BACKGROUND: Traditionally, Gustilo Anderson grade IIIb open tibial fractures have been treated by initial wide wound debridement, stabilization of fracture with external fixation, and delayed wound closure. The purpose of this study is to evaluate the clinical and radiological results of staged treatment using negative pressure wound therapy (NPWT) for Gustilo Anderson grade IIIb open tibial fractures.
MATERIALS AND METHODS: 15 patients with Gustilo Anderson grade IIIb open tibial fractures, treated using staged protocol by a single surgeon between January 2007 and December 2011 were reviewed

Topical negative pressure therapy is an alternative treatment for complex wounds that consists of the direct application of sub-atmospheric pressure, obtaining a number of effects that are beneficial for tissues, promoting the healing of both acute and chronic wounds and complementing surgical procedures. We report the case of a 75 year old man diagnosed with surgical wound dehiscence after a femorotibial bypass graft, who was hospitalized again with the aim to perform the implantation of a topical negative pressure therapy in the infected wound. We designed a care plan for this patient follow

The degree of viscero-abdominal disproportion often makes single-stage reduction difficult in large abdominal wall defects, without risking respiratory or hemodynamic compromise. As a consequence, clinicians have adopted a number of different methods to control these defects. Repair may be in the neonatal period, or later in life. Delayed repairs require epithelialization of the gastroschisis or omphalocele. Definitive repair may be in single or multiple stages. This paper describes four children in whom negative pressure wound therapy (NPWT) was used to facilitate closure of these complex def

Negative pressure wound therapy (NPWT) is considered to be an effective wound treatment, yet research has highlighted a clear need for its improvement, particularly in reducing pain. This is important not only for patient well-being and treatment satisfaction but also for the healing process. In parts 1 and 2, wound care clinicians were invited to share their views on patients' experiences of NPWT. Part 3, presented here, explores the views of patients who have had NPWT, through use of a questionnaire survey and interview (n = 50). Findings show that the majority of patients felt that NPWT had

Brown recluse spider (Loxosceles) bites cause lesions ranging from chronic necrotic ulcers to acute life-threatening sepsis. Based on our experience in treating acute and chronic wounds with negative pressure, we postulated that vacuum-assisted closure (VAC) would be valuable in this application. Chester pigs were procured and injected with purified brown recluse spider venom, 1 µl of venom in two anterior sites and 0·1 µl of venom in two posterior sites on their dorsum. For each concentration of venom, treatment consisted of either VAC or dry, non adherent dressings (control group). Each day,

Prophylactic closed incisional negative pressure wound therapy (NPWT) has emerged as a promising strategy to reduce postoperative wound complications following lower extremity amputation in patients with peripheral arterial disease (PAD). We conducted a meta-analysis to evaluate whether closed incisional NPWT reduces wound complications compared with standard wound care (SWC) in this high-risk vasculopathic population. A systematic search was performed to identify studies comparing closed incisional NPWT and SWC among patients with PAD undergoing lower extremity amputation. PubMed, Embase, Web

Chronic wounds are expensive and burdensome to the health care system, with some acute wounds, such as wounds after Mohs micrographic surgery (MMS), contributing to this problem. The rate of MMS performed is increasing, and if they become nonhealing, they can be challenging to treat and necessitate proper care. Both acellular dermal grafts and negative-pressure wound therapy are therapies that can be used to treat complicated wounds, but the combination of these 2 therapies has not been extensively studied. The study authors report the successful use of an acellular dermal graft in combination

Significance: Lower extremity traumatic wounds are associated with numerous perioperative challenges. Their etiologies determine the characteristics and extent of the injury. The timing of subsequent surgical intervention and wound healing optimization after lower extremity trauma are integral to successful perioperative lower extremity wound management. Recent Advances: Managing trauma to the lower extremities uses a multidisciplinary surgical approach. The objective of this review is to summarize lower limb trauma assessment, advancements in lower extremity trauma management, and the clinica

