Operationalizing Iterative RE-AIM in two pragmatic nursing home trials to support an adaptation learning cycle.

INTRODUCTION: Complex, heterogeneous settings such as nursing homes require ongoing adaptation of interventions and implementation strategies. Iterative RE-AIM offers a structured approach to guide adaptations, but how the approach is operationalized may need to vary across implementation contexts. We describe how two pragmatic nursing home trials operationalized Iterative RE-AIM with FRAME-IS to enable a systematic adaptation learning cycle. METHODS: We conducted a narrative analysis of two ongoing multi-site trials: TEAM EFFORT (focused on falls reduction in State Veterans Homes) and 40 WIN

[Exploring the Entrustment Decision Experiences of Preceptors Regarding New Nurses' Independent Patient Care].

BACKGROUND: Within the framework of competency-based medical education, determining whether new nurses are capable of making patient care decisions independently is a critical issue in clinical nursing education. However, there is a lack of in-depth exploration of clinical preceptor experiences related to entrustment decision-making in this context. PURPOSE: This study was designed to explore the experiences of nursing preceptors in terms of developing the confidence necessary in new nurses to make independent clinical decisions during the preceptorship process. METHODS: A qualitative resear

Empowering final-year student nurses through autonomous practice in district and community nursing placements.

Final-year student nurses were offered aspirant nurse roles to support busy clinical teams within district nursing services as part of the response to the COVID-19 pandemic. Student nurses in their final year of training had the opportunity to work as an aspirant nurse (paid band 4 position) for a 12-week period, as per the emergency education standards delivered by the Nursing and Midwifery Council in an effort to help out during the pandemic. This gave the students additional responsibilities to allow them to work autonomously but still under the supervision of the registered nurse. This rol

Interest in Osteoporosis among Nurses and Physicians before Introduction of a Fracture Liaison Service: A Questionnaire-Based Study.

BACKGROUND: Osteoporosis-related fragility fractures such as proximal femoral and vertebral fractures are associated with decreased activities of daily living and a shorter life span. Many persons with osteoporosis do not receive testing or treatment. Introducing fracture liaison services (FLSs) to hospitals is an effective approach for reducing this treatment gap. Understanding interest in osteoporosis among nurses and physicians, their knowledge of osteoporosis, and their thoughts on team medicine for treatment of osteoporosis might be helpful when implementing FLSs. METHODS: An FLS had not

Pushing boundaries in paediatric intensive care: training as a paediatric retrieval nurse practitioner.

Traditionally in the UK, the transportation of the critically ill child to a paediatric intensive care unit has been carried out by a medically led team of doctors and nurses. However, in countries such as the USA and Canada, appropriately trained nurse practitioners have proven to be competent in the transportation of these vulnerable children. This nurse-led team model has also been shown to be successful in the speciality of neonatal care in the UK. The impact of changes in the National Health Service (NHS) has led to an increased demand for the transportation of the child requiring paediat

Case Difficulty and Non-Routine Events by Surgical Team Members in a Large Prospective Observational Study at 5 Veterans Affairs Medical Centers.

BACKGROUND: Excessive workload is associated with degraded operator performance and outcomes. The construct of surgical "team workload" is poorly specified, and anesthesiologist workload has rarely been measured concurrent with that of operating room nurses and surgeons during the same cases. We sought to measure workload, operationalized as individual and team "case difficulty," as well as the occurrence of non-routine events, from all surgical team members. METHODS: This multicenter prospective observational study at 5 Veterans Affairs medical centers involved 1107 non-cardiac surgical proc

Nursing Cooperation Pattern for Patients With Advanced Pancreatic Cancer and Abdominal Pain Undergoing Endoscopic Ultrasound-guided Celiac Plexus Neurolysis.

CONTEXT: Patients with pancreatic cancer (PC) at a late stage often suffer from severe abdominal pain due to the invasion of celiac plexus, and the analgesics they receive often have intolerable side effects. Endoscopic, ultrasound-guided, celiac plexus neurolysis (EUS-guided CPN) can have a good therapeutic effect. OBJECTIVE: The study intended to evaluate the ability of two nursing cooperation patterns to reduce patients' pain, decrease operation times, increase operational efficiency, and increase nurses' satisfaction, for patients with advanced PC and abdominal pain who received EUS-guide

Legal Implications in the Care of Orthopedic Patients: Serious Complications.

To help reduce potentially devastating outcomes from spinal complications and venous thromboembolism, it is essential for the bedside nurse to have a comprehensive understanding of risk factors and assessments. The orthopedic nurse carries a responsibility for accurately assessing, documenting, and mobilizing the team when abnormal changes occur. Nurses act as patient advocates. The bedside nurse must communicate with the physician. If a patient or family sues the health care team, after a careful screening of the case for merit, the legal process begins. A health care provider should never gi
