Frequency of neuropathic pain and its effects on rehabilitation outcomes, balance function and quality of life among people with traumatic spinal cord injury.

BACKGROUND & OBJECTIVES: Traumatic Spinal cord injury (SCI) is a devastating condition that results in life long disability. Impairments associated with traumatic SCI such as sensory, motor, and autonomic dysfunctions lead to an array of secondary SCI-specific complications. Neuropathic pain is one of the most common medical complications of traumatic SCI which significantly affects motor function and activities of daily living (ADL) in people with traumatic SCI. Neuropathic pain is one of the main factors for dependency, decreased quality of life (QOL), poor rehabilitation outcomes, and depre

Use of Perturbation-Based Gait Training in a Virtual Environment to Address Mediolateral Instability in an Individual With Unilateral Transfemoral Amputation.

BACKGROUND AND PURPOSE: Roughly 50% of individuals with lower limb amputation report a fear of falling and fall at least once a year. Perturbation-based gait training and the use of virtual environments have been shown independently to be effective at improving walking stability in patient populations. An intervention was developed combining the strengths of the 2 paradigms utilizing continuous, walking surface angle oscillations within a virtual environment. This case report describes walking function and mediolateral stability outcomes of an individual with a unilateral transfemoral amputati

Successful Use of Peripheral Nerve Blocks for Emergent Trauma Surgery in a Patient With Perioperative Myocardial Infarction: A Case Report.

Patients with concurrent trauma-related surgical emergencies and ST-segment elevation myocardial infarction (MI) present a unique anesthetic challenge, as damage control surgery and rapid resuscitation must take priority over timely coronary revascularization. During these surgeries, the physiologic insult from general anesthesia can be detrimental to a failing heart, so regional anesthesia techniques and their implications in trauma should be considered. We present a 62-year-old male motorcyclist with hypertension, diabetes, and coronary artery disease and a history of percutaneous coronary i

Does form follow function in trochlear dysplasia? Case report of patellar instability in a below-knee amputee.

INTRODUCTION: Patellar instability can be considered as a rapidly increasing musculoskeletal subject of interest. It is now well accepted that multiple factors can produce this pathological condition, isolated, but more often in concert. One major static compound is found to be in trochlear dysplasia. It has been stated that the form of the trochlear surface is primary and genetically bilaterally determined already before use. According to these data, recurrent dislocation of the patella has been described as a hereditary disease with autosomal dominant transmission. Against this assumption, w

[Combined treatment of gaseous gangrene with hyperbaric oxygen therapy, surgery and antibiotics. A national cooperative multicenter study].

85 confirmed gas gangrene cases from general hospitals where hyperbaric oxygen chambers with single or multiple places are available were reviewed. The diagnosis was made on the basic of muscle necrosis with the characteristic color and odor of the wound, and suggestive clinical and bacteriological findings. The treatment consisted on surgical excision, multiple antibiotic therapy, and hyperbaric oxygen at 3 absolute atmospheres. There was a male predominance (76.5%); 49.4% of them had previous risk factors; the origin of the disease was spontaneous in 18.8% of cases, after trauma in 37.6%, an

Conus medullaris nerve root avulsions.

The association of avulsive lesions and pain has been well established in avulsions of the brachial plexus from the cervical spinal cord, but avulsive lesions of the conus medullaris have not previously been recognized or documented by direct observation. Six patients with intractable lower-extremity pain due to avulsion of nerve roots from the conus medullaris were treated by thoracolumbar laminectomy and dorsal root entry zone (DREZ) lesions. Patients with avulsion of lumbosacral roots from the conus medullaris have a characteristic clinical presentation. They are usually young men who, as a

Cardiomyopathy induced by sinus tachycardia in combat wounded: a case study.

Tachycardia induced cardiomyopathy is a potentially lethal cause of heart failure generally because of atrial tachycardia and less frequently ventricular tachycardia. We present two cases of Marines with severe traumatic blast injuries secondary to improvised explosive device attacks whose hospital courses included amputation, massive blood transfusions, and multiple surgeries. Both patients had prolonged sinus tachycardia averaging >110 beats per minute and developed depressed left ventricular function, which recovered when treated with β blockers. Sinus tachycardia is often considered a phys

Secondary ectopic transfer for replantation salvage after severe wound infection.

BACKGROUND: Ectopic transfer has been described as a salvage procedure in failing replants. The experience in three cases of infected failing replantations treated with secondary temporary ectopic transfer of the replanted part is presented. METHODS: Three patients with replanted traumatic amputations (one transhumeral, one transmetacarpal, and one transtibial) that developed severe wound infections and thrombosis of the anastomoses were treated with urgent ectopic transfer of the replanted part. The ectopic recipient vessels were the femoral, posterior tibial, and the descending branch of th

Reconstruction of the lower extremity with microvascular free flaps: a 10-year experience with 304 consecutive cases.

