Giant planum sphenoidale meningioma: rationale for Dolenc's step in the pterional approach.

Large meningiomas of the midline anterior skull base do not respect anatomical boundaries. Therefore, a giant planum sphenoidale meningioma has the unique risk of simultaneously endangering the olfactory and visual functions. A pterional approach to such cases has several advantages, and the Dolenc's modification allows a complete extradural anterior clinoidectomy, early release of compressed optic nerve, and exposure of basal portions of the internal carotid artery with a possible operative corridor expansion. This enhances tumor resection ergonomics with a more basal angle of attack while vi

[Metastasis in the inner auditory canal].

Metastasis in the inner auditory canal (IAC), or more precisely in the pontocerebellar angle (PCA) is extremely rare. Indeed acoustic neurinoma (AN), meningioma and other types of benign neoformations, in decreasing order, account for nearly all expansive pathologies in this anatomical region. The present paper reports a clinical case of a patient with the sudden onset of an hearing loss in the right ear. This hearing loss was promptly followed by paralysis of the homolateral facial nerve. The function of both cranial nerves worsened progressively and rapidly. Cerebral radiology, performed wit

Role of Bβ1 overexpression in the pathogenesis of SCA12.

BACKGROUND: Spinocerebellar ataxia type 12 (SCA12) is a neurodegenerative disease caused by a CAG/CTG repeat expansion at the PPP2R2B locus. OBJECTIVE: We investigated how the CAG repeat expansion within the PPP2R2B 7B7D transcript influences the expression of Bβ1 and a potential protein containing a long polyserine tract. METHODS: Transcript and protein expression were measured using quantitative PCR (qPCR) and Western blot, respectively, in an SK-N-MC cell model that overexpresses the full-length PPP2R2B 7B7D transcript. The apoptotic effect of a protein containing a long polyserine tract

The Phenotypes and Mechanisms of NOTCH2NLC-Related GGC Repeat Expansion Disorders: a Comprehensive Review.

The human-specific gene NOTCH2NLC is primarily expressed in radial glial cells and plays an important role in neuronal differentiation and cortical neurogenesis. Increasing studies were conducted to verify the relationship between NOTCH2NLC gene and many neurological diseases, such as neuronal intranuclear inclusion disease, essential tremor, multiple system atrophy, Parkinson's disease, Alzheimer's disease, and even oculopharyngodistal myopathy. Thus, we support the concept, NOTCH2NLC-related GGC repeat expansion disorders (NRED), to summarize all diseases with the GGC repeat expansion in the

Massive SCA7 expansion detected in a 7-month-old male with hypotonia, cardiomegaly, and renal compromise.

Infantile spinocerebellar ataxia type 7 (SCA7) is phenotypically different from the child-onset and adult-onset cases, presenting as a multisystem disorder associated with pathologically large CAG trinucleotide repeat sequences. We describe a case study of a male who presented at 5 months of age with marked motor delay, failure to thrive, and a patent ductus arteriosus. He later developed renal failure of uncertain aetiology. The infant became progressively hypotonic, and cardiac and renal function deteriorated further; he died at the age of 11 months of multisystem failure. Family history rev

Facial Nerve Schwannoma Mimicking Post-Traumatic Middle-Ear Disease: A Diagnostic Pitfall.

Facial nerve schwannoma may present with otological symptoms despite preserved facial nerve function, creating a diagnostic challenge when the lesion involves the middle ear. A 75-year-old man developed aural fullness and hearing loss following craniofacial trauma. Otoscopy revealed a dark reddish retrotympanic mass. Computed tomography demonstrated soft-tissue opacification of the middle ear and mastoid with a tegmen tympani defect, raising suspicion of post-traumatic middle-ear disease with cerebrospinal fluid leakage. Facial function remained normal. Surgical exploration confirmed a dural d

Spinous leafy nerve endings in the feline stomach wall.

The Champy-Maillet ZIO technique has been applied to the study of the muscular tunic of the stomach. In the corpus and antrum pyloricum some nerve fibres end in non-capsulated expansions owning thorn-like protrusions. Their morphology is studied and a possibly functional sensory significance is proposed, on the basis of previous experimental results.

Pathogenic mechanisms of a polyglutamine-mediated neurodegenerative disease, spinocerebellar ataxia type 1.

Spinocerebellar ataxia type 1 (SCA1) is one of nine inherited neurodegenerative diseases caused by the expansion of a CAG trinucleotide repeat encoding a polyglutamine tract. SCA1 patients lose motor coordination and develop slurred speech, spasticity, and cognitive impairments. Difficulty with coordinating swallowing and breathing eventually causes death. Genetic evidence indicates that the disease mutation induces a toxic gain of function in the SCA1 encoded protein ATXN1. The discovery that residues in ATXN1 outside of the polyglutamine tract are crucial for pathogenesis hinted that alterat

Bilateral impact on the lung of hemidiaphragmatic paralysis in the dog.

The present study was designed to test the hypothesis that the expansion of a particular lung during breathing is partly related to the action of the hemidiaphragm on the opposite side. Two endotracheal tubes were inserted in the right and left main stem bronchi of anesthetized dogs, and the changes in pleural pressure (DeltaPpl) over the two lungs were assessed separately, first before and then after section of one phrenic nerve, by measuring the changes in airway opening pressure (DeltaPao) in the two tubes during occluded breaths. After phrenic nerve section, DeltaPao in the ipsilateral lun

A Case Report and Literature Review: Intraneural Ganglion Cyst Causing Tarsal Tunnel Syndrome.

