Effect of Mobile Stroke Unit Dispatch in all Patients with Acute Stroke or TIA.

OBJECTIVE: To determine the effect of additional mobile stroke unit (MSU) dispatch on functional outcomes among the full spectrum of stroke patients, regardless of subtype or potential contraindications to reperfusion therapies. METHODS: We used data from the nonrandomized Berlin-based B_PROUD study (02/2017 to 05/2019), in which MSUs were dispatched based solely on availability, and the linked B-SPATIAL stroke registry. All patients with final stroke or transient ischemic attack (TIA) diagnoses were eligible. The intervention under study was the additional dispatch of an MSU, an emergency ph

Use of Accessible Weight Scales and Examination Tables/Chairs for Patients with Significant Mobility Limitations by Physicians Nationwide.

BACKGROUND: Mobility limitations are the most common disability type among the 61 million Americans with disability. Studies of patients with mobility limitations suggest that inaccessible medical diagnostic equipment poses significant barriers to care. METHODS: The study team surveyed randomly selected US physicians nationwide representing seven specialties about their reported use of accessible weight scales and exam tables/chairs when caring for patients with mobility limitations. A descriptive analysis of responses was performed, and multivariable logistic regression was used to examine a

Orthopedic Injuries and Their Treatment in Children During Earthquakes: A Systematic Review.

Orthopedic injuries commonly affect children during earthquakes, but reports about them are rare. This setting may lead to different standards of care, but guidelines are still missing in this field. A systematic review was performed to: (1) assess type and body distribution of pediatric earthquake-related injuries, treatment performed, length of stay, and complications; and (2) identify starting points to define standards of care. PubMed database was researched for papers (1999-2014 period) in agreement with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) State

Earthquake related scald injuries--experience from the IDF field hospital in Duzce, Turkey.

On the evening of November 12th, 1999, earthquakes stroke the area of Duzce and Bolu in the northwestern part of Turkey. The local medical system, still not recovered completely from the August earthquake, suffered severe damage. An Israeli field hospital was sent to the area to help cope with the earthquake trauma and replace the damaged medical system until it recovered. During the eight days of its active duty, approximately 40 burn patients were seen. Most of the burns were deep partial scald burns of the lower extremity, caused by hot water spill during the quake. Two massive burns were r

Mammographic screening: how to operate successfully at low cost.

To offer mammographic screening to asymptomatic women at low cost requires a different approach than is appropriate for solving the more complex problems presented by symptomatic patients. Since the goal of screening is to detect unsuspected abnormalities, not to characterize them fully, operational procedures can be streamlined to maximize patient throughput and achieve substantial cost savings. Such procedures will be successful only if applied to large numbers of patients, thereby permitting cost reduction by economies of large-scale operation. Computerization of record-keeping and of image

[An ophthalmologist's hints for organising medical help in remote areas of developing countries (author's transl)].

Judging by her experience gained in the south of Tunisia, the author suggests that instead of sending individual ophthalmologists to the developing countries it would be more advantageous to send mobile work teams in vehicles equipped with ophthalmological examination and surgical instruments and apparatus. This would be a suitable reply to the appeal made by the World Health Organization in 1973 for organising large-scale surgical treatment of cataracts with international participation within the framework of struggle against blindness in the world.

Presence, absence, and contingent engagement: mobilising care in the endgame of hepatitis C elimination.

BACKGROUND: In the 'endgame' of hepatitis C elimination, mobile and other decentralised care models instrumentalise point-of-care testing and direct-acting antivirals to innovate care pathways for underserved populations. While implementation science evidences their 'effectiveness', less is known about how these interventions work in practice and how their effects are produced in specific settings. APPROACH: This ethnographic case study traced a mobile, nurse- and peer-led hepatitis C clinic operating adjacent to community-based correctional services sites across Melbourne, Australia. Fieldwo

Mobile Dental Radiology-Evaluation of Quality Requirements for Radiographs Applying Handheld Mobile Radiography.

BACKGROUND: Radiography is required in domiciliary dentistry in order to ensure accurate diagnosis, but the available devices have not yet been approved due to a lack of research regarding the quality of single tooth radiography. AIM: To examine dental radiographs taken using the mobile X-ray device (Nomad Pro 2, Envista, Brea, CA) with regard to their quality, examinability and their benefit for treatment planning in mobile dentistry. METHODS: The image quality of radiographs using a mobile handheld X-ray system was assessed. The quality criteria catalogue of the German Dental Association w

Providing pharmacy services in a basketball arena: Reflections on building a pharmacy in a COVID-19 surge facility.

