[Myocardial perfusion detected using digital subtraction angiography as compared with X-ray CT and Tl-201 myocardial imaging].

Forty-two patients who underwent aorto-coronary bypass surgery were examined with intra-aortic digital subtraction angiography (IADSA) to detect abnormalities of myocardial blood perfusion. Tl-201 myocardial imaging and cardiac X-ray CT were performed nearly simultaneously about one month postoperatively. Cardiac imaging using IADSA was performed at a rate of 30 frames/sec with matrix size of 512 x 256. Time density curves of each pixel in the region of the myocardium of the left ventricle were post-processed to obtain functional images. The maximal concentration (Cmax), integration of the tim

Detection and quantitation of right ventricular reversible perfusion defects by stress SPECT myocardial perfusion imaging: A proof-of-principle study.

BACKGROUND: In patients with right dominant coronary circulation, the right ventricular (RV) myocardium and the inferior region of the left ventricular (LV) myocardium share a common source of blood flow. We hypothesized that stress/rest SPECT myocardial perfusion imaging (MPI) could detect reversible perfusion defects in the RV in some patients with LV inferior wall perfusion abnormalities. MATERIAL AND METHODS: We identified 2 groups of patients with LV inferior wall perfusion defects (with or without defects in other regions of LV myocardium) from our database. Patients in group 1 (n = 17)

The influence of arm positions on mechanical dyssynchrony measured by gated myocardial perfusion imaging.

BACKGROUND: In routine procedures, patient's arms are positioned above their heads to avoid potential attenuation artifacts and reduced image quality during gated myocardial perfusion imaging (G-MPI). However, it is difficult to achieve this action in the acute period following pacemaker implantation. This study aimed to explore the influence of arm positioning on myocardial perfusion imaging (MPI) in different types of heart disease. METHODS: This study was conducted retrospectively. A total of 123 patients were enrolled and underwent resting G-MPI using a standard protocol with arms positio

Chest Pain with Normal Thallium-201 Myocardial Perfusion Image - Is It Really Normal?

BACKGROUND: Thallium-201 myocardial perfusion image (MPI) is commonly used to detect coronary artery disease in patients with chest pain. Although a normal thallium-201 MPI result is generally considered to be a good prognosis and further coronary angiogram is not recommended, there are still a few patients who suffer from unexpected acute coronary events. The aim of this study was to investigate the clinical prognosis in patients with normal thallium-201 MPI. METHODS: From January 2006 to August 2012, a total 22,003 patients undergoing thallium-201 MPI in one tertiary center were screened. O

Quantification of SPECT myocardial perfusion images: methodology and validation of the Yale-CQ method.

BACKGROUND: Quantification of single photon emission computed tomography (SPECT) images is important for reproducible and accurate image interpretation. In addition, SPECT quantification provides important prognostic information. The purpose of this study was to validate the Yale circumferential quantification (Yale-CQ) method in phantom studies. METHODS: Myocardial perfusion defects of varying extent and severities were simulated in a cardiac phantom with fillable defect inserts. Forty-five different phantom configurations simulated 45 different myocardial perfusion defect sizes, ranging fro

Stress myocardial imaging in patients with mitral valve prolapse: evidence of a perfusion abnormality.

Twenty-four patients with mitral valve prolapse underwent cardiac catheterization, exercise testing, and exercise 201thallium scintigraphy. Of 10 patients with coronary artery disease, six had abnormal scintigrams. Two of these six had exercise-induced reversible defects, two had defects that persisted during redistribution, and two had both reversible and persistent defects. Of 14 patients with normal coronary arteries, five had negative scintigrams. Of the remaining nine patients, two had exercise-induced defects, and seven (50%) had defects involving the inferior or posterior wall that pers

Real-World Comparison of Cardiac MRI and Nuclear Perfusion Imaging for Myocardial Scar Detection.

BACKGROUND: Cardiovascular magnetic resonance late gadolinium enhancement (CMR-LGE) is the reference standard for scar detection, but data comparing LGE with fixed perfusion defects (FPD) on nuclear myocardial perfusion imaging (MPI) are limited. OBJECTIVES: This study aimed to evaluate concordance between myocardial scar detection by CMR-LGE and nuclear MPI. METHODS: In this retrospective single-center study, patients undergoing contrast CMR and nuclear MPI within 1 year were included: single-photon emission computed tomography (SPECT) with CT attenuation correction (SPECTAC, n = 328), SPEC

Utility of stress parameters to determine prognosis following normal exercise SPECT myocardial perfusion imaging.

