UNLABELLED: We report a case of bloody pleural effusion and infarction of the greater omentum caused by a non-traumatic diaphragmatic hernia with a late presentation. A 15-y-old boy with Down's syndrome developed abdominal pain and vomiting, as well as an elevated serum level of C-reactive protein. Chest roentgenograms showed a right-sided pleural effusion and computed tomography revealed a right diaphragmatic hernia. Barium enema confirmed the diagnosis. An operation revealed a right Bochdalek's hernia with strangulation of the greater omentum in the right pleural cavity.

Isolated inflammatory alterations of the greater omentum are rare diseases. Since the preoperative diagnosis is difficult these changes are usually ascertained upon laparotomy. Between 1999 and 2001 four patients (44-88-years-old, 2 males) underwent laparotomy for an inflammatory tumor mass or an acute abdomen, respectively. C-reactive protein was elevated in all cases. OBJECTIVES: To describe a novel technique for the surgical treatment of idiopathic chylothorax in a dog.
ANIMALS: A 6-year-old, male Rhodesian Ridgeback, which presented with a history of reduced exercise tolerance and dyspnea.
METHODS: Idiopathic chylothorax was diagnosed. Intermittent pleural drainage failed to resolve the problem. During surgery, extensive pleural fibrosis made it impossible to identify the thoracic duct. A dorsal omental pedicle was advanced through an incision in the diaphragm and into the cranial thoracic cavity to act as a physiological drain.

The authors report one case of primary torsion of the greater omentum. The presenting symptoms are similar to those of an acute appendicitis. Diagnosis is rarely considered before laparotomy. The presence of sero-sanguineous peritoneal fluid should lead to careful exploration of the omentum, with resection of the involved segment.

The treatment of 59 consecutive vesicovaginal fistulas is described. Five patients were treated by simple vaginal repair; in 52 patients repair was carried out with the aid of an omental pedicle. Only 2 of these patients are not fully continent; 1 awaits further surgery and 1 has stress incontinence but refuses a further operation. A further 2 patients were treated by urinary diversion. The cure rate was 93%. The value of omentum in overcoming fibrosis and tissue loss is discussed.
