Let's be more conscious about the misuse of nitrous oxide.

Following the 'No laughing matter' article published in 2020 by Dr Croser, this paper further highlights the increasingly concerning problem of the illicit use of nitrous oxide.Since 1884, the benefits of nitrous oxide have been exploited in the field of dentistry. Its analgesic and mild anaesthetic properties, alongside suggestive hypnosis and reassurance, are often enough to relieve the anxieties of our patients sufficiently to enable dental treatment. When used appropriately, it has a wide margin of safety and no significant side effects. However, the instantaneous feeling of euphoria exper

Nitrous oxide-induced neurological disorders: an increasing public health concern.

BACKGROUND: Neurological presentations resulting from nitrous oxide (N2 O) abuse are increasing in Australia and worldwide. Despite known neuropsychiatric sequelae, N2 O canisters remain readily available and its use unregulated. AIMS: To examine the demographics, clinical and electrophysiological findings of patients presenting with neurological complications of N2 O abuse, and thus inform clinicians and public health decision-makers of the significant public health concerns of this increasing practice. METHODS: Consecutive patients presenting to a tertiary referral metropolitan hospital we

Cerebral venous sinus thrombosis: a complication of nitrous oxide abuse.

Nitrous oxide (NO) is an inhalant that has become increasingly popular as a recreational drug. While it is presumed to be harmless, a number of adverse effects of NO have been described. We discuss the case of a 24-year-old man with no medical history, who initially presented to the emergency department with progressive polyneuropathy caused by vitamin B12 deficiency after NO abuse. Two days after being discharged with hydroxocobalamin supplementation, the patient returned with a severe headache, blurry vision and slurred speech. Imaging revealed cerebral venous sinus thrombosis. Hypercoagulab

Nitrous oxide-induced demyelination: Clinical presentation, diagnosis and treatment recommendations.

BACKGROUND: Recreational use of nitrous oxide (NO) in the general public has led to increasing reports of NO-induced demyelination (NOID). We describe the varying clinical presentations and pathophysiology, and offer a treatment paradigm. METHODS: A literature search of MEDLINE and EMBASE resulted in 42 publications with 37 studies meeting the inclusion criteria, for a total of 51 patients. Our case series included 5 patients seen from 2014 to 2018 followed over 3-60 months. RESULTS: Those with sensory symptoms and subjective weakness were categorized as having "mild" symptoms (25%). Symptom

Desflurane and nitrous oxide, but not nonimmobilizers, affect nociceptive responses.

UNLABELLED: Nonimmobilizers (previously called nonanesthetics) do not prevent movement in response to a noxious stimulus, even at doses predicted to produce anesthesia. We hypothesized they would also lack antinociceptive effects. We tested this prediction using the tail-flick latency (TFL) test. As predicted, the two nonimmobilizers tested (1,2-dichlorohexafluorocyclobutane and perfluoropentane) did not alter TFL, whereas desflurane and nitrous oxide both lengthened TFL (nitrous oxide at a lower minimum alveolar anesthetic concentration [MAC]-multiple than desflurane). In addition, we found t

Postoperative opisthotonus and torticollis after fentanyl, enflurane, and nitrous oxide.

Most drug-induced extrapyramidal symptoms are due to blockade of dopaminergic receptors and are treated with anticholinergic drugs. We report a patient with severe postoperative extrapyramidal symptoms which responded to physostigmine and indicated a different aetiology. A young, healthy female outpatient developed severe extrapyramidal symptoms after an uneventful 50 min anaesthetic with thiopentone, fentanyl (100 micrograms), enflurane, and nitrous oxide. Although the trachea was not extubated until she obeyed commands, the patient developed opisthotonus, which resolved initially after treat

Susceptible period of nitrous oxide teratogenicity in Sprague-Dawley rats.

