A Clinical Nurse Specialist-Directed Initiative to Reduce Postoperative Urinary Retention in Spinal Surgery Patients.

: Postoperative urinary retention (POUR) is the inability to void when the bladder is full after surgery. It is a common complication in postoperative patients, especially in patients undergoing spinal surgery. At our institution, patients who were discharged from the postanesthesia care unit (PACU) to the inpatient surgical unit typically had bladder distention and a bladder volume of more than 450 mL. In an effort to address this situation, an interprofessional group of advanced practice RNs and physicians formed a team, reviewed the existing literature, examined the PACU nursing practice gu

Effect of motivational behavioral change-integrated rehabilitation nursing on recovery and prognosis in patients with hypertension and diabetic foot ulcers.

A retrospective analysis was conducted on 72 patients with hypertension complicated by diabetic foot ulcers who were treated in our hospital between January 2020 and February 2023. According to the nursing approach documented in the medical records, patients were assigned to a motivation-oriented behavioral change-based rehabilitation nursing group (n = 36) or a routine nursing group (n = 36). Outcomes included systolic and diastolic blood pressure, fasting and 2-hour postprandial blood glucose, ulcer healing rates at 7, 14, and 28 days, foot function scores, overall clinical effectiveness, se

Transsphenoidal resection of pituitary adenoma: using a critical pathway.

The use of a transsphenoidal critical pathway can be a vital tool for critical care nurses in anticipating complications and improving patient outcomes. Complications such as diabetes insipidus and cerebrospinal fluid leak associated with posttranssphenoidal patients may result in prolonged hospitalization and worsened functional outcome. Implementing a transsphenoidal critical pathway for these patients can guide their care and alert critical care nurses to potential complications and their prevention and/or treatment.

A physiologic approach to bowel training.

Bowel training is an effective management option for many patients with dysfunctional bowel elimination patterns and neurogenic fecal incontinence. To be effective, a bowel training program must be based on sound physiologic principles and must be individualized for each patient. This article includes a review of the structures and physiologic processes controlling normal defecation and the physiologic principles governing bowel training. The steps involved in successful implementation of a bowel training program are discussed, and case studies are used to illustrate the principles.

Invisible nursing care: a concept analysis.

BACKGROUND: Invisible nursing care comprises essential nursing actions that are often unrecognized and undocumented, such as emotional support, empathetic presence, and preserving patient dignity. Despite their importance, these aspects are mostly overshadowed by technical tasks and lack clear conceptualization in nursing theory and practice. METHODS: Using Walker and Avant’s concept analysis approach, this study systematically reviewed literature from PubMed, Scopus, CINAHL, Web of Science, and Google Scholar up to April 2025. Out of 352 records, 15 relevant articles were analyzed to clarify

How to try this: Assessment of transient urinary incontinence in older adults.

Urinary incontinence in older adults is associated with an increased risk of institutionalization, as well as with urinary tract infections and depression. Transient urinary incontinence arises suddenly, lasts less than six months, and results from reversible causes. Many caregivers erroneously consider urinary incontinence to be inevitable in older adults, especially in hospitalized patients. Failure to identify and respond to transient urinary incontinence may lead to established incontinence and to other poor outcomes after hospital discharge. A bladder diary and a mnemonic are two methods

Panniculectomy, documentation, reimbursement, and the WOC nurse.

Panniculectomy surgery is a reconstructive procedure performed to remove a panniculus, sometimes referred to as a pannus or abdominal apron. The pannus frequently contributes to a number of health concerns, including chronic wounds. Panniculectomy surgery becomes necessary for many patients who have had massive weight loss as a result of successful bariatric weight-loss surgery. Bariatric weight-loss surgery is one of the fastest growing specialties in America, and therefore, nurses can expect to care for increasing numbers of patients who require this reconstructive procedure. Despite the med

How forensic mental health nurses' perspectives of their patients can bias healthcare: A qualitative review of nursing documentation.

AIMS AND OBJECTIVES: Our aim was to examine the notes produced by nurses, paying specific attention to the style in which these notes are written and observing whether there are concerns of distortions and biases. BACKGROUND: Clinicians are responsible to document and record accurately. However, nurses' attitudes towards their patients can influence the quality of care they provide their patients and this inevitably impacts their perceptions and judgments, with implications to patients' care, treatment, and recovery. Negative attitudes or bias can cascade to other care providers and professio

Improving antipsychotic prescribing practices in nursing facilities: The role of surveyor methods and surveying agencies in upholding the Nursing Home Reform Act.

BACKGROUND: Inappropriate prescribing of antipsychotics is a longstanding challenge to providing high quality care in nursing facilities. The 1987 Nursing Home Reform Act (NHRA) had an initial impact in reducing inappropriate prescribing, but rates returned to pre-NHRA levels. While the recent Partnership to Improve Dementia Care in Nursing Homes (Partnership) initiative has been successful in reducing the prescribing of antipsychotics in skilled nursing facilities/nursing facilities (NF), it is not known how this guidance initiative impacted prescribing practices and other care processes conc
