Refractive cataract surgery has evolved considerably in recent years owing to the rapid development in precise ocular biometry, surgical technique and instrumentation, and various astigmatism-correcting and multifocal intraocular lenses. However, inherent defects of these premium intraocular lenses should not be underestimated and neither should imperfections of measuring devices. Cataract surgery complications such as posterior capsular opacification and dry eye, which were originally infrequent or minor, are becoming increasingly prominent. Surgeons must properly understand and conduct refra

BACKGROUND: Severe decentration and subluxation of intraocular lenses (IOLs) may lead to double vision, glare and deterioration of vision to the point of functional aphakia. The purpose of the present study was to analyse causes for severe IOL dislocation.
MATERIAL AND METHODS: Between January 1989 and January 1996, 37 patients required IOL exchange because of decentrated or subluxated posterior chamber lenses. Twenty-five of the exchanged lenses were implanted in our hospital, 12 lenses elsewhere. After explantation the lenses were examined by light and electron microscopy.
RESULTS: In 10 e

Refractive lens exchange (RLE) allows to correct ametropias and presbyopia by replacing the crystalline lens with an extended depth of focus or multifocal intraocular lens (IOL). Retinal detachment (RD) is one of the most serious adverse events after RLE. This study aimed to review the evidence related to the risk of RD after RLE and clinical outcomes. A search using PubMed and a snowball search approach was conducted to identify articles and case reports. According to the literature, the risks of RD should be considered in patients <60 years old with axial lengths >23 mm. Only nine articles r

PURPOSE: To evaluate the differences in intraocular lens (IOL)-induced astigmatism according to differences in orientation of a multizonal multifocal IOL, the Precizon Presbyopic NVA IOL (Ophtec BV).
METHODS: The clinical study reviewed 80 eyes from 40 patients with cataracts who underwent Precizon Presbyopic IOL implantation. Residual astigmatism, as measured by autorefraction and manifest refraction, was investigated using vector analysis of eyes implanted with vertical (90 ± 30 degrees) and horizontal (180 ± 30 degrees) orientations of the first near segment of the IOL. In the ray-tracing

BACKGROUND: Premium intraocular lenses (IOLs), including multifocal, extended depth-of-focus (EDOF), and light adjustable lenses (LALs) are used to reduce spectacle dependence after cataract surgery. However, their use in glaucoma patients remains controversial due to concerns that reduced contrast sensitivity and pre-existing visual field loss limits functional benefits from these optics. Increasing use of minimally invasive glaucoma surgery (MIGS) at the time of cataract surgery has made combined refractive and glaucoma management more common, but data on real world outcomes remains limited.

PURPOSE OF REVIEW: To discuss the development of presbyopia-correcting intraocular lenses (IOLs), what we have learned since their introduction a few decades ago, what are the options currently on the market, and where the technology is heading in the future.
RECENT FINDINGS: Multifocal and accommodating IOLs have gone through several modifications to improve distance, intermediate and near vision compared to their predecessors. These modifications have also targeted unwanted side-effects such as glare and halos in the multifocal lenses and inconsistent near-vision results in the accommodatin

Presbyopia surgery is a compensation option which aims at creating an increased depth of field. Monovision preserves a good close-vision quality for myopes. Multifocality is well tolerated by hyperopes if it is compatible with good far-vision quality. Conductive keratoplasty and Lasik multifocal ablation patterns are newly emerging methods. Monovision and multifocality can be applied to surgery by using corneal or intraocular implantation techniques. Multifocal intraocular lenses (IOLs) are growing in popularity among patients and surgeons, and opened the way to refractive lens exchange. Still

PURPOSE OF REVIEW: This review aims to introduce recent updates in cataract surgery and lens implantation.
RECENT FINDINGS: Compared to phacoemulsification, femtosecond laser-assisted cataract surgery (FLACS) may offer benefit for cataract patients with other complicated ocular diseases, such as corneal disease and vitreous abnormalities. Meanwhile, several types of novel intraocular lenses (IOLs) have been introduced to the market, and each of them has particular features in improving patients' visual acuity. In addition, based on a series of studies, multifocal IOL is superior to monofocal

PURPOSE: To model the performance of multifocal lenses in polychromatic lighting.
METHODS: The defocus transfer function (DTF) is a mathematical technique for illustrating the optical transfer function for all levels of defocus at a given wavelength. A polychromatic version of the DTF is developed that accounts for changes in cutoff frequency, reduction in diffraction efficiency, ocular chromatic aberration, and photoreceptor spectral sensitivity. The differences between the monochromatic and polychromatic DTF are illustrated with a diffractive multifocal intraocular lens.
RESULTS: Polychrom

Glaucoma frequently coexists with cataract, but glaucomatous functional loss, ocular-surface disease, anatomical abnormalities, and treatment-related refractive variability may reduce tolerance of premium intraocular lenses. This narrative review critically appraises evidence relevant to lens selection in glaucoma. Central visual-field status, contrast sensitivity, fixation, disease stability, anterior-segment anatomy, intraocular pressure control, topical treatment, previous or anticipated glaucoma surgery, and lifetime progression risk should inform decision-making. Enhanced monofocal lenses

PURPOSE: To evaluate wavefront aberrations in eyes implanted with rotationally asymmetric refractive bifocal intraocular lenses (IOLs) using a modified Shack-Hartmann wavefront sensor (SHWS) designed to mitigate artifactual coma.
METHODS: In this simulation study, postoperative optical quality was evaluated using wavefront analysis in the presence of various ocular aberrations. A bifocal phase map (3.00 diopter [D] near addition, 50% energy distribution) was introduced in a computer eye model (5-mm pupil; 850-nm light source). The emerging wavefront displaced the simulated SHWS lenslet images

Presbyopia-treating intraocular lenses (IOLs) are now widely available and offer high predictability at all distances, better visual outcomes, and high patient satisfaction. In most patients, the same multifocal IOL is implanted bilaterally. In certain circumstances, such as during complex surgeries, it is important to be prepared with a backup plan to maintain surgical expectations. We report 3 patients who had femtosecond laser-assisted cataract surgeries in both eyes. The complicated eye had implantation of a bifocal IOL and the fellow eye of a trifocal IOL. This intraindividual analysis sh

In recent years, there has been an overwhelming influx of different types of intraocular lenses (IOLs) as treatment for presbyopia. The extended depth-of-focus (EDOF) technology creates a single elongated focal point to enhance depth of focus, in contrast to the multiple foci of multifocal (MF) lenses. In this way, the EDOF lenses aim to reduce photic phenomena, glare, and halos, which have been reported in MF IOLs. A potential disadvantage of this is a blur due to decreased retinal image quality when the amount of the aberrations is increased excessively. Multifocality and EDOF characteristic
