Journal of racial and ethnic health disparities2015eng
This study tested whether race and ethnic group differences exist for lifetime major depressive disorder and/or general anxiety disorder with one or more chronic medical conditions. Data from the National Survey of American Life, which included 3570 African American, 1438 Caribbean Black, and 891 non-Hispanic White adults were analyzed. Outcomes included at least one and multiple chronic medical conditions, from a list of 14 medical conditions (e.g., arthritis, cancer, diabetes, kidney disease, stroke, heart disease, etc.). Logistic regressions were fitted to data to determine how the associat

CONTEXT: The Chronic Care Model (CCM) has been developed to improve the quality of medical care delivered by general practitioners to patients with multiple chronic conditions. Despite an increasing use of this model, it remains unclear to what extent the different recommendations are valued by the patients.
OBJECTIVE: This study aims to identify the preferences of patients with multiple chronic conditions for recommendations of the Chronic Care Model.
METHODS: The patients' preferences were identified with a discrete choice experiment. The hypothetical general practice cares were described

For patients who do not achieve adequate disease control on biologic monotherapy, or monotherapy with an oral-systemic agent such as methotrexate, combination biologic therapy may be considered. To the best of our knowledge, we report the first case assessing the safety and efficacy of the combination of an interleukin-23 (IL-23) inhibitor (risankizumab) with a tumour necrosis factor-α (TNF-α) inhibitor (golimumab) in the treatment of psoriasis and psoriatic arthritis. After twelve months of treatment with risankizumab and golimumab, our patient experienced a significant improvement in his pso

BACKGROUND: More and more countries have been implementing chronic care programs, such as the Chronic Care Model (CCM) to manage non-acute conditions of diseases in a more effective and less expensive way. Often, these programs aim to provide care for single conditions instead of the sum of diseases. This paper analyzes the satisfaction and better management of single and multiple chronic patients with the core elements of chronic care programs in Siena, Italy. In addition, the paper also considers whether the CCM introduced in Siena has any influence on satisfaction and better self-management

Acceptance of chronic conditions often has been studied from the perspective of grief and loss. This paper presents a new conceptualization of acceptance of chronic conditions that is grounded in the perspective of those living with a chronic condition and focuses on adaptation to and acceptance of change in one's life. Data obtained from a sample of 822 persons with multiple sclerosis and a sample of more than 2,000 polio survivors support the reliability and validity of the new Acceptance of Chronic Health Conditions Scale.

Patients with chronic lung conditions, including cystic fibrosis, may be prone to severe COVID-19. Therefore, therapeutic intervention should be prompt and tailored to all associated comorbidities. We report the case of a 17-year-old male adolescent with cystic fibrosis and multiple chronic conditions (bronchiectasis, exocrine pancreatic insufficiency, chronic multidrug resistant Pseudomonas aeruginosa colonization, nasal polyposis, chronic sinusitis, ventricular extrasystoles and multiple drug allergies), who presented with an acute episode of productive cough, and was confirmed with moderate

Chronic pulmonary aspergillosis (CPA) is a condition caused by the ubiquitous fungus Aspergillus fumigatus in non-immunocompromised individuals. Numerous underlying conditions have been associated with CPA. Details of the underlying conditions of 126 CPA patients attending our tertiary referral clinic from all over the UK were extracted from the clinical notes, and the distribution of these underlying conditions was analysed. For those with several underlying pulmonary conditions, one was nominated as the primary condition. Many patients presented with multiple underlying conditions, and a tot

The population of persons living with HIV (PLWH) is growing older and more prone to developing other chronic health conditions. Disease progression has been shown to be related to quality of life (QoL). However, descriptions of chronic comorbid illnesses and the unique QoL challenges of older adults living with HIV are not well understood and have not been examined in multiple geographic locations. About 452 PLWH aged 50 years or older were recruited from AIDS Service Organizations in nine states. Participants completed a telephone survey that included measures of other chronic health conditio

INTRODUCTION: The Advancing Chronic Care with Effective Scalable Solutions (ACCESS) model is a new outcome-aligned payment model intended to support technology-enabled chronic disease management among Traditional Medicare (TM) beneficiaries. However, the national population potentially eligible for ACCESS remains poorly characterized, particularly those with multiple chronic conditions.
METHODS: Using nationally representative survey data from the 2023 Medical Expenditure Panel Survey, we described the health characteristics and health care utilization patterns of TM beneficiaries potentially

BACKGROUND: Alexithymia is common among older adults, particularly those with multiple chronic conditions; however, its heterogeneity in empty-nest populations remains insufficiently explored. This study aimed to identify latent subgroups of alexithymia and examine their characteristics among elderly empty-nesters with multiple chronic diseases.
METHODS: A total of 561 empty-nest older adults with at least two chronic conditions were recruited from six cities in Jiangsu Province, China (November 2022-March 2024). Alexithymia was assessed using the Toronto Alexithymia Scale (TAS-20), and laten