BACKGROUND: Negative pressure wound therapy (NPWT) reduces wound complications following oncoplastic breast surgery, yet there is limited evidence supporting its use in mastectomy without reconstruction. This study evaluated the effect of NPWT on post-mastectomy wound complications in a rural Australian centre.
METHODS: A single centre retrospective cohort study assessed simple mastectomy outcomes between 2020 and 2025. Wounds were grouped by post-operative dressing type (NPWT vs. no NPWT). The primary outcome was a composite of aspirated seroma, haematoma, wound dehiscence, and surgical site

Myiasis is a parasitic infection common in tropical areas, such as Brazil. It is caused by infestation of maggots of various fly species in humans. Here, the case of a 38-year-old female patient, with no comorbidities, who was referred to the authors' centre due to a scalp wound infested with larvae and complete soft tissue loss, is presented. A computed tomography scan revealed no evidence of osteomyelitis or intracranial compromise. Since the initial debridement and mechanical removal of the larvae resulted in a large and complex wound, the patient underwent a combined wound approach based o

Negative pressure wound therapy (NPWT) represents one of the many solutions for complex wounds of the upper extremity. The goal of this study was to investigate the most common indications for definitive treatment of wound defects in the upper extremity with NPWT and to report revision surgery outcomes after its use. A systematic review of the literature was performed. The following keywords and their combinations were used: "upper extremity," "arm," "forearm," "wrist," "hand," "finger" AND "negative-pressure wound therapy," "VAC therapy," "vacuum assisted closure." A total of 45 articles were

Management of large tissue defects resulting from local wide resection of perianal is a clinical challenge for surgeons. The aim of the present study was to investigate the efficacy of negative pressure wound therapy (NPWT) following skin grafting on perianal surgical wound healing.Included in this study were 12 patients with perianal tumors who received skin grafting after perianal tumor resection between December 2012 and December 2014. A self-designed negative pressure drainage device was then applied to maintain a standard negative pressure at -150 mm Hg and removed on day 8 postoperation.

To understand the changes in the peroxiredoxin-2 (PRDX2) expression level in the wound margin tissue (T-PRDX2) of patients with diabetic foot ulcer (DFU) before and after negative pressure wound therapy (NPWT). Additionally, the study aimed to explore the association between PRDX2 expression and the treatment outcome of DFUs to provide a new theoretical basis for revealing the mechanism of NPWT promoting the healing of DFUs. Fifty-six type 2 diabetes patients with foot ulcers undergoing NPWT (the DFU group) and 28 patients with chronic lower limb skin ulcers with normal glucose tolerance under

Higher closure rates of the open abdomen have been reported with negative pressure wound therapy (NPWT) than with other kinds of wound management. We have recently shown that NPWT decreases the blood flow in the intestinal wall, and that the blood flow could be restored by inserting a protective disc over the intestines. The aim of the present study was to investigate whether layers of Jelonet™ (Smith & Nephew) dressing (paraffin tulle gras dressing made from open weave gauze) over the intestines could protect the intestines from hypoperfusion. Midline incisions were made in ten pigs and were

BACKGROUND: Fournier's gangrene (FG) is a rare but life-threatening necrotizing fasciitis of the perineal and genital regions characterized by rapid progression, fulminant sepsis, and high mortality. Negative-pressure wound therapy (NPWT), also called vacuum-assisted closure (VAC), is becoming more commonly utilized today in the treatment of complex wounds. This study aimed to evaluate the association between NPWT/VAC and in-hospital mortality among patients with FG requiring extensive surgical debridement.
METHODS: This retrospective cohort study included 95 patients treated for FG at a tert

The use of negative-pressure wound therapy (NPWT) has expanded over the last 3 decades, paralleled and documented by an increase in research. This article discusses the evolution and current applications of NPWT in modern breast reconstruction. Negative-pressure wound therapy with instillation and dwell (NPWTi-d) technology can be used to remove infectious material, facilitate salvaging compromised tissue, and stabilize the soft-tissue environment. Published consensus NPWTi-d guidelines can aid in treatment selection and implementation of this new technology. The therapeutic approach of simult
