Practical Insights from Operationalizing a Remote Symptom Monitoring Program Using a Commercial Platform in a Canadian Community Cancer Clinic: A Post-Implementation Qualitative Evaluation.

Remote symptom monitoring using electronic patient-reported outcomes reduces symptom burden and acute care use in oncology, but most programs have been established in academic centers or funded trials using purpose-built software. We examined how the REmote Symptom and Patient mONitoring System (RESPONSe) was implemented and subsequently funded as a recurrent clinical service at a Canadian community cancer clinic using a configured commercial platform, patients' own devices, and no formal institutional mandate. We conducted a qualitative post-implementation process evaluation at Richmond Cance

Violence against nurses working in US emergency departments.

OBJECTIVE: The objective of this study was to investigate emergency nurses' experiences and perceptions of violence from patients and visitors in US emergency departments (EDs). BACKGROUND: The ED is a particularly vulnerable setting for workplace violence, and because of a lack of standardized measurement and reporting mechanisms for violence in healthcare settings, data are scarce. METHODS: Registered nurse members (n = 3,465) of the Emergency Nurses Association participated in this cross-sectional study by completing a 69-item survey. RESULTS: Approximately 25% of respondents reported ex

The Status of and Factors Associated With Quiet Quitting Among Clinical Nurses: An Evaluation and Latent Profile Analysis.

AIMS: This study aimed to examine the prevalence and latent profiles of quiet quitting among clinical nurses and identify factors associated with this condition across different nurse subgroups. BACKGROUND: Quiet quitting among clinical nurses poses a hidden threat to nursing workforce stability and quality of care. Burnout, work-family conflict, and professional identity are closely associated with quiet quitting in nurses. However, academic studies examined quiet quitting as a single global construct, without identifying the potentially heterogeneous profiles within the nursing workforce.

Nurse Roles in Virtual Care Within Team-Based Primary Care: Qualitative Findings From a Multiple Case Study in Two Canadian Jurisdictions.

INTRODUCTION: Canadians, particularly those in Newfoundland and Labrador (NL) and Nova Scotia (NS), face ongoing challenges in accessing primary care. To address this concern, many jurisdictions have adopted team-based primary care that enables nurses, physicians, and other providers to collaborate in delivering care. Virtual care has been widely integrated to promote efficiency and quality across team-based primary care. Nurses in primary care teams (i.e., nurse practitioners, registered nurses, licensed practical nurses) deliver care virtually; however, details of their role enactment remain

Conceptualizing nurse practitioner role ambiguity: A dual-perspective for contemporary nurse practitioner practice.

BACKGROUND: The sustained well-being of the nurse practitioner (NP) workforce is threatened by pervasive role ambiguity. This ambiguity is frequently cited as a barrier to successful professional integration and practice. Although existing literature acknowledges this issue, it lacks a formal definition within the context of NP practice. This omission prevents targeted evaluation of its true impacts and the development of effective interventions. PURPOSE: The purpose of this article is to present a concept analysis of role ambiguity within the NP context. METHODOLOGY: Rodgers' evolutionary c

Development of a Machine Learning-Based Patient Classification System for a Gastroenterology Ward.

BACKGROUND: Patient classification systems (PCSs) are frequently used to estimate hours of nursing care to support budgeting and guide staffing decisions. However, despite a clear clinical need, there is as yet no validated and reliable PCS designed specifically for gastroenterology wards. OBJECTIVE: To develop a PCS tailored to gastroenterology wards that can accurately quantify nursing workload and support evidence-based nurse staffing decisions. METHODS: This study used Orem's self-care theory and Henderson's human needs theory as a foundation and applied machine learning techniques to co

Skilled Nursing Facility Network Capacity and Hospital Length of Stay.

IMPORTANCE: Delays in hospital discharge to skilled nursing facilities (SNFs) are widely viewed by hospital administrators and policymakers as a significant contributor to hospital crowding and adverse patient outcomes. However, the extent to which regional SNF capacity is associated with hospital length of stay (LOS) is not well understood. OBJECTIVES: To construct empirically derived hospital-SNF markets using a network science approach and to examine whether variation in SNF capacity within these markets is associated with hospital LOS. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectio

Anticipating the primary care role in genomic medicine: expectations of genetics health professionals.

Our purpose was to explore genetics health professionals' (GHPs) expectations of primary care providers' (PCPs) role in genomic medicine now and in the future. Focus groups/interviews were conducted with GHPs in Ontario, Canada. Recordings were transcribed and analysed using qualitative descriptive analysis. Five focus groups (6 clinical geneticists, 24 genetic counselors, 1 nurse, 4 laboratory staff, 3 genetics program administrators) and 3 interviews (nurses) were conducted. GHPs described a key role for PCPs in genomic medicine that could be enhanced if GHPs and PCPs worked together more ef

An observational study of antipsychotic medication use among long-stay nursing home residents without qualifying diagnoses.

UNLABELLED: WHAT IS KNOWN ON THE SUBJECT?: In the United States, 15.5% of nursing home residents without qualifying diagnoses of schizophrenia, Huntington's' Disease, and/or Tourette Syndrome receive antipsychotic medications. Antipsychotic medications are used off-label (i.e., used in a manner the United States Food and Drug Administration's packaging insert does not specify) to treat neuropsychiatric symptoms, often before attempting nonpharmacologic interventions, despite evidence that this drug class is associated with significant adverse events including death. Less than optimal staffing

The American Society of Pain and Neuroscience (ASPN) Guidelines for Advanced Practice Providers in Interventional Spine and Pain Management Practice.

BackgroundChronic pain management has advanced with minimally invasive interventions that reduce surgical trauma and recovery time. Specialized interventional spine clinics rely on multidisciplinary approaches to improve outcomes. Advanced Practice Providers (APPs), including nurse practitioners, physician assistants, and clinical nurse specialists, play increasingly important roles in these settings. However, standardized guidance on APP integration, training, and scope of practice remains limited. This guideline, developed by an expert panel, is the first to address APP roles in intervention

Melatonin Prescribing in Assisted Living.

OBJECTIVES: Aging is associated with sleep problems, which are common among assisted living (AL) residents. More than 40% of residents have insomnia, and almost 70% have a sleep disturbance of some kind. Melatonin to treat sleep disorders has become common among older adults, but its use in AL is unknown, which is concerning because melatonin may have side effects and interfere with other medications. The purpose of this study was to determine the prevalence and correlates of melatonin prescribing in AL, to inform whether changes in care may be indicated. DESIGN: Cross-sectional chart abstrac
