Stone scattering during percutaneous nephrolithotomy: role of renal anatomical characteristics.

The purpose of this study was to evaluate how the upper calyx-lower calyx infundibular (ULI) angle influences intrarenal stone migration during percutaneous nephrolithotomy (PCNL) in patients with a solitary renal pelvis stone and significant hydronephrosis. 50 adult patients with a solitary renal pelvis stone larger than 20 mm were considered for PCNL with a pneumatic lithotriptor for stone fragmentation. Inclusion criteria were moderate to severe hydronephrosis and upper calyx infundibular width >10 mm, and access point was the lower calyx in all cases. The ULI angle as well as stone fragmen

Fate of residual stones after percutaneous nephrolithotomy: a critical analysis.

PURPOSE: To analyze the fate of residual stones after percutaneous nephrolithotomy (PCNL) and identify the factors that predict spontaneous passage. PATIENTS AND METHODS: We retrospectively analyzed the records of 2469 patients who underwent PCNL at our center between January 2000 to January 2008. RESULTS: Residual fragments (RF) were identified in 187 (7.57%) patients. The most common site of RF was lower calix (57.7%), and the mean size of RF was 38.6 +/- 52 mm(2). Eighty-four stones passed spontaneously at a mean follow-up of 24 months (range 1-100 mos). Of the stones that passed spontane

Management of colonic perforation during percutaneous nephrolithotomy in horseshoe kidney.

Few cases of colonic injury during percutaneous nephrolithotomy (PCNL) have been reported in orthotopic kidneys and none in horseshoe kidney, and the management protocol has not been standardized. A plain film on postoperative day 1 following PCNL showed contrast medium in the descending colon, leading to the diagnosis of colonic injury in a 53-year-old male patient with horseshoe kidney and multiple bilateral stones. He was asymptomatic and was treated successfully by minimally invasive techniques. An asymptomatic patient with a colonic injury following PCNL can be treated by minimal manipula

Gas-containing renal stones treated with percutaneous nephrolithotomy: case report.

We present a rare case of E. coli emphysematous pyelonephritis with sepsis. The radiologic presentation consisted of multiple radiolucent gas-filled, free-floating uric acid calculi in a hydronephrotic renal pelvis. The infection was treated by intravenous fluids and antibiotics and percutaneous nephrostomy drainage. The patient was rendered stone free by percutaneous nephrolithotomy and ultrasound lithotripsy.

Almost totally tubeless percutaneous nephrolithotomy: further evolution of the technique.

BACKGROUND AND OBJECTIVES: There is renewed interest in the concept of foregoing placement of the postoperative nephrostomy tube (PNT) after percutaneous nephrolithotomy (PCNL) with the intent of reducing postoperative discomfort and hospital stay. We have omitted the PNT and placed an internal ureteral stent or externalized ureteral catheter after PCNL in selected patients. We reviewed our experience in order to assess the efficacy and safety of this practice. PATIENTS AND METHODS: Primary PCNL was performed in 26 renal units in 21 patients (5 bilateral PCNL, 4 of which were simultaneous) by

The Hickman peel-away sheath: alternative for pediatric percutaneous nephrolithotomy.

Percutaneous nephrolithotomy presents a therapeutic challenge in children because of the disproportion between the sizes of the instruments and the kidneys. A technique for pediatric nephrolithotomy used on a 2-year-old female child is presented. The patient was born prematurely and developed kidney stones as a complication of furosemide therapy. She failed medical management with hydrochlorothiazide, and the stone did not disintegrate following extracorporeal shockwave lithotripsy (ESWL). A Chiba needle was used to access the renal collecting system percutaneously. Using a guidewire, sequenti

Massive haemorrhage presenting as a complication after percutaneous nephrolithotomy (PCNL).

Authors describe and discuss cases of massive haemorrhage after percutaneous nephrolithotomy. In 10 (0.3%) of 3080 patients who underwent PCNL massive haemorrhage from renal fistula drain or urinary bladder was observed. Renal arteriography was performed in this group of patients which revealed arteriovenous fistulae in 4 patients and aneurysms of renal artery branches in 2. In 4 patients arteriography did not reveal haemorrhage. In one patient nephrectomy was necessary to stop the haemorrhage.

