A needs assessment for a minor eye condition service within Leeds, Bradford and Airedale, UK.

BACKGROUND: There are a number of limitations to the present primary eye care system in the UK. Patients with minor eye conditions typically either have to present to their local hospital or GP, or face a charge when visiting eye care professionals (optometrists). Some areas of the UK have commissioned enhanced community services to alleviate this problem; however, many areas have not. The present study is a needs assessment of three areas (Leeds, Airedale and Bradford) without a Minor Eye Conditions Service (MECS), with the aim of determining whether such a service is clinically or economical

Clinical pearls in optometric management of the geriatric patient.

BACKGROUND: Painstaking attention to detail is absolutely necessary for optometrists to provide the appropriate clinical management of the geriatric patient. METHODS: Clinical management techniques (pearls) are discussed which aid in the diagnosis and treatment of problems specific to geriatric patients. Factors noted in patient presentation are stressed and include physical appearance, ability to move about, and use of other senses. The most commonly occurring eye anomalies occurring in the geriatric population are discussed including iatrogenic drug side effects, pupillary anomalies, extrao

Glaucoma Agreement in New Zealand (GAINZ).

CLINICAL RELEVANCE: With an ageing population, ophthalmologists are becoming burdened with glaucoma management, and patient care can be delayed. Therefore, the use of optometrists in glaucoma management can help alleviate the burden. BACKGROUND: The ageing population and subsequent rise of glaucoma prevalence are putting a strain on the public health system in New Zealand. Glaucoma collaborative care between optometrists and ophthalmologists has been gaining support with the aim to reduce this burden on ophthalmologists. There has been little investigation of the agreement in care and managem

Intraocular lenses and their follow-up in the optometric office.

A broad overview of intraocular lens implantation is presented for the optometrist. Included in this overview is a review of the most recent literature comparing complication rates for lens implants versus standard cataract surgery. A proportion of the paper addresses itself to the pre and post-operative optometric management of the pseudophakic patient. This paper is presented to give the optometrist a basic understanding of the expanding field of intraocular lens implantation as well as a reference list which will serve as a starting point for further review.

A review of intraocular lenses.

Intraocular lens implants are being used in ever increasing numbers for the correction of aphakia. For that reason optometrists should familiarize themselves with the types of implants used and the advantages and disadvantages of each. This article will review the types of implants the optometrist is likely to see. The changing trends in implant surgery will be considered along with explanations for the changes. Several studies of patients with implants are reviewed. Finally, a format for evaluating implant patients is presented.

Ocular syphilis associated with syphilitic resurgence in a human immunodeficiency virus-infected man.

Optometrists provide an important service to the community, often acting as the first health care provider a patient consults. Many patients will present with ocular complaints that have systemic associations. In addition to diabetes, hypertension, and rheumatologic disorders, optometrists can encounter the ocular manifestations of sexually transmitted diseases such as syphilis. Syphilis is a chronic systemic infection caused by the spirochete Treponema pallidum, which is transmitted primarily through sexual contact. A recent rise in the prevalence and clinical manifestation of syphilis increa

The use of drugs for behavior modification as it relates to the practice of optometry--Part I.

Medications used in behavior modification can result in untoward side effects, including ocular and visual changes. As in many other aspects of health care, optometrists can serve as the first line of defense to guard their patients against permanent damage from side effects they may not associate with use of their medication. Optometry's responsibility and role in management of side effects is delineated. Areas of behavior modification of concern to optometrists are discussed. These include: (1) learning problems and mental retardation; (2) mental disorders, menopause, and alcoholism; and (3)

Clinician Awareness of Stickler Syndromes Among Australian Allied Health Care Professionals.

PURPOSE: Stickler Syndromes are multisystem collagenopathies affecting 1 in 7500-9000 individuals and are associated with craniofacial, ocular, auditory, and musculoskeletal complications. Prophylactic retinopexy treatment reduces the risk of retinal detachment, emphasising the need for early detection and multidisciplinary referral. This study evaluated knowledge and awareness of Stickler Syndromes among allied health professionals and their perceived needs for targeted education to improve multidisciplinary care. METHODS: A cross-sectional survey was undertaken among audiologists, speech pa

Orbital fractures.

