Unilateral myokymia of the tongue after radiation therapy for cervical nodal melanoma.

Hypoglossal neuropathy with associated myokymia as a delayed effect of radiation is a rare occurrence, presumably due to the relative resistance of cranial nerves to injury from irradiation. The authors describe the first case of myokymia of the unilateral tongue with myokymic discharges on needle electromyography after hypofractionated radiation therapy for an extracranial melanoma of the neck. The earlier onset of myokymia than previous cases may represent more direct radiation exposure due to radiation site or the higher radiation dosage administered for treatment of melanomas.

Hereditary myokymia and paroxysmal ataxia linked to chromosome 12 is responsive to acetazolamide.

A sixth family with autosomal dominantly inherited myokymia and paroxysmal ataxia is described. The syndrome in this family is linked to the recently discovered locus for inherited myokymia and paroxysmal ataxia on the human chromosome 12p, and a missense mutation is shown in the KCNA1 gene. The attacks of ataxia in this family compare well with those of previously described families and similarly are precipitated by kinesigenic stimuli, exertion, and startle. Responsiveness of these attacks to low dose acetazolamide is confirmed, but some loss of efficacy occurs with prolonged treatment, and …

Magnetic resonance imaging of the superior oblique muscle in superior oblique myokymia.

High resolution, magnetic resonance imaging was used to quantitatively study the morphometry of the superior oblique muscles of two patients with superior oblique myokymia, as well as 18 superior oblique muscles of 14 patients with normal superior oblique function. The cross sectional area of each superior oblique muscle was measured at 3-millimeter intervals along the entire muscle length. In both cases of myokymia, the affected superior oblique muscles were significantly smaller than normal (P < .05). These anatomical changes in the superior oblique muscle of patients with myokymia suggest t…

Myokymia of the tongue in a case of brainstem tumour that simulated motor-neuron disease.

The diagnosis of a brainstem tumour, which presented as progressive bulbar palsy that resembled motor-neuron disease, was suspected because of the rare electromyographic finding of myokymia of the tongue, and was confirmed by magnetic resonance imaging.

[Nuclear magnetic resonance imaging in a case of facial myokymia with multiple sclerosis].

A 59-year-old female of facial myokymia with multiple sclerosis was reported. In this case, facial myokymia appeared at the same time as the first attack of multiple sclerosis, in association with paroxysmal pain and desesthesia of the neck, painful tonic seizures of the right upper and lower extremities and cervical transverse myelopathy. The facial myokymia consisted of grossly visible, continuous, fine and worm-like movement, which often began in the area of the left orbicularis oculi and spread to the other facial muscles on one side. Electromyographic studies revealed grouping of motor un…

Stiff man-like syndrome and generalized myokymia in spinocerebellar ataxia type 3.

We describe the novel association of spinocerebellar ataxia type 3/Machado-Joseph disease (SCA3/MJD) phenotype combining classical clinical presentation and semeiology mimicking stiff man syndrome (SMS). The studied pedigree comprises seven affected members in three generations. Their clinical picture consisted of cerebellar ataxia, pyramidal signs, facial myokymia, and ophthalmoplegia. The proband was a 39-year-old man in whom such a clinical picture, 5 years after onset at age 29, evolved to severe SMS and widespread myokymia. Electrophysiological study revealed continuous muscle activity in…

Neuropathies with prolonged conduction block, single and grouped fasciculations, localized limb myokymia.

Fourteen patients presented prolonged conduction block (CB) related to hereditary neuropathy with liability to pressure palsies (n = 7), neuropathy with proximal multifocal persistent CB (n = 2), radiation plexopathy (n = 3) and chronic acquired polyneuropathy (n = 2). All subjects presented both single and grouped fasciculation potentials that resulted in visible limb myokymia in 9 patients. This ectopic activity was recorded only in muscles affected by the CB, was more important in severe or complete CB, and often arose from terminal branches of blocked axons. These fasciculations and groupe…

A case of low-frequency myokymia visualized by simultaneous EMG-ultrasound recording.

BACKGROUND: Spontaneous muscle twitching observed on clinical examination can be challenging to distinguish between myokymia and fasciculation, particularly in peripheral nerve hyperexcitability syndromes where both phenomena may coexist. Because both fasciculations and myokymia may appear in Isaacs syndrome, ultrasonography alone can be misleading when discharge frequency is low, and correlation with needle EMG is essential for accurate differentiation. CASE PRESENTATION: We report a 53-year-old man with an 11-year history of progressive lower-limb twitching, cramps, and nocturnal muscle pai…

Facial twitches in ADCY5-associated disease - Myokymia or myoclonus? An electromyography study.

OBJECTIVE: A clinical feature in patients with ADCY5 gene mutations are perioral muscle twitches initially described as facial myokymia. METHODS: Five patients with ADCY5-associated disease with facial twitches and truncal jerks underwent electrophysiological investigations of the orbicularis oris and trapezius muscles to delineate neurophysiological characteristics of these phenomena. RESULTS: Electromyography (EMG) recordings showed a complex electrophysiological pattern with brief bursts of less than 100 ms and longer bursts with a duration of 100-300 ms up to several seconds in keeping w…

Hypothermia, hyperhidrosis, myokymia and increased urinary excretion of catecholamines associated with a thymoma.

OBJECTIVE: To report a rare syndrome associated with a thymoma and its first successful treatment with gammaglobulin. CLINICAL FEATURES: A 49-year-old white male presented with marked hypothermia, hyperhidrosis, myokymia and increased urinary excretion of catecholamines four weeks after complete excision of a malignant thymoma. He became increasingly drowsy, obtunded and required ventilatory support. INTERVENTION AND OUTCOME: A catecholamine secreting tumour was excluded and he was treated with intravenous gammaglobulin for five days, with dramatic improvement in his condition. Six months la…

Superior oblique myokymia. Quantitative characteristics of the eye movements in three patients.

Using the magnetic search coil technique, we measured horizontal, vertical, and torsional rotations of both eyes of two patients with idiopathic superior oblique myokymia, and of the affected eye in a third patient. Superior oblique myokymia was strictly monocular and consisted of an initial intorsion and depression of the affected eye and subsequent oscillations with torsional and vertical components. The peak-to-peak torsional and vertical amplitudes of the oscillations were less than 1 degree, but peak velocities frequently exceeded 4 degrees/sec in both planes. Fourier analysis indicated t…

Differential Response to Corticosteroids in Immune-Mediated Myokymia vs Grouped Fasciculation Discharges.

Myokymia and grouped fasciculation discharges (GFD) are electromyographic manifestations of peripheral nerve hyperexcitability syndromes. This case series explores the differential response to corticosteroids in patients with myokymia compared to those with GFDs. Six patients were included in this report: three with predominant myokymia and three with predominant GFDs, reviewed between March 2023 and February 2024. Electromyographic examinations were performed on various muscle groups, with the gastrocnemius EMG findings serving as an index. The neurologist conducting the EMG was blinded to th…

Selective Nasal Anterior Superior Oblique Tenectomy for the Treatment of Superior Oblique Myokymia.

Superior oblique myokymia is a rare condition characterized by spontaneous episodes of high frequency, low amplitude, usually unilateral torsional oscillopsia. Surgical treatment is indicated when medical therapy fails to improve symptoms. The traditional surgical recommendation is an ipsilateral full-tendon superior oblique tenectomy to remove all attachments of the superior oblique tendon to the sclera. This approach will likely result in a secondary superior oblique palsy, requiring simultaneous weakening of the ipsilateral inferior oblique muscle. We report a patient presenting with diplop…
