Outcome of pregnancies complicated by oligohydramnios or anhydramnios of renal origin.

OBJECTIVE: To evaluate the outcome of fetuses with oligohydramnios due to kidney anomalies. METHODS: A retrospective study was performed of all pregnancies diagnosed with oligohydramnios and associated kidney anomalies during the period 2000-2008. Outcome included pregnancy outcome, mortality, and morbidity. Morbidity included renal function based on the glomerular filtration rate (GFR) during follow-up. RESULTS: A total of 71 pregnancies were evaluated; 36 fetuses presented on ultrasound with cystic dysplasia, 15 with polycystic kidney disease (PKD) and 20 with hydronephrosis. Twenty-three

Practice patterns in the management of isolated oligohydramnios: a survey of perinatologists.

OBJECTIVE: Optimal management of isolated oligohydramnios (IO) remains debatable. We surveyed Society for Maternal-Fetal Medicine (SMFM) members regarding their opinions and practice patterns. STUDY DESIGN: Questionnaires were mailed to perinatologists across the US. IO was defined as sonographic low fluid (per the practitioner's definition) in the absence of intrauterine growth restriction, fetal anomaly or significant maternal comorbidity. RESULTS: The overall response rate was 35% (n = 632). Ninety-two percent of respondents consider IO to be a risk factor for various adverse outcomes. Wi

Isolated oligohydramnios in term pregnancy as an indication for induction of labor.

OBJECTIVE: To evaluate the outcome of active induction of labor for isolated oligohydramnios in low-risk term gestation. METHODS: This retrospective study analyzed the obstetric and perinatal outcome of 412 singleton term pregnancies with cephalic presentation and no maternal risk factors or fetal abnormalities. Two groups were compared: 206 deliveries after induced labor for isolated oligohydramnios, and 206 deliveries matched for gestational age following spontaneous labor with normal amniotic fluid index. RESULTS: The overall rate of cesarean deliveries and cesarean deliveries for nonreas

The experience of amnioinfusion for oligohydramnios during the early second trimester.

OBJECTIVE: In the past, oligohydramnios occurring early during the secondary trimester was the reason to terminate the pregnancy because of poor prognosis. Even though amnioinfusion has been reported for improving the prognosis of the infant, it is still not frequently used because of unfavorable results. We present our experience with amnioinfusion for oligohydramnios with or without preterm premature rupture of membranes. MATERIALS AND METHODS: A retrospective study was performed from July 2005 to December 2006 in our hospital. Amnioinfusions were performed in pregnant women found with olig

Effects of oligohydramnios on lung growth and maturation in the fetal rat.

Oligohydramnios (OH) retards fetal lung growth by producing less lung distension than normal. To examine effects of decreased distension on fetal lung development, we produced OH in rats by puncture of uterus and fetal membranes at 16 days of gestation; fetuses were delivered at 21 or 22 days of gestation. Controls were position-matched littermates in the opposite uterine horn. OH lungs had lower weights and less DNA, protein, and water, but no differences in saturated phosphatidylcholine, surfactant proteins (SP)-A and -B, and mRNA for SP-A, -B, -C, and -D. To evaluate effects on epithelial d

Relation between oligohydramnios and spinal flexion in the human fetus.

BACKGROUND: Oligohydramnios, a severe reduction in the volume of amniotic fluid, is associated with fetal lung hypoplasia but underlying processes are unclear. Studies in sheep suggest that oligohydramnios may lead to lung hypoplasia by causing increased flexion of the fetal spine, but this has not been demonstrated in the human, which has a different uterine anatomy. AIMS: Our aims were to quantify spinal flexion in the human fetus and to determine the relationship between oligohydramnios and the degree of spinal flexion. SUBJECTS AND METHODS: In 35 pregnancies, we used ultrasonography to a

The effect of polyhydramnios and oligohydramnios on fetal lung maturity indexes.

The influence of amniotic fluid (AF) volume on common fetal lung maturity (FLM) indices was evaluated. Cases diagnosed with altered AF volume as estimated by ultrasound (n = 71; polyhydramnios = 33, oligohydramnios = 38) were matched to controls by: gestational age (GA) at amniocentesis, GA at delivery, neonatal weight, sex, and pregnancy complication. FLM was assessed on AF specimens obtained by transabdominal amniocentesis by planimetric and stechiometric L/S, phosphatidylglycerol (PG), and lamellar bodies counts (LB). In cases with polyhydramnios, L/S ratios (planimetric and stechiometric)

Accuracy of intrapartum estimates of fetal weight. Effect of oligohydramnios.

OBJECTIVE: To study the effect of oligohydramnios on the accuracy of intrapartum clinical and sonographic estimates of fetal weight (EFWs). STUDY DESIGN: Intrapartum clinical and sonographic EFWs and an amniotic fluid index (AFI) were obtained on 124 term patients. Oligohydramnios was defined as an AFI < or = 5 cm. RESULTS: Clinical and sonographic EFWs in women with oligohydramnios were significantly less accurate than estimates derived in patients with normal amniotic fluid: P = .03 and .02, respectively. The incidence of clinical and sonographic EFWs within +/-10% of the actual birth weig

Reversal of oligohydramnios with subtotal immersion: a report of five cases.

