrotic changes because of compression and inflammation in the main biliary tract. MS remains to be a serious problem due to its high morbidity. In this study, it is aimed to evaluate the diagnostic tools, risk factors and clinical output data we apply to our patients with MS in the light of the literature. We retrospectively analyzed the data of patients treated for MS in the last decade in our hospital, where an average of 1350 cholecystectomies are performed annually. Clinical, laboratory and imaging data obtained from

Mirizzi syndrome (MS) is a common bile duct (CBD) obstruction caused by extrinsic compression from an impacted stone in the cystic duct or infundibulum of the gallbladder. Patients affected by MS may present abdominal pain and jaundice. A 37-year-old male with neurologic residuals post-encephalitis arrived at the emergency department reporting abdominal pain, jaundice and fever. An ultrasound of the abdomen identified cholecystolithiasis with a dilated CBD. He did not undergo CT or MRI due to poor compliance and parents' disagreement. Eventually, they accepted to perform endoscopic retrograde

BACKGROUND: Laparoscopic treatment is a viable option for Mirizzi syndrome (MS) type I, but it is not recommended for MS type II (McSherry classification). We introduce laparoscopic transfistulous bile duct exploration (LTBDE) as a simplified standardized technique for MS type II.
METHODS: Eleven consecutive LTBDEs performed by a surgeon for MS type II were analyzed retrospectively, including three successful single-incision LTBDEs (SILTBDEs). Transfistulous stone removal followed by primary closure of gallbladder remnant and partial cholecystectomy was performed. An additional choledochotomy

Gallstones cause various problems besides simple biliary colic and choplecystitis. With chronicity of inflammation caused by gallstone obstruction of the cystic duct, the gallbladder may fuse to the extrahepatic biliary tree, causing Mirizzi syndrome, or fistulize into the intestinal tract, causing so-called gallstone ileus. Stones may pass out of the gallbladder and travel downstream through the common bile duct to obstruct the ampulla of Vater resulting in gallstone pancreatitis, or pass out of the gallbladder inadvertently during surgery, resulting in the syndromes associated with lost gall

The following is a case report of a cholecystocholedochal fistula (Mirizzi syndrome Type II) in a 61 year old woman. The use of a pedicled graft of gall-bladder remnant in the treatment of this condition is presented. If the condition is recognized early, bile duct injury can be prevented and the need for more complex techniques of bile duct repair or bypass may be averted.

A 37-year-old woman underwent laparoscopic bailout subtotal cholecystectomy at a county hospital on December 20, 2025, for acute cholecystitis with cholelithiasis. Dense Calot's triangle adhesions prevented safe ductal exposure. Suspected cystic-duct impaction prevented intraoperative cholangioscopic entry; the remnant was sutured (no clip/stapler documented), and no formal intraoperative cholangiography was performed. MRCP showed a long, low-inserting cystic duct, a 6-mm confluence calculus and upstream biliary dilatation. Tissue friability and persistent obstruction may have contributed to t

Mirizzi syndrome (MS) is a rare complication of cholelithiasis, resulting from the extrinsic compression of the common hepatic duct or common bile duct by impacted gallstones in the cystic duct or Hartmann's pouch. MS is most commonly observed in the elderly with a long-standing history of gallstones. We present the case of MS type I diagnosed following magnetic resonance cholangiopancreatography (MRCP). Surgical management was performed with laparoscopic cholecystectomy. MS should be considered as a differential diagnosis in elderly patients presenting with asymptomatic obstructive jaundice.

INTRODUCTION: Mirizzi's Syndrome (MS) is a rare pathology, known to be a challenge for the surgeon. In the surgical management, open approach vs laparoscopic is a topic of discussion due to anatomic variations. The aim of this study is to analyze our experience in the laparoscopic management of this condition in Type Va.
METHODS: We made a descriptive retrospective study of patients diagnosed with MS type Va and treated by laparoscopic approach from 2014 to 2019, in two high volume centers of Bogotá, Colombia.
RESULTS: 1073 patients who presented complications from gallstones were evaluated,

BACKGROUND: An unusual complication of gallstone diseases is Mirizzi syndrome. This disorder is characterized by jaundice, fever and right upper quadrant pain. It is defined by an obstruction of the common hepatic duct due to an extrinsic compression from a stone in the gallbladder infundibulum or the cystic duct. It should be identified as soon as possible in order to prevent complications, notably biliary sepsis. This disorder can sometimes mimic the symptoms of cholangiocarcinoma (CCA).
CASE SUMMARY: We present the case of a 69 years old male presenting for painless jaundice and dark urine