A retrospective review of 304 consecutive microvascular free flaps to the lower extremity was done to identify the patterns of usage, results, and problems. The most common indication (91%) was for the replacement of extensive tissue loss caused by motor-vehicle injuries. Most of the defects were below the mid-tibia (73%). The associated compound fractures were predominantly gradable as Gustilo type IIIb&c. The latissimus dorsi, the rectus abdominus, and the scapular skin were the flaps used most commonly for coverage (77%). The overall flap failure rate was 8%, compared with 3% for our series

The use of avulsed soft tissue for immediate full-thickness skin grafting in traumatic extremity injuries: A multi-institutional 10-year retrospective review of outcomes with technical considerations.

BACKGROUND: Avulsion or degloving injuries of the extremities are often caused by high-energy trauma and can be associated with high morbidity. Multiple techniques have been proposed for management, including immediate full-thickness skin grafting (IFTSG). Despite previous publication of our experience with this technique in 1999, awareness and implementation by surgeons remains poor. Outcomes of patients who underwent IFTSG from traumatically avulsed extremity soft tissues over a 10-year period were reviewed, with technical considerations included. METHODS: A retrospective cohort study of pa

[Emergency management of polytrauma patients].

Adequate prehospital care of the severely traumatised patient is important to prevent or attenuate early as well as late life threatening complications, such as tissue hypoxia, ischemia/reperfusion injury and finally multiple organ failure. A mismatch of oxygen supply and oxygen demand is a hallmark in the pathophysiology of multiple trauma. Oxygen supply may be diminished by the following factors: shock-related decrease of cardiac output, anemia and hypoxia. On the other hand, oxygen demand may be increased by pain, panic, and agitation. Hence, it is a central point in prehospital care to red

Bilateral Finger Morton Neuroma after Primary Fingertip Amputation.

Morton's neuroma (MN) is a painful, proliferative fibrosis of perineural tissue caused by pressure or recurrent irritation that was previously thought to affect only the common interdigital nerve of the foot. We present a case of a bilateral MN of both the ulnar and radial proper digital nerves of the fifth finger in a 30-year-old patient, following multiple surgeries on the aforementioned finger. The patient was referred to our center by a peripheral hospital after traumatic fingertip amputation distal the DIP joint, where the fingertip was initially simply sutured. Because of progressive neu

Bionic reconstruction : Restoration of extremity function with osseointegrated and mind-controlled prostheses.

BACKGROUND: Loss of an extremity at any level has a major impact on a patient's life. Using bionic reconstruction, extremity function can be restored and the patient reintegrated into daily life. Surgical procedures including selective nerve transfer and anchoring of prostheses into bone are combined with structured rehabilitation and modern prosthetic fitting. The patient is thereby able to use the prostheses intuitively and with multiple degrees of freedom. METHODS: This article presents the concept and approach for modern bionic reconstruction in detail and the relevant literature. The ner

Prosthetic reconstruction to restore function in transcarpal amputees.

Mutilated hands at the distal level may pose a challenge for reconstruction. Biological treatment options may require multiple surgical interventions and a long rehabilitation course with little hope of good functional outcome. Standard hand prostheses are also not an ideal solution, as they are too long and cumbersome for partial hand injuries. This paper outlines the functional outcomes of prosthetic reconstruction with devices customized for the transcarpal amputation levels. The functional outcome was evaluated with the Action Research Arm Test (ARAT), Southampton Hand Assessment Procedure

The effect of tactile and visual sensory inputs on phantom limb awareness.

Multiple sensory stimuli contribute to the conscious awareness of the body. It is well known that limb amputation can result in abnormal body awareness, but the manner in which the CNS constructs and updates a body schema after injury is largely unknown. The purpose of the present study was to systematically evaluate the effects of sensory inputs on phantom limb awareness (PLA) shortly after unilateral upper extremity amputation. The location, quality and intensity of spontaneous and tactile-evoked phantom sensations and awareness were assessed in 13 amputees who were referred sequentially for

The outcomes of salvage surgery for vascular injury in the extremities: a special consideration for delayed revascularization.

A seven years retrospective study was performed in 45 consecutive vascular injuries in the extremities to investigate the pattern of injuries, managements and outcomes. Motor-vehicle accidents were the leading cause of injuries (80%), followed by industrial injuries (11.1%) and iatrogenic injuries (4.4%). Popliteal and brachial artery injuries were commonly involved (20%). Fifteen (33.3%) patients had fractures, dislocation or fracture dislocation around the knee joint and 6 (13.3%) patients had soft tissue injuries without fracture. Traumatic arterial transection accounted for 34 (75.6%) case

[Replantation in multiple injuries?].

While amputations of extremity parts are not uncommon in multiple trauma, replantation of these parts is not always feasible. Initial therapy will concentrate on plain survival. We suggest an aid to identify those patients with multiple trauma and extremity amputation, who should undergo replantation.
