Obsessive compulsive-disorder and reproductive life events.

We explored the association between reproductive life events and the onset or exacerbation of obsessive-compulsive disorder (OCD) among women in comparison with men with OCD. We chose men because there is some evidence of increased occurrence of OCD in expectant fathers as well. We recruited 150 eligible outpatients (58 men and 92 women) with OCD, who attended a specialty OCD Clinic over a period of one year. Occurrence of any life event including reproductive life events in the year before the onset of OCD was assessed using the Interview of Recent Life Events (IRLE). Reproductive life events

Cutting-edge genetics in obsessive-compulsive disorder.

This article reviews recent advances in the genetics of obsessive-compulsive disorder (OCD). We cover work on the following: genome-wide association studies, whole-exome sequencing studies, copy number variation studies, gene expression, polygenic risk scores, gene-environment interaction, experimental animal systems, human cell models, imaging genetics, pharmacogenetics, and studies of endophenotypes. Findings from this work underscore the notion that the genetic architecture of OCD is highly complex and shared with other neuropsychiatric disorders. Also, the latest evidence points to the par

[New pathophysiological approaches to obsessive-compulsive disorder].

New pathophysiological approaches to obsessive-compulsive disorder. Since the beginning of research in the 1980s, it has been consistently shown that obsessive-compulsive disorder (OCD) is neurobiologically underpinned by the dysfunction of a so-called cortico-striato-thalamo-cortical loop (CSTC). Within this loop, various structures have been identified as being affected, foremost among which are the orbitofrontal and anterior cingulate cortices, as well as the caudate nucleus, one of the structures of the striatum. More recently, a large number of studies have contributed to broadening this

The resting-brain of obsessive-compulsive disorder.

Obsessive-compulsive disorder (OCD) is one of the most debilitating psychiatric conditions, having a dramatic impact on patients' daily living. In this work, we aimed to explore resting-state functional connectivity in OCD patients, using an independent component analysis. Eighty individuals (40 patients and 40 healthy controls) performed a resting state fMRI protocol. OCD patients displayed reduced functional connectivity (FC) in visual and sensorimotor networks. In addition, patients displayed decreased FC between sensory networks and increased FC between default-mode and cerebellar networks

Deep brain stimulation for obsessive-compulsive disorder.

Neurosurgical interventions have been used for decades to treat severe, refractory obsessive-compulsive disorder (OCD). Deep brain stimulation (DBS) is a neurosurgical procedure that is used routinely to treat movement disorders such as Parkinson's disease and essential tremor. Over the past two decades, DBS has been applied to OCD, building on earlier experience with lesional procedures. Promising results led to Humanitarian Device Exemption (HDE) approval of the therapy from the United States Food and Drug Administration in 2009. In this review, the authors describe the development of DBS fo

Obsessive-Compulsive Disorder in Children and Adolescents.

Obsessive-compulsive disorder (OCD) can be a disabling condition for children, adolescents, and their families. The majority of children and adolescents with OCD have poor academic functioning and a comorbid diagnosis. Many patients will respond with exposure and response prevention therapy. However, for those who are unable, unwilling, or do not fully respond to conservative treatment, it is essential that advanced practice nurses are able to appropriately evaluate and prescribe for OCD in their young patients. [Journal of Psychosocial Nursing and Mental Health Services, 56(3), 15-18.].

Psychic euosmia and obsessive compulsive personality disorder.

Patients with obsessive compulsive personality disorder (OCPD) often refer to a prompt mood improvement upon encountering good scents in general, or fresh laundry borax on their clothes, pillows or home settings. The Authors propose the new term psychic euosmia in the mean of an overstated psychological predisposition for a real pleasant smell that elicits an immediate sense of pleasure, order and calm. The prompt reactions to a pleasant odor might be explained by the involvement of rhinencephalon and its proximity to mood-related limbic circuits, which bypass the cognitive awareness. Cleanlin

Decoding moral emotions in obsessive-compulsive disorder.

BACKGROUND: Patients with obsessive-compulsive disorder (OCD) exhibit abnormal neural responses when they experience particular emotions or when they evaluate stimuli with emotional value. Whether these brain responses are sufficiently distinctive to discriminate between OCD patients and healthy controls is unknown. The present study is the first to investigate the discriminative power of multivariate pattern analysis of regional fMRI responses to moral and non-moral emotions. METHOD: To accomplish this goal, we performed a searchlight-based multivariate pattern analysis to unveil brain regio

Pharmacologic treatment of obsessive-compulsive disorder comorbidity.

