Median nerve conduction studies and wrist magnetic resonance imaging in acromegalic patients with carpal tunnel syndrome.

PURPOSE: Carpal tunnel syndrome (CTS) often occurs with acromegaly; however, the pathophysiology of CTS in acromegalic patients remains unclear. This study evaluated the median nerve in acromegalic patients with and without CTS. METHODS: We examined the median nerves of 21 acromegalic patients (eight patients with CTS and 13 patients without CTS) using electrophysiological nerve conduction studies and wrist magnetic resonance images. They underwent transsphenoidal surgery to resect their growth hormone-secreting pituitary adenomas. The median nerves of the patients with CTS were reassessed by

Sonography versus nerve conduction studies in patients referred with a clinical diagnosis of carpal tunnel syndrome.

PURPOSE: To compare the diagnostic value of high-resolution ultrasound (US) with nerve conduction studies (NCS) in patients with clinically defined carpal tunnel syndrome (CTS). METHODS: A prospective study was conducted on 66 consecutive patients investigated for sensory hand symptoms. The gold standard was the clinical diagnosis of CTS. RESULTS: NCS showed greater diagnostic sensitivity (82%) than US (62%) in supporting a diagnosis of CTS. With increasing neurophysiologic severity of median neuropathy, there was increasing convergence of the two test methods. Abnormal US as the only diagno

Nerve Conduction Studies Correlation with Stage of Chronic Kidney Disease in Nondiabetic Chronic Kidney Disease Patients.

BACKGROUND: Peripheral neuropathy (PN), collectively termed uremic neuropathy, is one of the most prevalent neurological complications of chronic kidney disease (CKD). Nerve conduction studies (NCS) remain the gold standard for objective electrophysiological assessment of PN. There is limited Indian data examining NCS parameters across CKD stages in a nondiabetic population, where uremia can be studied without the confounding effect of hyperglycemia. METHODOLOGY: This cross-sectional observational study enrolled 103 nondiabetic patients with CKD Stage 3, 4, and 5 according to Kidney Disease:

Electrophysiological asymmetry in vincristine-exposed children with acute lymphoblastic leukemia: Evidence from bilateral nerve conduction studies.

Vincristine-induced peripheral neuropathy (VIPN) is a frequent complication of therapy for acute lymphoblastic leukemia (ALL) in children. Beyond acute toxicity, VIPN may affect motor development, balance, and quality of life in survivors. Although often assumed to be symmetric, the extent of electrophysiological asymmetry has not been systematically evaluated. This study aimed to quantify side-to-side differences in vincristine-exposed children using bilateral nerve conduction studies (NCS). Forty-seven bilateral NCS assessments were performed in 47 children with ALL (32 post-treatment surviv

Utilizing peroneal nerve conduction studies to differentiate L5 radiculopathy and peripheral neuropathies of the lower extremity.

BACKGROUND: L5 radiculopathy, peroneal neuropathy at the fibular head (PNFH), and sciatic neuropathy (SN) often share similar clinical features, such as foot drop. The ability of peroneal nerve conduction studies (NCSs) to distinguish these three conditions was investigated. METHODS: NCS findings of the patients with L5 radiculopathy, PNFH and SN were reviewed retrospectively. Additionally, needle electromyography (EMG) findings of the tibialis anterior (TA) and peroneus longus (PL) muscles were analyzed. RESULTS: Sixty-six patients with L5 radiculopathy, 53 patients with PNFH, and 75 patien

Relationship between nerve conduction studies and the Functional Dexterity Test in workers with carpal tunnel syndrome.

BACKGROUND: Dexterity impairments caused by carpal tunnel syndrome (CTS) make working and daily activities challenging. We aimed to investigate: i) the relationship between dexterity and nerve conduction studies (NCS) in workers with classic symptoms presentation; ii) the ability of the Functional Dexterity Test (FDT) to discriminate different levels of CTS severity as classified by NCS; iii) the diagnostic accuracy of a clinical battery composed of the FDT, Phalen's test and Tinel's sign. METHODS: In a convenience sample of individuals diagnosed with CTS, we correlated FDT net scores with th

A review of nerve conduction studies in cases of suspected compression neuropathies of the upper limb.

