A computational model to design neural interfaces for lower-limb sensory neuroprostheses.

BACKGROUND: Leg amputees suffer the lack of sensory feedback from a prosthesis, which is connected to their low confidence during walking, falls and low mobility. Electrical peripheral nerve stimulation (ePNS) of upper-limb amputee's residual nerves has shown the ability to restore the sensations from the missing limb via intraneural (TIME) and epineural (FINE) neural interfaces. Physiologically plausible stimulation protocols targeting lower limb sciatic nerve hold promise to induce sensory feedback restoration that should facilitate close-to-natural sensorimotor integration and therefore wal

Micro-Coil Design Influences the Spatial Extent of Responses to Intracortical Magnetic Stimulation.

OBJECTIVE: Electrical stimulation via cortically implanted electrodes has been proposed to treat a wide range of neurological disorders. Effectiveness has been limited, however, in part due to the inability of conventional electrodes to activate specific types of neurons while avoiding other types. Recent demonstrations that magnetic stimulation from a micro-coil can selectively activate pyramidal neurons (PNs) while avoiding passing axons suggest the possibility that such an approach can overcome some this limitation and here we use computer simulations to explore how the micro-coil design in

A system and method to interface with multiple groups of axons in several fascicles of peripheral nerves.

BACKGROUND: Several neural interface technologies that stimulate and/or record from groups of axons have been developed. The longitudinal intrafascicular electrode (LIFE) is a fine wire that can provide access to a discrete population of axons within a peripheral nerve fascicle. Some applications require, or would benefit greatly from, technology that could provide access to multiple discrete sites in several fascicles. NEW METHOD: The distributed intrafascicular multi-electrode (DIME) lead was developed to deploy multiple LIFEs to several fascicles. It consists of several (e.g. six) LIFEs th

Decreasing the physical gap in the neural-electrode interface and related concepts to improve cochlear implant performance.

Cochlear implants (CI) represent incredible devices that restore hearing perception for those with moderate to profound sensorineural hearing loss. However, the ability of a CI to restore complex auditory function is limited by the number of perceptually independent spectral channels provided. A major contributor to this limitation is the physical gap between the CI electrodes and the target spiral ganglion neurons (SGNs). In order for CI electrodes to stimulate SGNs more precisely, and thus better approximate natural hearing, new methodologies need to be developed to decrease this gap, (i.e.,

Influence of nerve cuff channel count and implantation site on the separability of afferent ENG.

OBJECTIVE: Recording of neural signals from intact peripheral nerves in patients with spinal cord injury or stroke survivors offers the possibility for the development of closed-loop sensorimotor prostheses. Nerve cuffs have been found to provide stable recordings from peripheral nerves for prolonged periods of time. However, questions remain over the design and positioning of nerve cuffs such that the separability of neural data recorded from the peripheral nerves is improved. APPROACH: Afferent electroneurographic (ENG) signals were recorded with nerve cuffs placed on the sciatic nerve of r

Analysis of using EMG and mechanical sensors to enhance intent recognition in powered lower limb prostheses.

OBJECTIVE: The purpose of this study was to determine the contribution of electromyography (EMG) data, in combination with a diverse array of mechanical sensors, to locomotion mode intent recognition in transfemoral amputees using powered prostheses. Additionally, we determined the effect of adding time history information using a dynamic Bayesian network (DBN) for both the mechanical and EMG sensors. APPROACH: EMG signals from the residual limbs of amputees have been proposed to enhance pattern recognition-based intent recognition systems for powered lower limb prostheses, but mechanical sen

Design and evaluation of a bone-anchored, neurally-controlled knee prosthesis.

BACKGROUND: Commercially-available microprocessor-controlled prosthetic knees are unable to fully replicate the biomechanical function of the missing biological limb. While powered prostheses have the capacity to restore joint level kinetics, current systems rely on intrinsic control schemes that do not allow the user to volitionally modulate movement under neural commands. This limitation may compromise functional performance and hinder prosthetic embodiment, the sense that the device is part of the user’s body. In a case study on one test participant, we evaluate the functional and perceptua

A robust neural prosthetic control strategy against arm position variability and fatigue based on multi-sensor fusion.

Objective. Multi-modal sensor fusion comprising surface electromyography (sEMG) and A-mode ultrasound (US) has yielded satisfactory performance in gesture recognition, aiding amputees in restoring upper limb function. However, prior research conducted in laboratory settings with consistent arm positions lacks practical application for amputees using prostheses. Additionally, motion tests utilized in current studies necessitate prolonged gesture execution, while constant muscle contractions introduce fatigue and increase misclassification risk in practical applications. Consequently, implementi

First-in-human implementation of a bidirectional somatosensory neuroprosthetic system with wireless communication.

BACKGROUND: Limitations in upper limb prosthesis function and lack of sensory feedback are major contributors to high prosthesis abandonment rates. Peripheral nerve stimulation and intramuscular recording can restore touch and relay motor intentions for individuals with upper limb loss. Percutaneous systems have enabled significant progress in implanted neural interfaces but require chronic lead maintenance and unwieldy external equipment. Fully implanted sensorimotor systems without percutaneous leads are crucial for advancing implanted neuroprosthetic technologies to long-term community use

An Implantable System Design Based on Neuromorphic Memristor for Post-Craniotomy Intracranial Pressure Monitoring.