BACKGROUND: Having a chronic disease such as multiple sclerosis (MS) may pose challenges for affected individuals in their work life. Given its progressive nature and wide range of symptoms, living with MS requires ongoing adaptation. In addition to work accommodations and support, individuals may develop their own strategies to manage their condition at work.
OBJECTIVES: To explore experiences of people with MS (PwMS) in managing MS in the workplace, focusing on strategies they employ to facilitate their work lives and challenges they navigate.
METHODS: Semi-structured individual interviews

This chapter reviews our current knowledge on the presence of overlapping syndromes in one form of chronic diffuse pain, fibromyalgia. Patients with fibromyalgia often present with signs and symptoms of other unexplained clinical conditions, including chronic fatigue syndrome, irritable bowel syndrome, temporomandibular disorders, and multiple chemical sensitivities. The high prevalence, impact on function and opportunities for treatment underscore the need for clinicians and researchers to screen routinely for co-morbid unexplained clinical conditions among persons with fibromyalgia. We, ther

BACKGROUND: For patients with diabetes, many with multiple complex chronic conditions, using a patient portal can support self-management and coordination of health care services, and may impact the frequency of in-person health care visits.
OBJECTIVE: To examine the impact of portal access on the number of outpatient visits, emergency visits, and preventable hospitalizations.
DESIGN: Observational study comparing patients' visit rates with and without portal access, using marginal structural modeling with inverse probability weighting estimates to account for potential bias due to confoundi

IMPORTANCE: The Dutch Regional Euthanasia Review Committees (RTEs) reviewed and reported an increasing number of cases of euthanasia and physician-assisted suicide (EAS) requested by older people with multiple geriatric syndromes (MGS). Knowledge of the characteristics of cases of EAS for MGS is important to facilitate societal debate and to monitor EAS practice.
OBJECTIVE: To examine the accumulation of patient characteristics, geriatric syndromes, and other circumstances as reported in the case summaries of the RTEs that led to unbearable suffering associated with a request for EAS and to a

Previously thought to be best described as a plasma cell disorder, Waldenström macroglobulinaemia (WM) is now understood to be a distinct clinicobiological entity. WM shares B-cell origin and certain other features with both chronic lymphocytic leukaemia (CLL) and multiple myeloma (MM). WM and CLL arise from B-cells at discrete stages in their maturation process, and MM arises from B-cells that have fully differentiated into plasma cells. While MM has a well-known precursor condition, monoclonal gammopathy of undetermined significance (MGUS), CLL and WM may also have associated precursor state

OBJECTIVES: To examine the associations between incarceration history and chronic medical conditions in a nationally representative sample of U.S. adults and whether risk for chronic medical conditions differ by sex and race among those with incarceration history.
METHOD: The data were from the 2012-2013 National Survey of Alcohol and Related Conditions III (N = 36,133). Multiple logistic regression was used to compare odds of a self-reported diagnosis for 24 chronic medical conditions by incarceration history, as well as by sex and race within those with incarceration history.
RESULTS: 12.4

PURPOSE OF REVIEW: This review aims to understand the foundations of cognitive behavioral therapy (CBT) and biofeedback, their indications for therapy, and evidence-based support.
RECENT FINDINGS: Both CBT and biofeedback are noninvasive therapy options for patients who are suffering from a variety of chronic pain conditions, including chronic low back pain, headache, fibromyalgia, and temporomandibular disorder (TMD). CBT has been shown to be effective in treating multiple chronic pain conditions.

BACKGROUND: Multimorbidity (the presence of multiple chronic conditions) is associated with high levels of healthcare utilization and associated costs. We investigated the association between number of chronic conditions and costs of care for nonelderly and elderly Veterans Affairs (VA) patients, and estimated mean VA healthcare costs for the most prevalent and most costly combinations of 3 conditions (triads).
METHODS: We identified a cohort of 5,233,994 patients who received care within the VA system in fiscal year 2010. We estimated the costs of VA care for each patient using established m

BACKGROUND: Over the next 10-year period in Sweden, the number of people aged 80 and older is will increase by about 50%. The care needs of the population are changing as more people live at home longer with chronic conditions. District nurses in primary care often have the first contact with patients of all ages and are especially involved in care of older individuals with multiple chronic conditions, common mental health problems, and complex care needs. Understanding how district nurses currently see their work with older people with complex multimorbidity is a key step in developing new mo

Patients with coronary artery disease (CAD) presenting with acute coronary syndrome or undergoing coronary stenting are indicated to treatment with dual antiplatelet therapy (DAPT) combining aspirin with a P2Y12 receptor inhibitor. The management of patients with CAD who present with a complex clinical profile due to multiple comorbidities, and/or undergoing complex interventional procedures, remains challenging as a high risk for both ischemic and bleeding events is often present; hence, the risk-benefit balance on the optimal DAPT duration is difficult to evaluate. The complexity of antiplat
