T1 Mapping In Differentiating Healthy And Pathological Myocardium.

BACKGROUND: This study aimed to identify the optimal measurement location and technique for native T1 mapping to establish a standardized approach. We evaluated the diagnostic performance of various T1 mapping measurement approaches by comparing non-ischemic dilated cardiomyopathy (NIDCM) and hypertrophic cardiomyopathy (HCM) cohorts with a control group. METHODS: We retrospectively reviewed patients who underwent 1.5T cardiac magnetic resonance (CMR) with standardized protocol functional sequences, T1 mapping, and late gadolinium enhancement (LGE) between November 2016 and January 2023. Understanding the role of Shroom3 in the developing mouse myocardium.

Loss of actin cytoskeleton control can hinder integral developmental and physiological processes and can be the basis for a subset of developmental defects. SHROOM3 is an actin binding protein, best characterized as being essential for neural tube closure in vertebrates. Shroom3 expression has also been identified in the developing heart, with some associated congenital heart defects. Here we show that the expression pattern of Shroom3 in the developing and adult mouse heart is specific to the myocardium. Using a gene trap line, we show that embryos with homozygous full-body Shroom3 loss die a

Pathology of the Coronary Arteries and Myocardium in Kawasaki Disease.

Kawasaki disease (KD) has replaced rheumatic fever as the most common cause of pediatric acquired heart disease across the globe. The acute illness, characterized by fever and associated mucocutaneous features, is associated with a coronary artery arteritis and myocarditis. The destruction of the arterial wall leads to aneurysm formation in 25% of untreated children. Myocardial inflammation accompanies the vasculitis, and the long-term consequences of this acute inflammation are still being defined. Our incomplete understanding of the pathology stems in part from the unknown etiology of this v

An Atypical Presentation of Hodgkin Lymphoma Invading the Myocardium.

Pericardial involvement by Hodgkin lymphoma has been found in up to 20% of children at presentation, but disease of the myocardium itself is rare. We describe an 18-year-old male with HL who presented with a large mediastinal mass, pericardial effusion, and tumor invasion of both atrial walls with intra-atrial extension. A PubMed search of publications between 1989 and 2022 was conducted and additional older references were identified among these publications. While pericardial disease is described in numerous case series, myocardial involvement by HL, diagnosed clinically rather than at autop

The role and mechanisms of microvascular damage in the ischemic myocardium.

Following myocardial ischemic injury, the most effective clinical intervention is timely restoration of blood perfusion to ischemic but viable myocardium to reduce irreversible myocardial necrosis, limit infarct size, and prevent cardiac insufficiency. However, reperfusion itself may exacerbate cell death and myocardial injury, a process commonly referred to as ischemia/reperfusion (I/R) injury, which primarily involves cardiomyocytes and cardiac microvascular endothelial cells (CMECs) and is characterized by myocardial stunning, microvascular damage (MVD), reperfusion arrhythmia, and lethal r

Leukemic Infiltration of Myocardium Presenting as Cardiac Arrest.

Clinically significant myocardial infiltration by leukemic cells is a rare phenomenon. We describe a case of a 47-year-old woman with newly diagnosed acute myeloid leukemia and pleuritic chest pain with rapid cardiopulmonary decompensation. Post-mortem analyses showed fibrinous pericarditis and extensive leukemic infiltration of the myocardium. (Level of Difficulty: Intermediate.).

Complications from Needle Thoracostomy: Penetration of the Myocardium.

We report a rare but serious complication of needle thoracostomy, penetration of the myocardium. Needle thoracostomy is typically performed in the prehospital setting or upon arrival in the emergency department for suspected tension pneumothorax. Needle decompression is generally taught and done anteriorly, in the 2nd intercostal space on the midclavicular line (MCL). An alternative approach is laterally, along the anterior axillary line (AAL) in the 4th intercostal space. Our case supports prior literature that the anterior MCL location has a low rate of efficacy to decompress the chest, as w

Metabolic remodeling in hypertrophied and failing myocardium: a review.

