The Role of Mitomycin C in Intermediate-risk Non-muscle-invasive Bladder Cancer: A Systematic Review and Meta-analysis.

BACKGROUND AND OBJECTIVE: Intravesical mitomycin C (MMC) instillations are recommended to prevent recurrence of intermediate-risk non-muscle-invasive bladder cancer (IR-NMIBC); however, the optimal regimen and dose are uncertain. Our aim was to assess the effectiveness of adjuvant MMC and compare different MMC regimens in preventing recurrence. METHODS: We performed a comprehensive search in PubMed, Scopus, and Web of Science in November 2023 for studies investigating recurrence-free survival (RFS) among patients with IR-NMIBC who received adjuvant MMC. Prospective trials with different MMC r

Efficacy of Intravesical Instillation Therapy with Low-Dose Tokyo-172 Bacillus Calmette-Guérin to Prevent Recurrence of Non-Muscle-Invasive Bladder Cancer and Treat Carcinoma in situ: A Multi-Institutional Retrospective Study.

INTRODUCTION: There are various doses, durations, and strains of bacillus Calmette-Guérin (BCG) intravesical instillation therapy, but optimal treatment has not yet been established. We retrospectively investigated the efficacy and safety of low-dose BCG therapy for non-muscle-invasive bladder cancer (NMIBC) and carcinoma in situ (CIS) in a multicenter study. METHODS: From 1991 to 2019, 323 patients who received BCG therapy to prevent recurrence of NMIBC were analyzed as group A. Similarly, 147 patients who received BCG therapy for the treatment of CIS were analyzed as group B. Patients recei

Adjuvant intravesical treatment in patients with intermediate and high-risk non-muscle-invasive bladder cancer with BCG versus MMC applied with COMBAT or EMDA. Results of a prospective study.

BACKGROUND: Bacillus Calmette-Guerin (BCG) maintenance therapy is the standard adjuvant treatment of high- and intermediate-risk non-muscle-invasive bladder cancer (NMIBC). However, the problems of shortages and the adverse effects, both local and systemic, that it causes lead to the search for alternatives with devices that improve the penetration of intravesical chemotherapeutics. MATERIALS AND METHODS: Prospective observational study was conducted from August 2018 to August 2022. Patients diagnosed with intermediate and high-risk NMIBC without CIS who received one of the following three tr

Conductive hyperthermic chemotherapy versus electromotive drug administration of mitomycin C as intravesical adjuvant treatment of patients with intermediate or high-risk non-muscle invasive bladder cancer.

BACKGROUND: Devices that increase the penetration of intravesical chemotherapeutic agents have been developed as alternatives to the use of bacillus Calmette-Guérin, in short supply at a time of increasing global incidence of non-muscle invasive bladder cancer (NMIBC). We performed a prospective observational study to compare 2 of these devices in the treatment of patients with high- and intermediate-risk NMIBC. The primary endpoint was the recurrence-free rate. Secondary endpoints were the rate of progression and adverse events. METHODS: After undergoing transurethral bladder resection, 98 p

Second Transurethral Resection of Bladder Tumor Can Be Safely Omitted in Selected Patients with T1 Non-muscle-invasive Bladder Cancer: Results from the Prospective HuNIRe Trial.

BACKGROUND AND OBJECTIVE: Current European Association of Urology guidelines universally recommend a second transurethral resection (ReTUR) for all T1 non-muscle-invasive bladder cancer (NMIBC) cases. However, ReTUR is an invasive and costly procedure that is often negative for residual disease, and may represent overtreatment in appropriately selected patients who have undergone complete initial TUR. Our aim was to report 2-yr oncological outcomes and confirm the safety of a novel, response-guided strategy with selective ReTUR for patients with T1 NMIBC. METHODS: The prospective, observation

Use of the Xpert Bladder Cancer Monitor Urinary Biomarker Test for Guiding Cystoscopy in High-grade Non-muscle-invasive Bladder Cancer: Results from the Randomized Controlled DaBlaCa-15 Trial.