The optometrist is often the first practitioner to see patients with ocular trauma and associated orbital fractures. Trauma is now the second leading cause of blindness in the United States. Early and accurate diagnosis of orbital fractures is required to avoid compromising the binocular vision and cosmetic appearance of the patient. With an understanding of the characteristic clinical presentations of fractures and a knowledge of current imaging techniques, these disorders can be accurately diagnosed, managed, or referred by the optometrist.

The false negative rate and the role for virtual review in a nationally evaluated glaucoma referral refinement scheme.

PURPOSE: To establish the safety of the CHANGES glaucoma referral refinement scheme (GRRS). METHODS: The CHANGES scheme risk stratifies glaucoma referrals, with low risk referrals seen by a community based specialist optometrist (OSI) while high risk referrals are referred directly to the hospital. In this study, those patients discharged by the OSI were reviewed by the consultant ophthalmologist to establish a 'false negative' rate (Study 1). Virtual review of optic disc photographs was carried out both by a hospital-based specialist optometrist as well as the consultant ophthalmologist (Stu

COVID-19 guidelines for optometry and optical services post-lockdown.

The COVID-19 pandemic has caused disruption in everyone's lives globally. Optometrists, being primary health care professionals are at a higher risk of infection in their practices during these testing times. Optometrists and optical store owners need practically implementable guidelines as lockdown gets lifted in various parts of our country. As these practices gear up to provide eye examination and vision correction to people, they need to take necessary precautions to avoid any cross contaminations. Optometry Council of India guidelines were circulated among optometry and optical associatio

Comparison of the effectiveness of two enhanced glaucoma referral schemes.

PURPOSE: To compare the clinical and financial effectiveness of two optometric-led enhanced glaucoma referral schemes in the Bexley Care Trust area. METHODS: Over a 12-month period all suspect glaucoma/Ocular Hypertension (OHT) referrals from optometrists relating to patients registered with Bexley GPs were analysed. All these patients were examined under one of two schemes. One was an enhanced glaucoma repeat measurement (EGRM) scheme in which the referring optometrist conducted the repeated tests him/herself prior to referral or non-referral. The alternative was a refinement pathway (RCAS)

The content of optometric eye examinations for a presbyopic patient presenting with symptoms of flashing lights.

BACKGROUND: Standardised patients (SPs) are the gold standard methodology for evaluating clinical care. This approach was used to investigate the content of optometric eyecare for a presbyopic patient who presented with recent photopsia. METHODS: A total of 102 community optometrists consented to be visited by an actor for a recorded eye examination. This actor received extensive training to enable accurate reporting of the content of the eye examinations, via an audio recording and a checklist completed for each clinical encounter. The actor presented unannounced (incognito) as a 59-year-old

Retinitis pigmentosa: visual function and multidisciplinary management.

Retinitis pigmentosa (RP) is a leading cause of blindness and visual disability in younger people. Optometrists have a major role in detecting RP and in reducing the visual disability associated with RP. This review summarises the literature relating to visual function in people with RP, with particular attention given to night-blindness, visual acuity decrease and visual field contraction. The range of low vision aids available for people with RP is reviewed and suggestions given on aids that have been found to be most successful. Most importantly, this review overviews the range of services

Albinism: an update and review of the literature.

BACKGROUND: Albinism can be a diagnostic challenge to the optometrist, with ocular albinism the entity most likely to be overlooked or misdiagnosed. Albinism should be suspect in a child with nystagmus. METHODS: Albinism is best diagnosed by electron microscopy of skin or hair bulbs. Measuring the flash visually evoked response (VER) is the best way to confirm the abnormal decussation of the optic nerve fibers. Transillumination of the iris can be performed to see if it lacks pigment on its posterior surface. Optometrists should also be alert for nystagmus, strabismus, lack of stereopsis, and
