Rat bite fever caused by Streptobacillus notomytis mimicking pyogenic polyarthritis: A case report.

We report the world's sixth case of rat bite fever caused by Streptobacillus notomytis that mimicked pyogenic polyarthritis and required surgical debridement in combination with prolonged antibiotic therapy. This case report highlights the higher severity of rat bite fever caused by S. notomytis compared to S. moniliformis.

[Zoonoses transmitted by mouse and rat maintained as laboratory or pet animals].

Large numbers of mice (Mus spp.) and rats (Rattus spp.) are maintained for scientific reasons and as pet animals in Germany. While laboratory animals are monitored for pathogenic agents, the hygienic status of pet animals is usually completely unknown. Despite great efforts, zoonotic infections were reported even in laboratory settings, e.g. with Hantavirus (Seoul virus), Streptobacillus moniliformis, and Trichphyton mentagrophytes. However, in current reports, zoonotic infections were transmitted by mice and rats maintained as pet animals. This includes infections by Lymphocytic choriomeningi

Oceanivirga salmonicida gen. nov., sp. nov., a member of the Leptotrichiaceae isolated from Atlantic salmon (Salmo salar).

A pleomorphic, Gram-negative, rod-shaped, indole-, oxidase- and catalase- negative, non-spore-forming, non-motile bacterium was originally isolated in 1992 from moribund, seawater farmed Atlantic salmon with multifocal tissue necrosis. Strain AVG 2115T displayed considerable similarities with Streptobacillus moniliformis, one of the two etiological agents of rat bite fever, and has been stored as Streptobacillus sp. NCIMB 703044T. On the basis of 16S rRNA gene sequence analyses, this strain displayed >99 % sequence similarities with uncultured bacterial clones from the digestive tracts of mari

Difficulties encountered in identification of a nutritionally deficient streptococcus on the basis of its failure to revert to streptococcal morphology.

Difficulties were encountered in the recognition of a nutritionally deficient streptococcus which continued to display aberrant morphologic forms (especially bulbous swellings and filament formation) despite provision of growth factors. With isolates displaying diverse morphologic entities not characteristic of a given species, e.g., Streptobacillus moniliformis or Erysipelothrix rhusiopathiae, nutritionally deficient streptococci should be considered.

Rat-bite fever.

A 23-year-old woman presented with fever, arthralgias and a skin rash. She possessed nine pet rats, and denied that she had been bitten. Blood culture was positive for Streptobacillus moniliformis, which can cause rat-bite fever. The patient fully recovered after treatment with clarithromycin.

Rat-bite fever in animal research laboratory personnel.

A laboratory animal technician experienced undulating fever, chills, and myalgia 3 days after he was bitten by a laboratory rat. The clinical symptoms subsided with antibiotic therapy, but recurrent fever, malaise, and joint pain occurred when therapy was discontinued. Streptobacillus moniliformis was cultured from the patient's blood.

Clinical infection in house rats (Rattus rattus) caused by Streptobacillus notomytis.

Rat bite fever is an under-reported, under-diagnosed emerging zoonosis with worldwide distribution. Besides Spirillum minus, Streptobacillus moniliformis is the major causative microorganism although it usually colonises rats without any clinical signs. A group of house rats (Rattus rattus) kept in a zoo exhibition for educational purposes suffered from neurological signs including disorientation, torticollis, stall walking, ataxia and death. Gross pathological and histo-pathological examinations of the investigated rats revealed high-grade otitis interna et media, from which Streptobacillus n

[Rat-bite--an infant bitten by a rat].

Three days after a rat-bite a two-month-old girl developed clinical symptoms of Streptobacillus moniliformis infection, called rat-bite fever. She became seriously ill, but responded well to treatment with benzylpencillin. In the light of our case and previous publications, we recommend prophylactic penicillin to be given to infants after rat-bites.

A woman in her twenties with headache, fever and a rash.

BACKGROUND: Fever in combination with a rash is a presentation regularly seen in medicine. The causes clinicians must consider include infections, medications, autoimmune diseases. CASE PRESENTATION: A previously healthy young woman presented with a 3 to 4 day history of fever, headache and a maculopapular rash that also affected her palms. She was in a stable condition and was admitted for observation and further investigations without initiating antibiotic treatment. During the next two days her condition improved spontaneously, and her symptoms were initially interpreted as a viral infecti

[Rat bite fever in a three-year-old child].

Rat bite fever (RBF) is rare in Denmark. It can be very difficult to diagnose, as the causative agent is an extremely fastidious organism which needs microaerophilic conditions to grow. We report a case of arthritis in the right ankle due to Streptobacillus moniliformis in a three-year-old girl who fully recovered after 14-day treatment of intravenous administrated penicillin followed by four weeks of orally administrated amoxicillin. Social history revealed the presence of two domestic rats living in the child's home. As rodents are becoming more popular as pets, RBF must be considered in pat

Rat Bite Fever: Variability in Clinical Presentation and Management in Children.