The energy starvation hypothesis proposes that maladaptive metabolic remodeling antedates, initiates, and maintains adverse contractile dysfunction in heart failure (HF). Better understanding of the cardiac metabolic phenotype and metabolic signaling could help identify the role metabolic remodeling plays within HF and the conditions known to transition toward HF, including "pathological" hypertrophy. In this review, we discuss metabolic phenotype and metabolic signaling in the contexts of pathological hypertrophy and HF. We discuss the significance of alterations in energy supply (substrate u

New and revisited approaches to preserving the reperfused myocardium.

Early coronary artery reperfusion improves outcomes for patients with ST-segment elevation myocardial infarction (STEMI), but morbidity and mortality after STEMI remain unacceptably high. The primary deficits seen in these patients include inadequate pump function, owing to rapid infarction of muscle in the first few hours of treatment, and adverse remodelling of the heart in the months that follow. Given that attempts to further reduce myocardial infarct size beyond early reperfusion in clinical trials have so far been disappointing, effective therapies are still needed to protect the reperfu

Calcific aortic stenosis: a disease of the valve and the myocardium.

Although aortic stenosis is a common condition associated with major morbidity, mortality, and health economic costs, there are currently no medical interventions capable of delaying or halting its progression. Re-evaluation of the underlying pathophysiology is therefore required so that novel therapeutic strategies can be developed. Aortic stenosis is characterized by progressive aortic valve narrowing and secondary left ventricular hypertrophy. Both processes are important because in combination they drive the development of symptoms and adverse events that characterize the latter stages of

[Ventricular conduction disorder in presence of dead myocardium].

In the presence of an advanced degree left bundle branch block (LBBB) with an extensive "Jumping wave" phenomenon, the septal activation abnormally spreading originates septal electromotive forces of greater duration and consequently more important than normal ones. The coexistence of an inactive anteroseptal myocardium with an advanced degree block causes the phenomenon of "Jumping wave" begins in postero-septal regions. Therefore the external left electrodes see the first ventricular activation fronts moving away and register Q waves. The presence of transmural inactive myocardium in the fre

Acute myelitis with neurogenic stunned myocardium in a boy.

Neurogenic stunned myocardium is characterized by reversible left ventricular dysfunction, which occurs after severe central nervous system injury in patients without coronary artery disease. It has been described in association with subarachnoid hemorrhage, Guillain-Barré syndrome, and metastatic brain tumors in adults, but has been rarely reported in association with acute myelitis. Described here is a novel case of acute myelitis in a child associated with neurogenic stunned myocardium. This case demonstrates that patients with acute myelitis may harbor a risk of sympathetic dysregulation,

Noncompaction syndrome of the myocardium: pathophysiology and imaging pearls.

Patients with noncompaction syndrome of the myocardium may present with cardiac arrhythmia, thromboembolic events, or left ventricular failure. Associations with other diseases, including neurologic or other cardiac syndromes, have been noted. Noncompaction syndrome of the myocardium is primarily diagnosed by echocardiography; however, CT scanning and magnetic resonance imaging are useful tools for determining severity and the patient's prognosis. With these methods, high-resolution images of the myocardium are obtained that enable better recognition of the areas over which the trabeculae are

The myocardium in tetralogy of Fallot: a histological and morphometric study.

BACKGROUND: Patients with Tetralogy of Fallot frequently develop ventricular dysfunction in the postoperative period. The histological basis of this functional alteration has been scarcely studied. OBJECTIVE: To evaluate myocardial remodeling in anatomical specimens, comparing the subepicardial and subendocardial regions, especially because the subendocardial region is easily approached by means of endomyocardial biopsy. METHODS: Transmural sections of myocardium from the right ventricular (RV) inflow tract, anterior wall and infundibulum, and from the left ventricular (LV) free wall were ev

MRI features of tuberculoma of the right atrial myocardium.

Myocardial tuberculosis is an infrequent disease. Until now, the diagnosis has largely been made at necropsy. Only a few reports describe the antemortem diagnosis of myocardial tuberculosis. We describe the MRI features in a 12-year-old boy with myocardial tuberculoma of the right atrium. He also had intracerebral tuberculoma. The diagnosis was later confirmed by biopsy.