The susceptible period of nitrous oxide (N2O) teratogenicity was studied in 170 Sprague-Dawley rats. Seven groups of 20 timed-pregnant rats were exposed to 60% N2O for 24 hours on each of days 6-12 of gestation; a control group of 30 timed-pregnant rats was exposed to air on day 9. On day 20 of gestation, dams were killed and reproductive indices were determined; their fetuses were subsequently examined for external, skeletal, and visceral abnormalities. There were no differences among the groups in the number of implantations and live fetuses, mean fetal weight, and sex ratio. The incidence o

Effects of low concentrations of nitrous oxide on rat fetuses.

The long-term inhalation of air polluted with nitrous oxide has been implicated in spontaneous abortions and fetal abnormalities in operating room personnel. Studies with 1% and 0.5% N2O in air have confirmed the adverse effects of dilute concentrations of N2O in pregnant rats. The present study was designed to determine the threshold concentration of nitrous oxide necessary to produce adverse responses in pregnant rats. Wistar albino rats were mated in an environmental chamber and conception confirmed by vaginal smears. Immediately thereafter, the rats were randomly divided into control and e

Anesthetics as teratogens: nitrous oxide is fetotoxic, xenon is not.

Exposure of pregnant rats to the anesthetic nitrous oxide on the ninth day of gestation causes fetal resorption, skeletal anomalies, and macroscopic lesions including encephalocele, anophthalmia, microphthalmia, and gastroschisis. The inert gas xenon, which has anesthetic properties similar to those of nitrous oxide, does not cause teratogenic effects under the same experimental conditions.

Post-operative nausea and vomiting in out-patient gynecologic laparoscopy: a comparison of thiopental-nitrous oxide, propofol-nitrous oxide and total intravenous anesthesia using propofol.

An intravenous anesthetic drug, propofol was considered to pose antiemetic action. A randomized controlled trial was conducted to evaluate whether propofol could effectively reduce post-operative nausea and vomiting (PONV) compared to thiopental-nitrous oxide (N2O). One-hundred and eight patients undergoing outpatient gynecologic laparoscopy were assigned to receive 3 techniques of anesthesia; thiopental-N2O (T/N), propofol-N2O (P/N) and total intravenous anesthesia (TIVA) using propofol (P/P). The results showed that in the early period (0-5 hours), post-operative nausea in T/N, P/N and P/P g

Unveiling the overlooked nitrous oxide reduction enzyme in wastewater treatment systems.

Nitrous oxide (N2O) is a potent greenhouse gas with steadily rising atmospheric concentrations. N2O reductase (N2OR), encoded by the nosZ gene, is the key enzyme capable of reducing N2O to dinitrogen. While the canonical nosZ clades (I and II) have been extensively investigated, the recently proposed clade III (L-nosZ) remains largely uncharacterized in engineered ecosystems. Here, by analyzing 226 metagenomes from globe 114 wastewater treatment plants (WWTPs), we demonstrate that l-nosZ is both widespread and phylogenetically diverse within activated sludge communities. Although its abundance

The rise of nitrous oxide in toxicological casework: no laughing matter.

Nitrous oxide (N2O), the colorless, odorless gas known as "laughing gas," has gained recent attention for its misuse as a recreational drug. As an anesthetic, N2O produces sedation, euphoric, and possible hallucinogenic effects. Adverse effects may include disorientation, psychomotor retardation, hypoxia, and asphyxia. N2O misuse has grown due to its ease of availability, rapid onset of effects, and increased social media attention, leading to anticipated increases in forensic testing needs. Due to its short half-life and volatility, analytical detection can be challenging. From January 2022 t

Peripheral Neuropathy Linked to Recreational Nitrous Oxide Use: A Case Report.

BACKGROUND Nitrous oxide (N₂O) deactivates vitamin B12 and, when inhaled regularly, can lead to vitamin B12 deficiency, resulting in temporary or permanent peripheral neuropathy. This case report describes a young adult with N₂O-induced B12 deficiency, whose neurological symptoms were initially misattributed to a distracting traumatic injury from a motor vehicle collision (MVC). CASE REPORT We present a case of a previously healthy 24-year-old man who presented with low back pain and bilateral leg paresthesia that began 2 days after being a passenger in an MVC. There was no head trauma, loss o