INTRODUCTION: Entrapment neuropathies, particularly those affecting upper limbs, are common reasons for referral for nerve conduction studies (NCS). However, concordance between clinical findings and NCS findings, especially in patients being considered for intervention including decompressive surgery, has not been assessed. METHODS: We conducted a retrospective study using records from a tertiary referral centre's neurophysiology database. We aimed to establish the proportions of agreement between the suspected clinical diagnosis as defined by the referring clinician and NCS findings in the

Normal motor nerve conduction studies using surface electrode recording from the supraspinatus, infraspinatus, deltoid, and biceps.

OBJECTIVE: Proximal peripheral nerve conduction studies can provide useful information to the clinician. The difficulty of measuring the length of the proximal nerve as well as a frequent inability to stimulate at 2 points along the nerve adds a challenge to the use of electrodiagnosis for this purpose. The purpose of this article is to present normal values for the suprascapular, axillary, and musculocutaneous nerves using surface electrodes while accounting for side-to-side variability. DESIGN: Prospective, observational study. SETTING: Patients were evaluated in outpatient, private practi

[Sequential studies of motor nerve conduction in relation to the disease stages of Guillain-Barré syndrome].

We retrospectively studied sequential changes in motor nerve conduction in forearm segments of 17 median nerves in 9 consecutive patients with Guillain-Barré syndrome (GBS). The disease course was divided into 6 stages: progressing, nadir, early-, mid- and late-convalescent, and cured stages. All patients were tested on at least 3 occasions in each different stage. Low amplitude compound muscle action potentials (CMAP) were noted in the progressing and nadir stages, suggesting the presence of conduction block. In the early- and mid-convalescent stages, on the other hand, the delays in distal m

Sensory conduction study of the infrapatellar branch of the saphenous nerve.

Although neuropathies of the infrapatellar nerve (infrapatellar branch of the saphenous nerve, IPBSN) have been reported clinically, no electrophysiological method has been defined to evaluate IPBSN conduction. We therefore studied a total of 60 saphenous nerves and 60 IPBSNs from 36 volunteers. The IPBSN was stimulated medially with a surface electrode 2 cm below the patella. The response was recorded with a needle electrode located close to the nerve 1 cm lateral to the femoral artery in the inguinal region. Sensory nerve action potentials were obtained from each subject; mean latency of the

Technique for studying conduction in the lateral cutaneous nerve of calf.

We describe a novel technique for assessing conduction in the lateral cutaneous nerve of the calf (LCNC), a branch of the common peroneal nerve, based on a study of 32 healthy subjects. Both antidromic and orthodromic techniques were used in each of the 64 limbs to obtain a sensory nerve action potential (SNAP) of the LCNC over a distance of 12 cm. In 60 limbs (93.7%) a SNAP was obtainable with either the antidromic or orthodromic technique. In 21 limbs (32. 8%), the SNAP was obtained both antidromically and orthodromically. In 33 limbs (51.6%), the SNAP was obtained only antidromically, and i

Early diagnosis in Bell's palsy. A nerve conduction study.

In 100 consecutive patients affected by idiopathic facial palsy the final degree of recovery was found to be inversely related to the distal latency time (DLT) measured within 72 hours of the onset of the disease. Incomplete recovery occurred in 77% of patients in whom the DLT was prolonged more than 10.0 msec, while complete recovery was found in 92% of cases in whom the DLT was less than 6.0 msec. This inexpensive test is easily and quickly performed and does not depend on the subjective responses or the cooperation of the patients. It may fill the gap between the onset of the palsy and the

Conduction studies in peripheral cat nerve using implanted electrodes: II. The effects of prolonged constriction on regeneration of crushed nerve fibers.

Arrays of chronically implanted electrodes were used to examine the time course of elongation and maturation of peripheral nerve fibers in the cat after crush of the tibial nerve in the proximal calf. Regeneration after crush alone was compared with crush 5 mm proximal to a tight constriction of the nerve. Regeneration was monitored by the progression of excitability along the electrode arrays on the tibial and plantar nerves. The sensitivity was sufficient to record the averaged activity in single nerve fibers allowing detection of the earliest regeneration. The diameters of the fastest regen

Lateral dorsal cutaneous branch of the sural nerve. Standardization in nerve conduction study.

Lateral ankle or foot pain may have various etiologies including entrapment of or injury to the distal sural nerve or early peripheral neuropathy. Antidromic distal sural conduction studies have not been used in clinical neurophysiologic evaluation, however, for technical reasons. The purpose of this study was to determine if by modifying recording parameters and using electronic averaging adequate responses could be obtained to permit standardization of distal sural conduction. Standard surface recording electrodes with an inter-electrode separation of 37 mm were used. The distal sural nerve