Intraneural ganglion cysts are benign mucinous lesions that form within joints and enter adjacent nerves via an articular branch. Despite being morphologically characterized as benign, they can demonstrate considerable intrafascicular destruction and expansion, resulting in worsening compressive neuropathies or nerve injury. There have been several suggested theories of pathogenesis, but the most widely accepted articular (synovial) theory describes a capsular defect in a neighboring joint that allows joint fluid to egress and track along the epineurium of the innervating articular branch. Reversal of behavioural and electrophysiological correlates of experimental peripheral neuropathy by the NK1 receptor antagonist GR205171 in rats.

In adult rats response latencies to innocuous mechanical stimuli were found to be reduced and, in electrophysiological studies, the receptive fields of dorsal horn neurones were enlarged 7-14 days after chronic constriction injury of the sciatic nerve. The NK1 receptor antagonist GR205171 at 3 mg kg(-1) blocked responses to NK1 agonist evoked activity and reversed the mechanical hypersensitivity following nerve ligation in behavioural assays. GR205171 also reversed the receptive field expansion of spinal dorsal horn neurones caused by loose ligation of the sciatic nerve in an electrophysiologi

Adipose lesions of nerve: the need for a modified classification.

OBJECT: Adipose lesions of nerve are rare and poorly understood. Their current classification, although not universally accepted, generally includes lipomatosis of nerve with or without localized macrodactyly, and intra- as well as extraneural lipoma. The authors believe that the spectrum of these lesions and their interrelationships are not currently appreciated. They propose an adaptation to the existing framework to illustrate the expanding spectrum of adipose lesions of nerve by considering lipomatosis and lipoma singly or in combination. METHODS: Fourteen representative cases are present

Imaging the hypoglossal nerve.

The hypoglossal nerve is a pure motor nerve. It provides motor control to the intrinsic and extrinsic tongue muscles thus being essential for normal tongue movement and coordination. In order to design a useful imaging approach and a working differential diagnosis in cases of hypoglossal nerve damage one has to have a good knowledge of the normal anatomy of the nerve trunk and its main branches. A successful imaging evaluation to hypoglossal diseases always requires high resolution studies due to the small size of the structures being studied. MRI is the preferred modality to directly visualiz

A Comparative Optical Coherence Tomography Study of Spinocerebellar Ataxia Types 3 and 10.

SCA3 presents with a CAG expansion at 14q24.3-q32 while SCA10 shows an ATTCT expansion at 22q13-qter. SCA10 seems to be less aggressive than SCA3. For an in vivo, noninvasive approach of the correlation between central nervous system and clinical evolution, we can use optic coherence tomography (OCT) to measure retinal nerve fiber (RNFL) and ganglion cell layer (GCL) thickness. To describe OCT findings in SCA10, correlate it with expansion size and disease severity and compare with those of SCA3. We analyzed ten individuals with SCA3 and nine with SCA10 recruited from the neurology service of

Customization of Soft Tissue Expanders: Improving Safety, Accuracy, and Efficiency for Treatment of Large and Complicated Skin Lesions.

BACKGROUND: Soft tissue expansion is a common technique for restoring large skin defects. Fixed-type expanders may be inappropriate for the following reasons: (1) the shapes and sizes of the defects vary in different patients; and (2) the bulged base of the fixed-type expander does not fit the curve of the human body, which may induce complications such as concave deformities or nerve palsy from continuous mechanical compression. The customized expander adjusts better to the shape and the topography of the expansion site compared with the fixed-type expander. It improves expansion efficiency a

Volume expansion potentiates cardiac sympathetic afferent reflex in dogs.

Our previous study (27) showed that the cardiac sympathetic afferent reflex (CSAR) was enhanced in dogs with congestive heart failure. The aim of this study was to test whether blood volume expansion, which is one characteristic of congestive heart failure, potentiates the CSAR in normal dogs. Ten dogs were studied with sino-aortic denervation and bilateral cervical vagotomy. Arterial pressure, left ventricular pressure, left ventricular epicardial diameter, heart rate, and renal sympathetic nerve activity were measured. Coronary blood flow was also measured and, depending on the experimental

Repair of peripheral nerve defect by direct gradual lengthening of the distal nerve stump in rats: effect on nerve regeneration.

We investigated the use of direct lengthening of the distal stump of a peripheral nerve to compare the results of nerve regeneration using the direct lengthening method with simple end-to-end suture and autografting in rats. A segment 10 mm long was resected from the rat sciatic nerve (n=18 in each group). The distal nerve stump was fixed to a ring and pulled at a rate of 1 mm/day for 20 days using an original external nerve distraction device. The results showed that the degree of nerve regeneration in the lengthened group was superior to that of the grafted group electrophysiologically and h

Optic Nerve Compression From MIRAgel Implant Migration in a Monocular Patient.

The MIRAgel implantation was popularized in the 1980s as an alternative to silicone for scleral buckle retinal detachment repair. However, long-term follow-up has revealed that the implants can expand, creating globe compression and potentially visually devastating intraocular invasion. The authors document a 33-year-old monocular male patient presenting 20 years after MIRAgel implantation with extensive expansion, posterior migration, globe compression, and possible optic nerve compression. Debulking of the MIRAgel stabilized the vision and restored affected extraocular movements. The authors

Perioperative management for one day hospital admission: regional anesthesia is better than general anesthesia.

The past ten years have showed real and considerable growth in the number and complexity of ambulatory surgeries. The remaining real problems are the postoperative pain and the adverse effects due to parenteral opioids promoting hospital readmissions and increasing costs. This events limit the expansion of outpatient surgery. Regional anesthesia techniques such as spinal anesthesia and peripheral nerve blocks are ideal techniques for one day hospital admissions surgical procedures. It is now fully demonstrated that these techniques allowed rapid and complete anesthetic blocks, a limitation of