Navigating Entire Collecting System During Supine Percutaneous Nephrolithotomy: Is Rigid Nephroscopy Enough? A Prospective Study by International Alliance of Urolithiasis Supine Percutaneous Nephrolithotomy Working Group.

Background: During percutaneous nephrolithotomy (PCNL), accessibility to the entire collecting system is crucial to check the presence of any residual stone fragments. In this study, we aimed to identify the rate of accessibility of all caliceal cavities using lower-, middle- and, upper-pole punctures and the eventual benefit of simultaneous utilization of retrograde/antegrade flexible nephroscopy. Materials and Methods: Data of patients undergone supine PCNL in five different institutions were collected prospectively. Access status to other poles of the kidney with a rigid nephroscope, antegr

Impact of percutaneous access point number and location on complication and success rates in percutaneous nephrolithotomy.

OBJECTIVES: Percutaneous nephrolithotomy (PNL) is sometimes associated with complications, especially in patients with complex stones. Herein, we review our experience with PNL to determine the impact of percutaneous access number and location on success and complication rates. PATIENTS AND METHODS: During a 2-year period, a total of 275 patients with a mean age of 42.3 +/- 14.8 (range: 13-75) years underwent PNL. Stones were classified as simple in 51.6%, and complex (staghorn calculi or renal pelvis stones coexisting with caliceal stones) in 48.4%. Percutaneous access was done under C-armed

Safety and complications of upper pole puncture in supine percutaneous nephrolithotomy.

OBJECTIVE: Supine percutaneous nephrolithotomy (PCNL) has gained attention as a viable alternative to the traditional/historical prone approach for the treatment of large renal stones. However, concerns remain regarding the safety and feasibility of upper calyx punctures in the supine position, particularly for upper-pole access. This multicenter, retrospective cohort study evaluated the safety and stone-free rates of upper-pole punctures in supine PCNL, comparing outcomes with middle and lower calyx punctures and with literature on prone PCNL. METHODS: This retrospective study analyzed supin

Effects of Percutaneous Nephrolithotomy on Renal Morphology and Arterial Vascular Resistance.

Introduction Urolithiasis is the most common disorders seen in urology clinics around the globe, leading surgeons to adopt percutaneous nephrolithotomy (PCNL) as the standard approach for treating large stones. However, data about how this procedure alters kidney structure and blood flow, particularly arterial resistance, remain surprisingly scarce. The present study therefore set out to measure the effects of PCNL on renal anatomy and vascular resistance by means of grey scale and color Doppler ultrasound. Methodology At Mahatma Gandhi Medical College and Research Institute, Puducherry, a cro

Safety and Effectiveness of Externalized Ureteral Catheter in Tubeless Percutaneous Nephrolithotomy.

UNLABELLED: Constipation is a common gastrointestinal disorder affecting approximately 15% of the population, with a higher prevalence in women and older adults. It can be classified as primary, including functional constipation, pelvic floor dysfunction, and slow transit, or secondary, resulting from neurological, endocrine, metabolic disorders, cancer, or medications. In individuals over 75, constipation ranks as the seventh most common condition in primary care. A severe complication, fecal impaction, occurs more frequently in older adults and accounts for 0.03% of emergency department visi

Extraperitoneal laparoscopy-assisted percutaneous nephrolithotomy in a patient with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) patients represent a challenge to all physicians, as they do for anesthetists and urologists, when they develop symptomatic stones in the urinary tract. We recently treated an OI patient with renal pelvic stone by extraperitoneal laparoscopy-assisted percutaneous nephrolithotomy (PCNL). To our knowledge, this combined treatment modality has not been reported previously in OI. An 18-year-old paraplegic girl with OI presented to our urology department because of right-sided flank pain. She pointed out that she had right kidney stone for the previous 2 years, and beca
