Neostigmine myopathy is a calcium ion-mediated myopathy initially affecting the motor end-plate.

Morphological techniques were used to determine the acute and chronic effects of neostigmine on rat muscles. Transient calcium deposits, eliminated by prior treatment of sections with ethyleneglycol bis (aminoethylether) tetracetate (EGTA), were independent of fiber type and found at sites corresponding to neostigmine-induced focal lesions. The dimension and number of focal lesions and calcium deposits gradually decreased with chronic drug treatment. Size, shape, and density of the calcium deposits varied. Alterations in the motor nerve terminal, synaptic space, and junctional fold persisted e

Sugammadex, neostigmine, and postoperative pulmonary complications: protocol of the SNaPP multicentre randomised controlled trial.

BACKGROUND: Sugammadex, a novel cyclodextrin reversal agent, reduces the incidence of residual neuromuscular block compared with neostigmine and was associated with fewer postoperative pulmonary complications in a systematic review of small randomised trials. However, evidence from a large RCT is required. METHODS: We designed an international, multicentre RCT (the SNaPP study). A total of 3500 patients, aged ≥40 yr and undergoing abdominal or thoracic surgery, will be enrolled and randomly allocated in a 1:1 ratio to receive sugammadex or neostigmine for reversal of neuromuscular block, stra

Suspected Neostigmine-Associated Bronchospasm Complicated by Pulmonary Edema During General Anesthesia: A Case Report.

Neostigmine inhibits cholinesterase activity and increases acetylcholine at the neuromuscular junction. Neostigmine is thus administered as an antagonist for non-depolarizing muscle relaxants during general anesthesia. However, neostigmine-induced pulmonary edema is extremely rare, making its diagnosis and appropriate treatment in the perioperative period challenging for anesthesiologists. A middle-aged obese woman underwent surgery for ptosis under general anesthesia because she was allergic to local anesthetics. At the end of surgery, she developed severe hypoxia (peripheral oxygen saturatio

Noncardiogenic Pulmonary Edema After Neostigmine Administration During Emergence From General Anesthesia: A Case Report.

We report the case of a previously healthy 27-year-old man who developed acute pulmonary edema shortly after receiving neostigmine-atropine for neuromuscular blockade reversal during appendectomy under general anesthesia. Before extubation, he developed severe hypoxemia, increased airway pressures, pink frothy tracheal secretions, and bilateral diffuse pulmonary infiltrates on chest radiography. Cardiogenic pulmonary edema, fluid overload, negative-pressure pulmonary edema, and perioperative hypersensitivity were considered unlikely based on the clinical findings, bedside cardiac ultrasound, l

Regional differences in neostigmine-induced contraction and relaxation of stomach from diabetic guinea pig.

Delayed gastric emptying and autonomic neuropathy have been documented in patients with diabetes mellitus. Some medications used to treat delayed gastric emptying enhance release of acetylcholine from autonomic neurons to strengthen gastric contractions. Autonomic coordination among gastric regions may be altered in diabetes resulting in poor outcomes in response to prokinetic drugs. Fundus, antrum, and pylorus from STZ or control guinea pigs were treated with neostigmine to mimic release of acetylcholine from autonomic neurons by prokinetic agents. In diabetic animals, neostigmine-induced con

Investigation of potential neuropharmacological activity of neostigmine-glycopyrrolate for intraoperative neural monitoring in thyroid surgery.

Intraoperative neuromonitoring (IONM) is frequently used in thyroid surgery to reduce recurrent laryngeal nerve injury. The use of neuromuscular blockade agent to facilitate tracheal intubation, is a common cause of IONM failure. We performed a retrospective analysis to assess the efficacy of neostigmine-glycopyrrolate as a neuromuscular blockade reversal agent for IONM during thyroid surgery. Rocuronium (0.6 mg/kg) was administered for muscle relaxation. Neostigmine (2 mg) and glycopyrrolate (0.4 mg) were administered immediately after intubation. Retrospective Analysis of the Safety and Efficacy of Sugammadex Versus Neostigmine for the Reversal of Neuromuscular Blockade in Children.