Rat bite fever is a systemic illness most commonly caused by Streptobacillus moniliformis that is classically characterized by fever, rash and migratory polyarthritis. Here, we highlight the highly variable clinical presentations of rat bite fever in children and the importance of maintaining a high degree of suspicion so appropriate treatment can be promptly initiated.

Rat bite fever in a Greek child.

Rat bite fever was diagnosed in a 10-year-old boy in a rural area of south-west Greece. The clinical presentation was typical for this disease and the relevant causative agent (Streptobacillus moniliformis) was isolated from blood cultures of the febrile patient. Erythromycin treatment was efficient. Although extremely rare in continental Europe, this infection must be taken into account as a potential hazard of a rat bite.

[Digital purpura revealing septicaemical rat-bite fever].

INTRODUCTION: Infection with Streptobacillus moniliformis is an uncommon illness which can lead to death if untreated. We report the case in which initially cutaneous signs permitted diagnosis and further identification of the organism. CASE REPORT: A 42 year-old woman presented with a three-day history of acrally distributed purpuric macules on her fingers. Two days later, she was admitted for arthritis of the knees and wrists. There were two large pustules on the left elbow and the right knee. Laboratory studies showed inflammatory changes. The diagnosis of streptobacillary rat-bite fever w

Rat bite fever without the bite.

The case is reported of a patient who developed an acute arthropathy, which was initially diagnosed as acute septic arthritis. The true diagnosis of rat bite fever due to Streptobacillus moniliformis was delayed because of difficulty in growing the organism, which has fastidious growth requirements. The patient had no history of rat bite, which is the usual form of transmission of this disease.

Unusual infections in humans.

Nine cases of unusual infections in humans are presented. In each case, we present the clinical history, histopathologic changes (if indicated), morphologic features of the causative organism, diagnosis, discussion, differential diagnosis, therapy, and current literature. All of the cases are illustrated with pertinent photographs. The nine cases are as follows: (i) acanthocephaliasis, the first acquired human infection by Moniliformis moniliformis in the United States; (ii) dipylidiasis, an uncommon infection caused by the dog tapeworm, Dipylidium caninum; (iii) granulomatous amebic encephali

Rat-bite fever: a cautionary tale.

We present the first ever report of streptobacillary rat-bite fever in New Zealand. The patient was a young man who was admitted with systemic sepsis. He presented with a high fever, hypotension, and tender axillary lymphadenopathy. He had been bitten by a rat a week earlier. Blood cultures grew Streptobacillus moniliformis, thus confirming the diagnosis. The literature on rat-bite fever is also reviewed.

Rat bite fever on Vancouver Island: 2010-2016.

BACKGROUND: Rat bite fever (RBF) is a rare bacterial zoonotic infection caused by Streptobacillus moniliformis and Spirillum minus, which are found naturally in rodent respiratory tracts. Recently, multiple cases of RBF were observed on Vancouver Island, British Columbia. OBJECTIVE: To conduct a case series analysis of cases of RBF on Vancouver Island between 2010 and 2016 to characterize the epidemiology, presentation, microbiology and treatment of RBF. METHODS: Cases were identified through queries of discharge diagnosis and microbiology laboratory information. Clinical details were collec

Rat bite fever as a presenting illness in a patient with AIDS.

The etiology of culture-negative septic arthritis is poorly characterised in persons infected with human immunodeficiency virus (HIV). New molecular methods may assist in the investigation of culture-negative infections of sterile sites, including septic arthritis. We describe the first case of septic arthritis due to the cause of rat bite fever (RBF), Streptobacillus moniliformis, confirmed by 16S rRNA sequence analysis, in a patient with newly diagnosed HIV infection.

Rat bite fever misdiagnosed as Rocky Mountain spotted fever.

A patient who had been exposed to ticks and who had also been bitten by a laboratory rat developed fever, headache, and a rash. He was treated with chloramphenicol for Rocky Mountain spotted fever, and recovered. Blood cultures, however, grew Streptobacillus moniliformis, a causative agent of rat bite fever. The case report illustrates the clinical similarities between rat bite fever and Rocky Mountain spotted fever.

An unusual tale of rat-bite fever endocarditis.

A 49-year-old man presented to our department with an acute history of right leg tenderness, rash, swelling and fever. CT of the chest, abdomen and pelvis and a transoesophageal echocardiogram confirmed the diagnosis of mitral valve infective endocarditis with distal splenic emboli. Positive blood cultures revealed the causative organism to be Streptobacillus moniliformis. The patient was treated with high-dose antibiotics and had mitral valve replacement surgery.