BACKGROUND AND OBJECTIVE: Urinary biomarker tests have long been suggested as a noninvasive alternative to current cystoscopy-based follow-up of high-grade non-muscle-invasive bladder cancer (NMIBC) to reduce both morbidity and costs. However, the clinical impact of urinary biomarker tests remains uncertain owing to a lack of randomized trials. METHODS: In this randomized noninferiority trial, we evaluated the impact of alternating a urinary biomarker test (Xpert Bladder Cancer Monitor, XBCM) with cystoscopy in the intervention arm as a method for reducing the number of cystoscopies during fo

Recent advancements in the diagnosis and treatment of non-muscle invasive bladder cancer: Evidence update of surgical concept, risk stratification, and BCG-treated disease.

In the management of non-muscle invasive bladder cancer (NMIBC), disease progression and long-term control are determined by the intensity of delivered treatment and surveillance and the cancer cells' biological nature. This requires risk stratification-based postoperative management, such as intravesical instillation of chemotherapy drugs, Bacillus Calmette-Guérin (BCG), and radical cystectomy. Advancements in mechanical engineering, molecular biology, and surgical skills have evolved the clinical management of NMIBC. In this review, we describe the updated evidence and perspectives regarding

The changing landscape of urothelial carcinoma: on the edge of a paradigm shift.

Urothelial cancers of the urinary tract are the fourth most common malignancy in men, with a shifting demographic affecting younger patients and an increasing incidence in females. In this Review, we discuss recent discoveries and paradigm-shifting clinical trials that impact all stages of urothelial cancer. New therapeutics and drug-delivery devices have led to multiple approvals for treatments of non-muscle invasive bladder cancer. The addition of chemotherapy, immunotherapy, and antibody-drug conjugates is transforming perioperative treatment for patients with muscle-invasive bladder cancer

Role of Enhanced Imaging in Superficial Bladder Cancer Management.

White-light cystoscopy (WLC) may miss small, non-papillary, flat lesions, commonly associated with high-grade lesions such as carcinoma in situ. Enhanced cystoscopy techniques, including blue-light cystoscopy (BLC), narrow-band imaging, and IMAGE1 S, aim to address the shortcomings of traditional WLC in detecting bladder cancer. These techniques enhance the detection of tumors, although whether the improved sensitivity translates into better recurrence-free and progression-free survival is less certain. BLC should be utilized in patients with suspicious cytology and unrevealing WLC.

Bacillus Calmette-Guérin Therapy: Optimizing Outcomes amid Challenges.

Bacillus Calmette-Guérin (BCG) remains the cornerstone adjuvant therapy for intermediate-risk and high-risk non--muscle invasive bladder cancer, yet its clinical utility is increasingly challenged by supply shortages, treatment intolerance, and variable oncologic response. This article summarizes the immunologic basis, efficacy, and limitations of BCG, examines evidence for alternative administration strategies, evaluates combination and biomarker-based approaches to optimize patient selection, and outlines emerging therapies for BCG-unresponsive disease. Practical, evidence-driven recommendat

The Contemporary Pattern of Radical Cystectomy in the Treatment of High-Grade Non-Muscle Invasive Bladder Cancer.

INTRODUCTION: Radical cystectomy (RC) remains the most effective treatment for high-grade non-muscle invasive bladder cancer (HG NMIBC) with high-risk features or after failed bladder preservation. Yet, utilization varies and understanding contemporary treatment patterns can help guide personalized care, especially amid expanding bladder-sparing options. This study assessed national trends and outcomes in HG NMIBC management with RC from 2010 to 2022. PATIENTS AND METHODS: The National Cancer Database (NCDB) was queried for clinically localized HG NMIBC. Determinants of RC use, time to RC, pa

Cohort profile: COBLAnCE: a French prospective cohort to study prognostic and predictive factors in bladder cancer and to generate real-world data on treatment patterns, resource use and quality of life.

PURPOSE: Bladder cancer is a complex disease with a wide range of outcomes. Clinicopathological factors only partially explain the variability between patients in prognosis and treatment response. There is a need for large cohorts collecting extensive data and biological samples to: (1) investigate gene-environment interactions, pathological/molecular classification and biomarker discovery; and (2) describe treatment patterns, outcomes, resource use and quality of life in a real-world setting. PARTICIPANTS: COBLAnCE (COhort to study BLAdder CancEr) is a French national prospective cohort of p

Type I-interferon β induces a strong anti-tumour response in bladder cancer cells.

