Neonatal sepsis: a 6-year analysis in a neonatal care unit in Taiwan.

BACKGROUND: Neonatal sepsis is the most serious problem in neonatal intensive care, resulting in significant morbidity and mortality. We evaluated the causative pathogen, drug sensitivity, hematological parameters, clinical course and mortality rate of neonatal sepsis in a Taiwanese medical center and compared our results to those of previous studies conducted in Taiwan. METHODS: Neonates admitted to the neonatal intensive care unit (NICU) at National Taiwan University Hospital (NTUH) between January 2001 and December 2006 were included in this study. Patients were divided into early-onset se

Practice variation in suspected neonatal sepsis: a costly problem in neonatal intensive care.

OBJECTIVE: The most common admission to intensive care nurseries is the infant with suspected neonatal sepsis. To determine the clinical practice of neonatologists with respect to this diagnosis, we examined a large neonatal database during a 2-year period of time. The goal of this study was to define whether there were optimal practice strategies that could identify a "benchmark" clinical approach for this diagnosis. DESIGN: The PROACT database of ParadigmHealth was examined for all term infants with an admitting ICD - 9 code for suspected neonatal sepsis between January 1, 2001 and December

Neonatal bacterial sepsis in a neonatal intensive care unit: a 5 year analysis.

OBJECTIVE: To study the pattern of neonatal sepsis in a neonatal intensive care unit (NICU) during a 5 year period and assess the relationship between maternal risk factors and early onset sepsis (EOS). METHODOLOGY: The study reported here was a retrospective analysis of 209 episodes of septicaemia and 5 episodes of bacterial meningitis in 198 newborn infants, 22 of whom died. Eighty-one infants had EOS (< or = 72 h) and 117 infants had late onset sepsis (LOS > 72 h). All infants had clinical evidence of sepsis, a computerized haematological score for sepsis of 4 or greater, and either treatm

Differences between congenital-syphilis presenting as sepsis and neonatal sepsis: A case-control study.

Congenital syphilis (CS) can cause serious impact on the fetus. However, congenital syphilis presenting as sepsis is a critical condition but hardly identified by the clinic for the first time. In this study, we aimed to identify the benefit of earlier and accurate diagnosis for the infants who suffer congenital syphilis presenting as sepsis.A retrospective study was performed with patients diagnosed of congenital syphilis presenting as sepsis who were the inpatients in the West China Second Hospital between 2011 and 2018. The control group was collected in the neonatal sepsis patients whose b

The neonatal SOFA score in very preterm neonates with early-onset sepsis.

BACKGROUND: Sepsis is associated with increased neonatal morbidity and mortality. The Neonatal Sequential Organ Failure Assessment (nSOFA) score was developed as a tool to assess neonates with late-onset sepsis (LOS). Absolute values and changes in nSOFA scores are associated with mortality in LOS. Few studies have investigated the nSOFA score in neonatal early-onset sepsis (EOS). This study assesses the relationship of the nSOFA score with morbidity in preterm neonates with EOS. METHODS: Retrospective, single-center, cohort study. nSOFA scores were determined for very preterm neonates at sev

Neonatal sepsis, bacterial isolates and antibiotic susceptibility patterns among neonates.

Data from hospital records of 96 neonates hospitalised with sepsis were analysed using SPSS 11.5 version to identify sepsis--its signs and symptoms with which they were admitted, bacterial isolates and antibiotic susceptibility patterns among neonates admitted during 2007-2009. The retrospective data revealed that majority of the neonates 61 (63.5%) were males. Of the 96 neonates 52 (54.2%) were pre-term, and 44 (45.8%) were referred from various institutes after initial trial of management for the same. Majority ie. 66 (68.8%) had respiratory distress. Lethargy was noted in 56 (58.30%), fever

Neonatal cerebral sinovenous thrombosis: an uncommon complication of fungal sepsis in a neonate.

A term neonate with no significant perinatal events presented to us with sudden onset seizures and altered sensorium in the second week of life. The seizures were initially managed with antiepileptic drugs and antibiotics for suspected sepsis. The initial investigations did not reveal meningitis. However, the blood culture grew Candida tropicalis A neurosonogram was suggestive of echogenic foci in bilateral parasagittal regions. Further neuroimaging was suggestive of thrombosis of multiple major cortical veins and dural sinuses. Keeping the possibility of Cerebral Sinovenous Thrombosis (CSVT),

Innate immune training in the neonatal response to sepsis.

