Therapeutic value of continuous passive motion after anterior cruciate replacement.

The aim of this study is the evaluation of the therapeutic value of continuous passive motion after ACL replacement. After 41 ACL reconstruction in 13 cases only active motion, in 28 cases active motion and CPM were used postoperatively. Stability, the range of motion and complications were evaluated at the time of discharge, 3 and 6 months after the operation. The flexion-extension in the CPM group was significantly greater at the time of discharge, but this difference was practically eliminated in 6 months after the operation. There were not any other significant difference between the two g

[Physical prevention of thrombosis with the ankle joint with the motorized ankle joint movement device. Initial results of a clinical study].

Between 1993 and 1994 the value of the "Artroflow" device in deep venous thrombosis was tested in 95 trauma surgical high risk patients. Parallel to the application of Heparin and physical methods, the "Artroflow" device was employed. The test and compare group showed an equal amount of risk parameters and comparable injuries. In all patients until full mobilization a weekly clinical examination, a compression sonography of the deep leg veins and a venous Doppler examination was performed. In the test group 1 deep venous thrombosis (2.3%) and no clinically manifest lung embolism occurred. In t

Autonomous Patient-Controlled Mobilization Protocol After Flexor Tendon Repair: A Case Series.

BACKGROUND: Despite many publications on rehabilitation after repair of flexor tendon injuries of the hand, there is no consensus as to which method is superior. It is clear that nonadherence to postoperative therapy adversely affects the outcome after flexor tendon surgery. In the context of a developing country, the most important factor associated with poor outcome is late onset of rehabilitation therapy. An autonomous rehabilitation program is proposed, with the use of a low-cost splint and based on an online illustrative video with the expectation to improve adherence and patient complian

Interventions for congenital talipes equinovarus (clubfoot).

BACKGROUND: Congenital talipes equinovarus (CTEV), which is also known as clubfoot, is a common congenital orthopaedic condition. It is characterised by an excessively turned in foot (equinovarus) and high medial longitudinal arch (cavus). If left untreated it can result in long-term disability, deformity and pain. Interventions can be conservative (such as splinting or stretching) or surgical. OBJECTIVES: To evaluate the effectiveness of interventions for CTEV. SEARCH METHODS: We searched CENTRAL (2011, Issue 2), NHSEED (2011, Issue 2), MEDLINE (January 1966 to April 2011), EMBASE (January

The effects of immediate continuous passive motion on pain during the inflammatory phase of soft tissue healing following anterior cruciate ligament reconstruction.

Continuous passive motion (CPM) may have potential application as a physical modality in decreasing acute pain. The purpose of this study was to examine the effects of CPM immediately following an arthroscopically-assisted anterior cruciate ligament (ACL) reconstruction utilizing bone-patella-bone autograft on acute pain during the inflammatory phase of soft tissue healing. Acute pain was measured by assessing the amount of pain medication (amount of narcotic delivered from the patient-controlled analgesia (PCA) pump during the first postoperative 24 hours and the total intake of oral medicati

Fresh osteochondral allografting of the femoral condyle.

Fresh osteochondral shell allografting is conceived as an interim response to the localized loss of articular cartilage in young patients for whom there is no reasonable alternative after conservative procedures have failed. The concept is not new and has been investigated extensively in in vivo animal investigations. The functional and anatomic results of these studies, however, have been consistently unsatisfactory because of technical deficiencies and supposed immunogenic responses. The results of early clinical studies were variable, and despite more recent clinical work, the procedure has

Evaluation of robotic training forces that either enhance or reduce error in chronic hemiparetic stroke survivors.

This investigation is one in a series of studies that address the possibility of stroke rehabilitation using robotic devices to facilitate "adaptive training." Healthy subjects, after training in the presence of systematically applied forces, typically exhibit a predictable "after-effect." A critical question is whether this adaptive characteristic is preserved following stroke so that it might be exploited for restoring function. Another important question is whether subjects benefit more from training forces that enhance their errors than from forces that reduce their errors. We exposed hemi

Rhythmic bilateral movement training modulates corticomotor excitability and enhances upper limb motricity poststroke: a pilot study.

The recovery of coordinated motor function after stroke onset has been associated with the practice of upper limb movements that required the activation of homologous muscles. This pilot study investigated whether repetitive bimanual coordinated movements enhanced upper limb corticomotor (CM) excitability and motor function poststroke. Patients practiced driving their paretic wrist through passive rhythmical flexion-extension by active flexion-extension of their unaffected wrist using purpose-built manipulanda over a 4-week period. Both preintervention and postintervention motricity was assess

The combined effects of continuous passive motion treatment and acellular PLGA implants on osteochondral regeneration in the rabbit.

We investigated the active role of clinical rehabilitation in osteochondral regeneration using continuous passive motion (CPM) treatment together with acellular PLGA implants. CPM treatment was performed and compared with immobilization (Imm) treatment and intermittent active motion (IAM) treatment upon full-thickness osteochondral defects either with or without an PLGA implant in the PI (PLGA-implanted) and ED (empty defect) models. The PI and ED tests were performed in 38 rabbits for 4 and 12 weeks. At the end of testing, the PI-CPM group had the best regeneration with nearly normal articula

Development and Evaluation of a Novel Upper-Limb Rehabilitation Device Integrating Piano Playing for Enhanced Motor Recovery.

This study developed and evaluated a novel upper-limb rehabilitation device that integrates piano playing into task-oriented occupational therapy, addressing the limitations of traditional continuous passive motion (CPM) training in patient engagement and functional recovery. The system features a bi-axial sliding platform for precise 61-key positioning and a ten-link, four-loop robotic hand for key striking. A hierarchical control framework incorporates MIDI-based task mapping, finger optimization using an improved Hungarian algorithm, and impedance-admittance hybrid control for adaptive forc

Management of a Post-traumatic Stiff Elbow: A Case Report.

INTRODUCTION: Elbow movement is crucial for proper upper limb function, allowing the hand to be positioned accurately in space. However, the elbow is highly susceptible to stiffness, which can result from a variety of traumatic or non-traumatic causes. Diagnosis of elbow stiffness involves a thorough medical history and physical examination, often supported by relevant imaging studies. Managing a stiff elbow can be difficult, making prevention a key consideration. If preventive measures are insufficient, treatment typically progresses from non-surgical to surgical options. For patients present

[Immobilization damage].

Prolonged immobilization can threaten the surgical result. The reduction of the muscle diameter is reported to be between 10 and 60% after four weeks of immobilization depending on the different muscle groups. Particularly the oxidative type-1 fibers, the "slow twitch fibers" are concerned. However, the sequelae of immobilization of muscle are reversible, which is in contrast to the detrimental effects immobilization has on the bone. Inactivity osteoporosis is occurring in three stages with bone loss as much as five to twenty times that of other calcipenic disorders. The physiological coupling

The stiff elbow: Current concepts.

Elbow stiffness is defined as any loss of movement that is greater than 30° in extension and less than 120° in flexion. Causes of elbow stiffness can be classified as traumatic or atraumatic and as congenital or acquired. Any alteration affecting the stability elements of the elbow can lead to a reduction in the arc of movement. The classification is based on the specific structures involved (Kay's classification), anatomical location (Morrey's classification), or on the degree of severity of rigidity (Vidal's classification). Diagnosis is the result of a combination of medical history, physic
