A 52-year-old heart-lung transplant patient presented to the emergency department with acute onset of neurologic symptoms. MRI showed ballooning of the left ventricle, midline shift and contrast enhancement in the anterior horn of the left ventricle. Ventricle neuroendoscopy revealed whitish, floccose aerial structures within the left ventricle. Brain biopsy cultures grew Rhizopus arrhizus. Therapy with liposomale amphotericin B and posaconazole was performed. Except for hemianopsia and deficits in minute motor activity, the patient completely recovered.

INTRODUCTION: Pediatric hydrocephalus is highly prevalent and therefore a major neurosurgical problem in Africa. In addition to ventriculoperitoneal shunts, which have high cost and potential complications, endoscopic third ventriculostomy is becoming an increasingly popular technique especially in this part of the world. However, performing this procedure requires trained neurosurgeons with an optimal learning curve. For this reason, we have developed a 3D printed training model of hydrocephalus so that neurosurgeons without previous experience with endoscopic techniques can acquire these ski

The development of minimally invasive neuroendoscopy has advanced in recent years. The introduction of the neuroendoscopic ultrasonic aspirator (NUA) increased the treatment spectrum of neuroendoscopy. This review aimed to present a systematic overview of the extent of resection, lesion characteristics, technical aspects, complications, and clinical outcomes related to using the NUA. Articles were identified by searching the PubMed/Medline, Embase, and Web of Science database through June 2022 with restriction to the last 20 years. We included case series, case reports, clinical trials, contro

The motion of cerebrospinal fluid (CSF) within the subarachnoid space and ventricles is greatly modulated when propagating synchronously with the cardiac pulse and respiratory cycle and path through the nerves, blood vessels, and arachnoid trabeculae. Water molecule movement that propagates between two spaces via a stoma, foramen, or duct presents increased acceleration when passing through a narrow area and can exhibit "turbulence." Recently, neurosurgeons have started to perform fenestration procedures using neuroendoscopy to treat hydrocephalus and cystic lesions. As part of the postoperati

Since its introduction in the neurosurgical field, neuroendoscopy has played an important role in the treatment of pineal germ cell tumors. The authors report a case of pineal germinoma revealing spontaneous regression before neuroendoscopic surgery. A 15-year-old boy presented with chronic headache, deterioration of his visual acuity, and diplopia. Head magnetic resonance imaging (MRI) revealed a large tumor in the pineal region and triventricular hydrocephalus due to occlusion of the aqueduct. As levels of tumor markers in serum and cerebrospinal fluid, including those of alpha-fetoprotein,

OBJECT: Frameless computerized neuronavigation has been increasingly used in intracranial endoscopic neurosurgery. However, clear indications for the application of neuronavigation in neuroendoscopy have not yet been defined. The purpose of this study was to determine in which intracranial neuroendoscopic procedures frameless neuronavigation is necessary and really beneficial compared with a free-hand endoscopic approach.
METHODS: A frameless infrared-based computerized neuronavi- gation system was used in 44 patients who underwent intracranial endoscopic procedures, including 13 third ventri

BACKGROUND: Intraventricular fungal infections are rare and associated with high morbidity and mortality. Diagnosis is often delayed due to nonspecific clinical and radiological findings, and treatment is complicated by limited cerebrospinal fluid (CSF) penetration of systemic antifungal agents. Neuroendoscopic visualization and intrathecal antifungal therapy have been reported only sporadically, and standardized management strategies are lacking. CASE PRESENTATION: We report a 75-year-old male who developed a right temporal lobe abscess caused by Cladophialophora bantiana, confirmed by histop

OBJECTIVE: Neurocysticercosis is the most common parasitosis of the central nervous system. Many forms, especially those inside the ventricles, carry a poor prognosis. Drug therapy is far from ideal.
METHODS: We propose and comment on the use of the endoscope to remove cysts, treat hydrocephalus, and to perform membrane fenestration as a way to simplify treatment.
RESULTS: Many patients, especially those with obstructive forms, benefit from the adjunctive use of neuroendoscopy.
CONCLUSIONS: Neurocysticercosis treatment remains challenging and multimodal. Endoscopes can be helpful in this se

Tuberous sclerosis is a rare, autosomal dominant neurocutaneous disease. The disease can also result from spontaneous mutations. Manifestations of this disease are typically characterized by involvement of the CNS (early childhood seizures, multiple brain tumours), skin (facial angiofibromas, Pringle's birthmarks), kidneys and liver (angiomyolipomas), heart and others. We present a case of 26 years old patient with brain tumour, with clinical and radiological signs of hydrocephalus due to a cystic lesion in the III ventricle. The patient was operated upon using neuroendoscopy, with clinical an

BACKGROUND: Brainstem hemorrhage accounts for a relatively small proportion of spontaneous intracerebral hemorrhages (∼10 %) but tends to occur earlier in life and has poorer prognosis. Numerous studies support the therapeutic potential of minimally invasive hematoma evacuation for intracerebral hemorrhage; however, there have been few assessments of the benefits for brainstem hemorrhage.
METHODS: We evaluated the safety and efficacy of a minimally invasive approach under neuroendoscopic guidance with pneumatic arm fixation for removing the hematoma in severe brainstem hemorrhage patients. 14

BACKGROUND: Endoscopic third ventriculostomy (ETV) has become the procedure of choice in the treatment of obstructive hydrocephalus. In certain cases, standard ETV might not be technically possible or may engender significant risk.
OBJECTIVE: To present an alternative through the lamina terminalis (LT) by a transventricular, transforaminal approach with flexible neuroendoscopy and to discuss the indications, technique, neuroendoscopic findings, and outcomes.
METHODS: Between 1994 and 2010, all patients who underwent endoscopic LT fenestration as an alternative to ETV were analyzed and prospe

INTRODUCTION: The field of neuroendoscopy is rapidly expanding with increasing indications for endoscopic third ventriculostomy (ETV).
DISCUSSION: As a treatment for hydrocephalus, ETV has the advantage of providing a more physiological cerebrospinal fluid diversion without shunt hardware which reduces the risk of recurrent infection and malfunction. The success rate of ETV has been increasing with decreasing morbidity and mortality.
CONCLUSION: Originally, ETV was indicated for cases of obstructive hydrocephalus, however the indications are expanding. To highlight this, we present a small s

INTRODUCTION: Neuroendoscopy is a rapidly growing field, but the risk of intraventricular hemorrhage is signficant. There are few novel ways of obtaining hemostasis for major hemorrhage described in the literature.
PATIENT: A 5-year-old boy with a history of intraventricular hemorrhage and multi-cystic hydrocephalus presented with worsening brainstem compression from dilation of a 4 (th) ventricle cyst.
RESULTS: During endoscopic surgery to fenestrate the cyst, arterial hemorrhage was emergently controlled with the hemostatic agent, FloSeal.
CONCLUSION: To the best of the authors' knowledge

INTRODUCTION: An intraventricular cysticercus cyst is observed in 7%-30% patients of neurocysticercosis (NCC). Apart from causing arachnoiditis, intraventricular NCC (IVNCC) can cause sudden death due to acute episodes of hydrocephalus. Various treatment modalities available are external cerebrospinal fluid (CSF) diversion, microsurgical removal, and endoscopic management. There is no consensus regarding the optimal surgical treatment strategy. We are presenting our experience by doing a retrospective analysis of 26 patients having endoscopic removal of IVNCC with a rigid endoscope and angioca