BACKGROUND: Sugammadex, with its novel mechanism of action of encapsulation and noncompetitive binding of aminosteroid neuromuscular-blocking agents (rocuronium and vecuronium), may offer distinct advantage to pediatric patients where residual neuromuscular blockade may be poorly tolerated. Data describing its use in the pediatric population are limited, and no large-scale studies are available evaluating the occurrence of adverse event across the full spectrum of ages. We sought to measure the occurrence of adverse events, assess the severity and clinical significance of the events, and quant

Safety and Efficacy of Intermittent Bolus and Continuous Infusion Neostigmine for Acute Colonic Pseudo-Obstruction.

PURPOSE: To compare clinical response of intermittent bolus versus continuous infusion neostigmine for acute colonic pseudo-obstruction (ACPO). Acute colonic pseudo-obstruction occurs due to reduced colonic parasympathetic activity. Neostigmine is an acetylcholinesterase inhibitor that increases frequency of smooth muscle contraction by increasing acetylcholine at autonomic nervous system synapses. Although these administration modalities have been studied separately, they have never been compared. METHODS: This retrospective study compared bolus versus continuous infusion neostigmine for ACP

Sepsis Strengthens Antagonistic Actions of Neostigmine on Rocuronium in a Rat Model of Cecal Ligation and Puncture.

BACKGROUND: The antagonistic actions of anticholinesterase drugs on non-depolarizing muscle relaxants are theoretically related to the activity of acetylcholinesterase (AChE) in the neuromuscular junction (NMJ). However, till date the changes of AChE activity in the NMJ during sepsis have not been directly investigated. We aimed to investigate the effects of sepsis on the antagonistic actions of neostigmine on rocuronium (Roc) and the underlying changes of AChE activity in the NMJ in a rat model of cecal ligation and puncture (CLP). METHODS: A total of 28 male adult Sprague-Dawley rats were r

A comparison between chronotropic effects of neostigmine and edrophonium in isolated guinea pig right atrium.

Bradycardia is one of the inevitable and undesirable responses when the muscle weakness induced by nondepolarizing muscle relaxants is reversed by AChE inhibitors. The current study was designed to compare the bradycardiac effects of the two AChE inhibitors used widely in clinical anesthesia, neostigmine and edrophonium. Isolated, spontaneously beating guinea pig right atrial preparations were used as the experimental model, and in some cases, electrical field stimulation was utilized to stimulate parasympathetic nerve terminals within the atria. Neostigmine decreased the spontaneously beating

Neostigmine in the treatment of snake accidents caused by Micrurus frontalis: report of two cases (1).

Antivenom in order to be effective in the treatment of coral snake accidents must be injected very soon after the bite owing to the rapid rate of absorption of the venom neurotoxins. As this is not always possible, other forms of treatment besides serotherapy must be employed to avoid asphyxia and death. Neostigmine and artificial respiration are used for this purpose. Neostigmine restores neuromuscular transmission if the venom-induced blockade results from a reversible interaction of its neurotoxins with the end-plate receptors. This is the mechanism of the neuromuscular blockade produced by

[A double-blind study of neostigmine versus placebo in paralytic ileus as a result of surgical interventions].

A fully randomized double-blind study, stratified according to diagnosis, was carried out in order to assess the effect of endonasal neostigmine in the treatment of post-laparotic paralytic ileus. A total of 40 patients (16 M, 24 F), aged between 22 and 76 years old, were admitted to the study; of these 20 were cholecystectomized and 20 had undergone emergency surgery. According to a special randomization list, 10 patients from each pathological group were treated with 6% neostigmine en (1 puff = 5.4 mg) and the other ten were treated with placebo. Both treatments were administered at a dose o