PURPOSE: For non-muscle invasive bladder cancer (NMIBC), instillation with Bacillus Calmette-Guérin (BCG) is a standard therapy. With a still unclear mechanism, instillation activates the innate immune system, resulting in an immunological effect on the tumour. The aim of this work was to investigate which bladder cancer (BLCA) cells can be activated by interferon (IFN) exposure. METHODS: The BLCA cell lines RT4 and SW780 were stimulated with IFN-α2, -β and -λ1 over 4-72 h. Quantitative PCR (qPCR) was used to determine the expression of IFN receptor subunits (RS) and selected interferon-stimu

Use of Shared Decision-Making in Non-Muscle-Invasive Bladder Cancer: Protocol for a Scoping Review.

BACKGROUND: With a growing range of treatment options for non-muscle-invasive bladder cancer (NMIBC), the emergence of longitudinal patient-reported quality-of-life data, and an aging population managing the treatment burden of coexisting conditions, there is an increasing need for shared decision-making processes and tools to support urologists and patients in navigating complex treatment decisions. What remains unknown is how shared decision-making is currently incorporated into NMIBC treatment decisions and which factors inform key decisions in NMIBC. OBJECTIVE: The objective of this scopi

Decoding Immune Mechanisms in BCG-unresponsive Non-muscle Invasive Bladder Cancer.

Non-muscle-invasive bladder cancer (NMIBC) accounts for roughly 75% of all bladder cancer cases. For patients with intermediate- and high-risk disease, intravesical Bacillus Calmette-Guérin (BCG) remains the standard treatment, yet it fails in up to 40% of cases. While radical cystectomy is the most effective salvage option, it carries significant morbidity and long-term quality-of-life consequences, highlighting the urgent need for bladder-sparing alternatives. Advancing such therapies requires a deep understanding of the immunologic mechanisms within the tumor microenvironment (TME). This re

A Multi-Gene Signature of Non-Muscle-Invasive Bladder Cancer Identifies Patients Who Respond to Immunotherapies Including Bacillus Calmette-Guérin and Immune Checkpoint Inhibitors.

Approximately 75% of bladder cancer cases originate as non-muscle-invasive bladder cancer (NMIBC). Despite initial diagnosis, NMIBC commonly recurs, with up to 45% advancing to muscle-invasive bladder cancer (MIBC) and metastatic disease. Treatment for high-risk NMIBC typically includes procedures like transurethral resection and, depending on recurrence risk, intravesical chemotherapy or immunotherapy such as Bacillus Calmette-Guérin (BCG). However, persistent shortages of BCG necessitate alternative first-line treatments. We aim to use a multi-gene signature in high-risk NMIBC patients to de

Sequential Intravesical Gemcitabine and Docetaxel is an Alternative to Bacillus Calmette-Guérin for the Treatment of Intermediate-risk Non-muscle-invasive Bladder Cancer.

Adjuvant treatment with either chemotherapy or bacillus Calmette-Guérin (BCG) is recommended for patients with intermediate-risk (IR) non-muscle-invasive bladder cancer (NMIBC). In this multi-institutional retrospective review, we evaluated oncological outcomes for 182 patients with IR-NMIBC treated with BCG (n = 100) or intravesical sequential gemcitabine and docetaxel (Gem/Doce; n = 82). Median follow-up was 48.6 mo (interquartile range 24.9-70.9). No patient had a previous diagnosis of high-grade disease. Recurrence rates were similar in the two treatment groups (hazard ratio [HR] 1.06, 95%

Noninvasive studies may have potential to replace cystoscopy in non-muscle invasive bladder cancer follow-up.

Bladder cancer has a high recurrence rate which requires frequent follow-up. Cystoscopy is currently the gold standard for follow-up which is invasive and undesirable procedure for patients. We aimed to investigate the feasibility of noninvasive studies for follow-up of non-muscle invasive bladder cancer. This retrospective study was done for non-muscle invasive bladder cancer patients with abnormal lesion at follow up cystoscopy, therefore those needed transurethral resection of bladder tumor (TUR-BT). Inclusion criteria was patients who had preoperative bladder magnetic resonance imaging (MR