BACKGROUND: Exercise parameters achieved during an exercise stress myocardial perfusion imaging (MPI) with single-photon emission computed tomography (SPECT) study may help to predict clinical outcomes. The aim of our study was to compare different exercise stress parameters to determine their prognostic utility following a normal rest- exercise stress MPI. METHODS: We performed a retrospective cohort study on 3290 consecutive patients who had undergone rest-exercise-stress MPI from 2017 to 2024. 1020 patients had normal perfusion images and were included in the analysis. Association of rheumatoid arthritis with major adverse cardiovascular events despite normal myocardial perfusion imaging.

BACKGROUND: Rheumatoid arthritis (RA) is recognized as a cardiovascular risk-enhancing condition. Whether a normal myocardial perfusion imaging (MPI) study confers similar prognostic reassurance in RA as in non-RA patients remains unclear. METHODS: We retrospectively analyzed 282 patients with RA and 282 matched controls without autoimmune disease who underwent SPECT or PET MPI. Matching included age, sex, cardiovascular comorbidities, and stress modality. The primary outcome (MACE) was a composite of cardiovascular-specific mortality, myocardial infarction, late revascularization, or heart f

Coronary Flow Reserve Measured by Myocardial Perfusion Imaging in Patients Following Percutaneous Coronary Intervention.

BACKGROUND: Despite achieving complete revascularization, patients with coronary artery disease (CAD) may experience adverse outcomes, potentially attributable to coronary microvascular dysfunction (CMD). Coronary flow reserve (CFR), which integrates both epicardial and microvascular coronary function, and a reduced CFR following complete revascularization indicates the presence of CMD. AIMS: This study aimed to measure CFR in patients who underwent percutaneous coronary intervention (PCI) for complete revascularization using myocardial perfusion imaging (MPI) and analyzed the prevalence of C

Myocardial perfusion imaging-derived left ventricular strain: Regional abnormalities associated with transthyretin cardiac amyloidosis.

BACKGROUND: Transthyretin (ATTR) cardiac amyloidosis is associated with an apical-sparing strain pattern on TTE. We hypothesize that strain indices derived from myocardial perfusion imaging (MPI) can identify this abnormality. METHODS: A group with ATTR amyloidosis was compared to age-matched controls with LVH but without amyloidosis who underwent PET or SPECT MPI. Strain values were used to calculate the apical strain index (ASI), apex-to-base ratio (ABR), and ejection fraction to global strain ratio in multiple planes. RESULTS: A direct comparison using Welch's t-tests reveals 6 statistica

Optimizing stress-only myocardial perfusion imaging: a clinical prediction model to improve patient selection.

BACKGROUND: Stress-only single photon emission computed tomography myocardial perfusion imaging (MPI) offers numerous advantages in terms of improved workflow, cost and radiation reduction but is currently not widely utilized due to challenges in selecting appropriate patients for this technique. METHODS: Data from 5959 individuals were used to derive (N = 4018) and validate (N = 1941) a binomial logistic regression model to predict normal stress MPI studies (stress total perfusion deficit  < 4%, ejection fraction ≥ 50%). Model performance was analyzed using receiver operator characteristic c

Added prognostic value of plaque burden to computed tomography angiography and myocardial perfusion imaging.

BACKGROUND AND AIMS: Cardiac computed tomographic angiography (CCTA) - derived measures of coronary artery disease (CAD) burden have been shown to independently predict incident cardiovascular events. We aimed to compare the added prognostic value of plaque burden to CCTA anatomic assessment and single photon emission computed tomography (SPECT) physiologic assessment in a cohort with high prevalence of risk factors undergoing both tests. METHODS: Consecutive patients who underwent clinically indicated CCTA and SPECT myocardial imaging for suspected CAD were included. Stenosis severity and se

Adenosine Stress Induced Left Bundle Branch Block During Technetium-99m Tetrofosmin Myocardial Perfusion Imaging.

The occurrence of left bundle branch block (LBBB) in electrocardiogram during exercise testing is a relatively rare finding. The incidence of LBBB during exercise testing ranges from 0.5% to 1.1%. The mechanism of exercise-induced LBBB (EI-LBBB) is poorly understood, but ischemia is a proposed etiology. Stress myocardial perfusion imaging (MPI) can be useful in patients with EI-LBBB to rule out coronary artery disease. Adenosine vasodilator stress is the preferred mode of stress in patients with LBBB for performing stress-MPI. Here we present an interesting case of adenosine-induced LBBB durin

Correction of hysteretic respiratory motion in SPECT myocardial perfusion imaging: Simulation and patient studies.

PURPOSE: Amplitude-based respiratory gating is known to capture the extent of respiratory motion (RM) accurately but results in residual motion in the presence of respiratory hysteresis. In our previous study, we proposed and developed a novel approach to account for respiratory hysteresis by applying the Bouc-Wen (BW) model of hysteresis to external surrogate signals of anterior/posterior motion of the abdomen and chest with respiration. In this work, using simulated and clinical SPECT myocardial perfusion imaging (MPI) studies, we investigate the effects of respiratory hysteresis and evaluat