OBJECTIVE: The purpose was to summarize five cases where subtotal immersion was associated with an increase in the amniotic fluid index. STUDY DESIGN: Five women with oligohydramnios, as defined by an amniotic fluid index < 8 cm, who underwent subtotal (shoulder-deep) immersion therapy are described. RESULTS: The mean pretreatment amniotic fluid index was 4.9 +/- 3 cm. After immersion therapy was instituted, the amniotic fluid index increased an average of 6 +/- 2.2 cm. In three subjects whose immersion therapy was discontinued, the amniotic fluid index fell an average of 4.7 cm. CONCLUSION

Transabdominal amnioinfusion for the management of nonlaboring postdates with severe oligohydramnios.

A retrospective study compared two comparable consecutive groups of nonlaboring primiparas beyond 41 weeks with severe oligohydromanios (no amniotic pocket more than 2 cm deep). Transabdominal amnioinfusions were not done in the historical control group (n = 14) but were done in the second group (n = 11). Fetal heart rate patterns and neonatal outcome were better in the group with amnioinfusion than in controls, and the rate of cesarean delivery was significantly lower (18 vs. 93%, p < 0.001). These results suggest that prepartum transabdominal amnioinfusion may have obstetrical and perinatal

Amnioinfusion: an aid in the ultrasonographic evaluation of severe oligohydramnios in pregnancy.

OBJECTIVE: Amnioinfusion was used in pregnancy complicated by severe oligohydramnios to enable a more comprehensive ultrasonographic evaluation of the pregnancy. STUDY DESIGN: Thirteen patients were referred to the Division of Obstetrical and Gynecological Ultrasound at the University of Miami because of severe oligohydramnios in which a definitive diagnosis could not be made. Nine patients were in the second trimester and four were in the early third trimester. All patients underwent amnioinfusion with warm physiologic saline solution and instillation of indigo carmine. Genetic amniocentesis

[Electron microscopic study of the amniotic epithelium in polyhydramnios and oligohydramnios].

Totally 25 cases without any fetal anomaly, 10 with polyhydramnios, 10 with oligohydramnios and 5 with normal volume of amniotic fluid were taken into consideration. Their amnion covering the placenta and umbilical cord were examined under electron microscope. In the group with polyhydramnios the microvilli facing the amniotic cavity were denser in certain regions. The intercellular space was widened and the terminal bars were opened. Within the cells the number of vesicles were increased and there were large cysternas within these vesicles. It was postulated that the large cysternas found in

Infarction of the upper limbs associated with oligohydramnios and intrauterine compression.

We describe a case of neonatal infarction involving both upper extremities. Oligohydramnios, resulting in compression of both arms within the uterus, is proposed as the cause of this congenital gangrene. Despite attempted conservative management, both arms became flaccid and moist. By the third day of life, the infant's worsening cardiovascular status necessitated amputation. This rare disorder has varying degrees of presentation. Limbs have been salvaged with conservative management of the extremity, coupled with appropriate surgical intervention, and neonatal intensive care.

Subcellular effects of experimental oligohydramnios on the developing rat limb.

In the rat, the development of the limb bud can be impaired by amniotic puncture. Selective haemorrhages are observed within 1 hour of the treatment. An ultrastructural analysis was undertaken to uncover the cytological changes associated with these haemorrhages. The earliest effect of oligohydramnios is the appearance of a periendothelial oedema which separates the mesodermal cellular processes from the endothelial basal lamina. The loosening of the latter and the distortion of the endothelial cells occur almost simultaneously. Subsequent damage of the mesodermal cells ensues, the ectoderm an

Renal anomalies and oligohydramnios in the cerebro-oculofacio-skeletal syndrome.

We describe two infants with the cerebro-oculofacio-skeletal syndrome in whom oligohydramnios is an additional finding. The oligohydramnios could be accounted for in one by renal agenesis but cannot in the other, who had a functioning and histologically normal kidney. The diagnosis of Potter syndrome was suspected in both patients. It is important in counseling to recognize the distinctive phenotype of this autosomal-recessive syndrome.

The Association between Oligohydramnios and the Risk of Malpresentation: A Meta-Analysis.

BACKGROUND: There is no consensus on the role of oligohydramnios on the increased risk of fetal malpresentation. Therefore, we decided, for the first time, to collect all published studies on this topic to synthesize all available data through a systematic review and meta-analysis to evaluate the effect of oligohydramnios on the increased risk of fetal malpresentation. METHODS: A search was done in the following international electronic bibliographic databases: PubMed, Web of Science, and Scopus to April 2021. Heterogeneity among studies was determined by the Q-test and I2 statistic. Publicat
