AIMS AND OBJECTIVES: Laparoscopic Heller myotomy (LHM) is the preferred surgical method for treating achalasia. However, peroral endoscopic myotomy (POEM) is providing good short-term results. BACKGROUND: Open Zenker's peroral endoscopic myotomy (OZ-POEM) was developed to overcome clinical failures arising from a persistent mucosal flap and difficult closure in Z-POEM. This study compared the outcomes of OZ-POEM and Z-POEM.

This study presents a modified surgical approach to laparoscopic myotomy for achalasia using stepped dilation with a Rigiflex balloon and contrast medium under image guidance. BACKGROUND: Surgical myotomy is the best therapeutic option for patients with achalasia. The minimally invasive technique is considered to be the preferred method for many surgeons. BACKGROUND AND AIMS: Peroral endoscopic myotomy (POEM) with fundoplication (POEM-F) combines POEM with endoscopic fundoplication to prevent postprocedural gastroesophageal reflux. The conventional techniques using the loop-ligating device can be technically challenging, with durability issues. Endoscopic suturing may address these limitations.