Neonates, especially those born prematurely, are highly vulnerable to infection-induced mortality. Numerous observational and immunological studies in newborns have shown that live attenuated vaccines have beneficial, non-specific effects (NSEs) against secondary infections to unrelated pathogens. These beneficial effects have been attributed to trained immunity, and emergency granulopoiesis plays an essential role. However, trained immunity has been shown to affect multiple myeloid subsets and how trained immunity influences the host protective response is still undefined. Here we show that B

Probiotic biogeography and sepsis prevention in the neonatal intestine.

Neonatal infection is one of the leading causes of neonatal morbidity and mortality worldwide, particularly in those born prematurely or with low birth weight. Probiotic bacteria have been demonstrated to protect against the development of neonatal intestinal dysbiosis and are widely used in peri- and post-natal clinical settings. However, formulations and efficacy are highly variable, highlighting a critical gap in the current understanding of the mechanistic underpinnings of successful probiotic interventions in this population. Furthermore, current studies on probiotic efficacy largely rely

Pentoxifylline for treatment of sepsis and necrotising enterocolitis in neonates.

BACKGROUND: Mortality and morbidity due to neonatal sepsis and necrotising enterocolitis (NEC) remain high despite the use of potent antimicrobial agents. Agents that modulate inflammation may improve outcomes. Pentoxifylline (PTX), a phosphodiesterase inhibitor, is one such agent. This is an update of a review first published in 2003 and updated in 2011 and 2015. OBJECTIVES: To assess the effectiveness and safety of intravenous PTX as an adjunct to antibiotic therapy on mortality and morbidity in neonates with suspected or confirmed sepsis and neonates with NEC. Diagnosis of Neonatal Sepsis: The Role of Inflammatory Markers.

This is a narrative review on the role of biomarkers in the diagnosis of neonatal sepsis. We describe the difficulties to obtain standardized definitions in neonatal sepsis and discuss the limitations of published evidence of cut-off values and their sensitivities and specificities. Maternal risk factors influence the results of inflammatory markers as do gestational age, the time of sampling, the use of either cord blood or neonatal peripheral blood, and some non-infectious causes. Current evidence suggests that the use of promising diagnostic markers such as CD11b, CD64, IL-6, IL-8, PCT, and

Candida pelliculosa sepsis in a neonate: a case report.

Candida pelliculosa is a rare fungal cause of neonatal sepsis. Premature and very low birthweight infants are at especially high risk of neonatal fungal infections. There have been no reports of C. pelliculosa infection in Anhui Province, China. Here, we report a case of C. pelliculosa fungemia in a newborn boy admitted 30 minutes after delivery with grunting, cyanosis, and asphyxia. C. pelliculosa was identified as the causative organism using blood culture, DNA sequencing, and mass spectrometric analysis. After 20 days of fluconazole therapy, the patient's symptoms stabilized. Together with

The Problem of Microbial Dark Matter in Neonatal Sepsis.

Neonatal sepsis (NS) kills 750,000 infants every year. Effectively treating NS requires timely diagnosis and antimicrobial therapy matched to the causative pathogens, but most blood cultures for suspected NS do not recover a causative pathogen. We refer to these suspected but unidentified pathogens as microbial dark matter. Given these low culture recovery rates, many non-culture-based technologies are being explored to diagnose NS, including PCR, 16S amplicon sequencing, and whole metagenomic sequencing. However, few of these newer technologies are scalable or sustainable globally. To reduce

Nontypeable Haemophilus Influenzae Sepsis in a Term Neonate.

We reported a term newborn case of early onset sepsis caused by nontypeable Haemophilus Influenzae (NTHi) with massive bacterial invasion in the placenta. Based on the consistent results of maternal placental pathology and neonatal bacterial culture, we diagnosed this as vertical transmission of NTHi via vaginal delivery. In general, NTHi infections occur in preterm infants, and our term infant case is very unusual. In conclusion, clinicians should consider NTHi as a cause of neonatal sepsis, even in term infants.
