An 11-year-old male neutered domestic shorthair cat underwent investigations for right mandibular and prescapular lymph node enlargement and weight loss. Cytology of the mandibular and prescapular lymph nodes revealed marked pyogranulomatous inflammation and mild neutrophilic, non-septic inflammation, respectively. Excisional biopsy of both lymph nodes was performed. Histopathology of the mandibular lymph node revealed severe pyogranulomatous inflammation, with prominent Splendore-Hoeppli phenomenon. Oropharyngeal Kingella kingae DNA was identified in 16 of 16 children with proven K. kingae bone and joint infection (BJI) (sensitivity 100%; 95% confidence interval: 79-100) and in 33 of 112 with disproven K. kingae BJI (specificity 71%; 95% confidence interval: 61-79). Due to low specificity, we advocate against oropharyngeal K. kingae DNA testing as a diagnostic tool in children suspected of BJI.

BACKGROUND: Although Kingella kingae is recognized as an important pediatric pathogen, our knowledge of the virulence factors involved in the invasion of specific host's tissues is limited. Outbreaks of K kingae infections in daycare centers represent natural experiments in which a single virulent strain, introduced into a cohort of susceptible young children, causes multiple infections. An epizootiologic study was conducted for infectious bovine keratoconjunctivitis (IBK) on a large grassland farm rearing a total of 1,300-1,400 heifers during May to October in both 1991 and 1992. A patient with acute leukaemia developed a maxillary sinusitis and septicaemia with Branhamella catarrhalis during the period of bone marrow hypoplasia. One blood culture also grew Staphylococcus aureus. In the same period, the patient developed tender red skin lesions and disseminated intra-vascular coagulation. The patient recovered when therapy with high dosages of amoxicillin was instituted. At the same time, however, the bone marrow recovered.