PURPOSE: As Coronavirus disease 19 (COVID-19) has spread globally, hospital systems have seen an increasing strain on their ability to accommodate the growing caseload. This demand has led countries to adopt varying surge-facility or alternate care site (ACS) models to manage patient overflow. This report describes the experience of setting up pharmacy services at a city-run surge facility in Philadelphia. SUMMARY: The COVID-19 Surge Facility at the Liacouras Center (CSF-L) was initially developed to serve as a site for patients convalescing from acute inpatient stays in order to free up heal

Simulated impact of mobile opioid treatment program units on increasing access to methadone for opioid use disorder.

OBJECTIVE: To model the potential impact of mobile methadone unit implementation in Louisiana on net medication for opioid use disorder (MOUD) treatment rates. DATA SOURCES/STUDY SETTING: We use secondary Louisiana Medicaid claims data between 2020 and 2021. STUDY DESIGN: We simulate the impact of mobile methadone units in Louisiana using two approaches: (1) a "Poisson regression approach," which predicts the number of opioid use disorder (OUD) patients that might use methadone at mobile locations based on the underlying association between methadone use and proximity to a brick-and-mortar m

Regression calibration of self-reported mobile phone use to optimize quantitative risk estimation in the COSMOS study.

The Cohort Study of Mobile Phone Use and Health (COSMOS) has repeatedly collected self-reported and operator-recorded data on mobile phone use. Assessing health effects using self-reported information is prone to measurement error, but operator data were available prospectively for only part of the study population and did not cover past mobile phone use. To optimize the available data and reduce bias, we evaluated different statistical approaches for constructing mobile phone exposure histories within COSMOS. We evaluated and compared the performance of 4 regression calibration (RC) methods (

A lifeline on wheels: perspectives of stakeholders on the implementation and impact of a mobile medications for opioid use disorder unit.

INTRODUCTION: Mobile health units (MHUs) providing medications for opioid use disorder (MOUD) have emerged as a critical strategy to address gaps in opioid treatment access, particularly for marginalized populations. Research has yet to explore stakeholder perspectives on MHUs' implementation, challenges, and long-term sustainability. METHODS: We conducted semi-structured interviews with 15 stakeholders, including MHU staff, administrators, community partners, and policymakers. Interviews explored experiences with the MHU, barriers and facilitators to patient engagement, and operational chall

Do mobile clinics provide high-quality antenatal care? A comparison of care delivery, knowledge outcomes and perception of quality of care between fixed and mobile clinics in central Haiti.

BACKGROUND: Antenatal care (ANC) is an important health service for women in developing countries, with numerous proven benefits. Global coverage of ANC has steadily increased over the past 30 years, in part due to increased community-based outreach. However, commensurate improvements in health outcomes such as reductions in the prevalence of maternal anemia and infants born small-for-gestational age have not been achieved, even with increased coverage, indicating that quality of care may be inadequate. Mobile clinics are one community-based strategy used to further improve coverage of ANC, bu

Health status and social conditions of the elderly in remote districts. Results of a screening programme.

In 1978 a cohort of 1040 individuals aged 65 years or over was screened by public health nurses in Kuusamo. The screening produced basic data about health status and social conditions for an intervention of active medical care of the elderly by means of a mobile unit in rural districts. The response rate of the screening was 90%. 72% of the men and 35% of the women lived with their spouse; 6% of the men and 20% of the women lived alone. Poor health was assessed least commonly (36%) by those living alone, and most commonly by women living with their spouse (51%), even though 87% of them were un

Efficient engagement: opportunities for mobile methadone maintenance in geographically underserved areas.

In 2021 the Drug Enforcement Agency changed regulations to reduce barriers in the registration and operation of mobile methadone clinics for treating opioid use disorder in rural and underserved areas. How these routes might best be operated and how many clients might be served by mobile units are open questions. This work identifies candidate dispensing locations from mobile units in areas that do not have geographic access to a fixed clinic. Optimal routes for mobile methadone are identified by solving a mixed integer program that maximizes the number of clients without access to care that c

Outreach services to brothels facilitate sexually transmissible infection testing for Asian sex workers in regional Australia.

Outreach services providing sexually transmissible infection (STI) testing on-site in brothels were introduced in 2012 by Pacific Clinic Newcastle, Australia. Asian female sex workers (FSW) were more likely than non-culturally and linguistically diverse (CALD) FSW to access testing via outreach services than in clinics (OR 2.38, 95% CI 1.82-3.11, P < 0.0001) and to test positive for chlamydia or gonorrhoea (OR 2.58, 95% CI 1.58-4.20, P = 0.0001). Outreach services offering testing on-site in brothels are recommended to facilitate access for Asian FSW in similar settings.