Craniotomy, a complex neurosurgical intervention, carries significant risks of postoperative complications including intracerebral hemorrhage (ICH) and cerebral edema, causing elevated intracranial pressure (ICP) and life-threatening cerebral herniation. However, the current ventricular catheter ICP monitoring technologies pose risks of infection and hemorrhage, and restrict patient mobility during medical procedures. There is thus an urgent need to develop ICP monitoring technologies that simultaneously achieve sensitivity, safety, and portability. Memristors, with their integrated memory, se

Proprioception from a neurally controlled lower-extremity prosthesis.

Humans can precisely sense the position, speed, and torque of their body parts. This sense is known as proprioception and is essential to human motor control. Although there have been many attempts to create human-mechatronic interactions, there is still no robust, repeatable methodology to reflect proprioceptive information from a synthetic device onto the nervous system. To address this shortcoming, we present an agonist-antagonist myoneural interface (AMI). The AMI is composed of (i) a surgical construct made up of two muscle-tendons-an agonist and an antagonist-surgically connected in seri

Myoelectric neural interface enables accurate control of a virtual multiple degree-of-freedom foot-ankle prosthesis.

Technological advances have enabled clinical use of powered foot-ankle prostheses. Although the fundamental purposes of such devices are to restore natural gait and reduce energy expenditure by amputees during walking, these powered prostheses enable further restoration of ankle function through possible voluntary control of the powered joints. Such control would greatly assist amputees in daily tasks such as reaching, dressing, or simple limb repositioning for comfort. A myoelectric interface between an amputee and the powered foot-ankle prostheses may provide the required control signals for

Deep learning-based control of electrically evoked activity in human visual cortex.

Visual cortical prostheses offer a promising path to sight restoration, but current systems elicit crude, variable percepts and rely on manual electrode-by-electrode calibration that does not scale. These limitations reflect a deeper challenge: electrical microstimulation evokes nonlinear, state-dependent population responses in the human visual cortex, complicating the link between stimulation and perception. Here, we present a deep learning framework that leverages a bidirectional cortical implant to causally shape stimulation-evoked population activity in the human visual cortex. The framew

Hand Prosthesis with Soft Robotics Technology and Artificial Intelligence for Fine Motor Control.

The development of prostheses that accurately reproduce fine motor skills remains a key challenge for daily assistance applications. This research presents the development of a soft robotic hand prosthesis prototype inspired by the natural behavior of muscles and tendons, incorporating internal vacuum-based reinforcement and textured fingertip surfaces to enhance friction and grasp adaptability, without relying on force sensors. The prosthesis reproduces open-hand and tripod pinch movements through myoelectric signals (EMG) acquired via a wearable armband equipped with eight surface electrodes

Spatial Restriction of Neural Activation Using Focused Multipolar Stimulation With a Retinal Prosthesis.

PURPOSE: The resolution provided by present state-of-the-art retinal prostheses is severely limiting for recipients, partly due to the broad spread of activation in the retina in response to monopolar (MP) electrical stimulation. Focused multipolar (FMP) stimulation has been shown to restrict neural activation in the cochlea compared to MP stimulation. We extended the FMP stimulation technique to a two-dimensional electrode array and compared its efficacy to MP and hexapolar (HP) stimulation in the retina. METHODS: Normally-sighted cats (n = 6) were implanted with a suprachoroidal electrode a

Stimulation of a Suprachoroidal Retinal Prosthesis Drives Cortical Responses in a Feline Model of Retinal Degeneration.

PURPOSE: Retinal prostheses have emerged as a promising technology to restore vision in patients with severe photoreceptor degeneration. To better understand how neural degeneration affects the efficacy of electronic implants, we investigated the function of a suprachoroidal retinal implant in a feline model. METHODS: Unilateral retinal degeneration was induced in four adult felines by intravitreal injection of adenosine triphosphate (ATP). Twelve weeks post injection, animals received suprachoroidal electrode array implants in each eye, and responses to electrical stimulation were obtained u

ESI-GAL: EEG source imaging-based trajectory estimation for grasp and lift task.

BACKGROUND: Electroencephalogram (EEG) signals-based motor kinematics prediction (MKP) has been an active area of research to develop Brain-computer interface (BCI) systems such as exosuits, prostheses, and rehabilitation devices. However, EEG source imaging (ESI) based kinematics prediction is sparsely explored in the literature. METHOD: In this study, pre-movement EEG features are utilized to predict three-dimensional (3D) hand kinematics for the grasp-and-lift motor task. A public dataset, WAY-EEG-GAL, is utilized for MKP analysis. In particular, sensor-domain (EEG data) and source-domain

Safety evaluations for transtympanic laser stimulation of the cochlea in Mongolian gerbils (Meriones unguiculatus).

Infrared laser stimulation of the cochlea has been proposed as a possible alternative to conventional auditory prostheses. Whereas previous studies have focused primarily on the short-term effects of laser stimulation, the practical application of this technics requires an investigation into whether prolonged laser exposure can induce neural responses and safely. This study assessed the effect of laser-induced damage to the cochlea on auditory perception using Mongolian gerbils (Meriones unguiculatus) trained with a classical conditioning task. The broadband noise was presented as a conditione