INTRODUCTION: Obsessive-compulsive disorder (OCD) is clearly a heterogeneous syndrome in which comorbidity is the rule rather than the exception and is often 'phase-specific'. Comorbid conditions have a negative impact on OCD outcome and may clearly impact the disease trajectory. Nevertheless, in the current literature there is an impressive neglect of comorbidities in clinical trials and treatment approaches for these conditions are still not evidence-based. AREAS COVERED: In this paper we summarized the available data on the treatment of the main OCD comorbidities (mood and anxiety disorder

Quality of life in obsessive compulsive disorder.

Obsessive-compulsive disorder (OCD) has a profound impact with a high disease burden. In order to truly understand the scope of the effect OCD has on the patient population, one must take into account not only the relentless symptoms beleaguering the patients but also examine their overall ability to enjoy their life. Quality of life (QOL) assessments/improvements are becoming an increasingly important component of healthcare, especially in the mental health field. This review examines QOL in OCD, as well as the influence of comorbidities, and the impact that OCD treatment has on QOL. We searc

Early orbitofrontal hyperactivation in obsessive-compulsive disorder.

Dysfunctional activity in the orbitofrontal cortex (OFC) is one of the core features in the pathophysiology of obsessive-compulsive disorder (OCD). Neuroimaging studies indicate orbitofrontal hyperactivation during the resting state as well as during symptom provocation, whereas orbitofrontal hypoactivation has been reported during tasks designed to dissociate specific cognitive processes. Combined magnetoencephalic and functional magnetic resonance imaging studies show early involvement of the OFC in stimulus processing in healthy subjects. However, it is unclear whether OFC activation is dys

Dysfunctional reward circuitry in obsessive-compulsive disorder.

BACKGROUND: Obsessive-compulsive disorder (OCD) is primarily conceived as an anxiety disorder but has features resembling addictive behavior. Patients with OCD may develop dependency upon compulsive behaviors because of the rewarding effects following reduction of obsession-induced anxiety. Reward processing is critically dependent on ventral striatal-orbitofrontal circuitry and brain imaging studies in OCD have consistently shown abnormal activation within this circuitry. This is the first functional imaging study to investigate explicitly reward circuitry in OCD. METHODS: Brain activity dur

Second-generation antipsychotics for obsessive compulsive disorder.

BACKGROUND: Obsessive compulsive disorder (OCD) is a psychiatric disorder which has been shown to affect 2 to 3.5% of people during their lifetimes. Inadequate response occurs in 40% to 60% of people that are prescribed first line pharmaceutical treatments (selective serotonin reuptake inhibitors (SSRIs)). To date not much is known about the efficacy and adverse effects of second-generation antipsychotic drugs (SGAs) in people suffering from OCD. OBJECTIVES: To evaluate the effects of SGAs (monotherapy or add on) compared with placebo or other forms of pharmaceutical treatment for people with

An attractor hypothesis of obsessive-compulsive disorder.

We propose a top-down approach to the symptoms of obsessive-compulsive disorder (OCD) based on a statistical dynamical framework. An increased depth in the basins of attraction of attractor network states in the brain makes each state too stable, so that it tends to remain locked in that state and can not easily be moved on to another state. We suggest that the different symptoms that may be present in OCD could be related to changes of this type in different brain regions. For example, the difficulty in attentional and cognitive set switching could be related to networks operating in this way

[The neurobiological approaches to obsessive-compulsive disorder].

The lifetime prevalence rate of obsessive-compulsive disorder (OCD) is more than 2 percent of the population. Its contemporary pathophysiological models have been explored. As serotonin reuptake inhibitors and cognitive behavior therapy are both considered first-line treatments of OCD, the treatment interventions provide us with clues. In this review, the authors summarized that genetics, neuropathology in the cortico-striatal-thalamic-cortical (CTSC) circuits, the association between OCD and Tourette's syndrome, the possibility of autoimmune-mediated pathophysiology containing PANDAS, serolog

Immunological alterations in adult obsessive-compulsive disorder.

BACKGROUND: Some recent findings suggest the involvement of autoimmune mechanisms in childhood onset of obsessive-compulsive disorder (OCD), on the basis of a parallel drawn with Sydenham's chorea, a manifestation of rheumatic fever. A monoclonal antibody called D8/D17 characterizing a B-lymphocyte antigen, present in almost all patients with rheumatic fever, has been found also in children affected by OCD, Tourette syndrome, and chronic tics to a greater degree than in healthy control subjects. The few observations of disturbances of some immunologic parameters in adult OCD patients, prompted

Complaints of constipation in obsessive-compulsive disorder.

Psychoanalytic observation has launched a long tradition in medical literature that links constipation with obsessive-compulsive traits. This association, however, has never been tested empirically. The current investigation sought to test this hypothesized association empirically using a large, randomly sampled population database. Data from the Epidemiologic Catchment Area project collected with the Diagnostic Interview Schedule were analyzed to determine the association, if any, of complaint of medically unexplained constipation (ascertained from the somatization disorder section of the int
