Reliability of and comparisons among methods of measuring dissociated phoria.

Many methods of heterophoria measurement are available clinically. This paper reviews several studies which have examined the reliability of phoria measurements, and have compared various tests of phoria measurement. Different methods of data analysis make comparison of studies difficult. Two studies indicated 95% limits of agreement of 2 to 4 delta for reliability of modified Thorington, von Graefe, and Maddox rod phoria tests. Studies using correlational analysis to compare different measurement methods have found a high degree of association of the results of these methods. Studies using st

Spectacles for the emmetropic presbyopic optometrist.

Presbyopia brings special seeing problems for an emmetropic optometrist. Incorporating the bifocal into a half-glass offers several advantages. The best parameters for this spectacle form are derived from experience, and also by comparing it to single-vision half-glasses, bifocals, and trifocals. Particular facility in both prescribing and fabricating derives from the great number of power and size relations between upper and lower segments available. One can extrapolate similar benefits in this spectacle form for other occupations and hobbies as well as for presbyopic wearers of single-vision

The negative effects of dry eye disease on quality of life and visual function.

In recent years, dry eye has become a hot topic within ophthalmology and optometry, especially in regards to new frontiers in treatment modalities which include novel devices, procedures, and medications. However, some of the more understudied areas in dry eye involve its impact on quality of life. Although ocular discomfort symptoms are well known to be associated with dry eye, its negative effects on visual function remain underrecognized. This paper reviews these topics within the currently published literature to heighten awareness among clinicians.

Optometric findings in velocardiofacial syndrome.

Velocardiofacial syndrome (VCFS) is a congenital genetically determined condition with skeletal, heart, and facial anomalies. Almost all children have learning problems, despite an often normal or above average IQ. We have examined 10 children with a prior diagnosis of VCFS. All were mildly to moderately hyperopic. Most had poor accommodation, and convergence was clinically poor. About 30% had tortuous retinal vessels. VCFS may be detected in the optometry clinic, and optometric management may ameliorate the learning problems in some cases.

Phoria, vergence, and fixation disparity in oculomotor problems.

Measures of horizontal phoria, vergence, and fixation disparity were obtained for 2 samples: 28 optometry students without symptoms and 32 orthoptic patients with symptoms associated with binocular oculomotor difficulties. Discriminant analysis was used to determine which tests or group of tests best discriminated between the 2 samples. Analysis was done for both samples and for exophoric and esophoric subsamples. Sheard's criterion was the best discriminator for the exophoric group, and amount of heterophoria was the best discriminator for the esophoric group. Fixation disparity was the next

Spectacle fitting with ear, nose and face deformities or abnormalities.

Spectacle frame selection and dispensing remain significant components of optometry. Occasionally, we encounter patients who are unable to wear conventional spectacles due to abnormalities or deformities following injury and/or surgery to their nose, ears or head. In these cases, spectacle frame fitting may be more complex and customized frame adjustments may be required to account for the anatomical variations. A patient with a microtia (hypoplastic pinna) was fitted with a tailored spectacle frame. The details are presented together with a summary of different frames and modifications availa

Traumatic cataract.

Cataract formation is commonly observed as a result of direct penetration of the crystalline lens by a foreign object or by blunt trauma to the globe or adnexa, creating a "shock wave" within the eye. Partial or total damage to the zonules may also occur, resulting in subluxation or total displacement of the crystalline lens. The primary care doctor of optometry plays an important role in evaluating patients with cataract, both immediately and long after the injury has occurred. Many of these patients can be managed conservatively by careful observation, while others will require surgical inte

Prescribing patterns for paediatric hyperopia among paediatric eye care providers.

PURPOSE: To survey paediatric eye care providers to identify current patterns of prescribing for hyperopia. METHODS: Paediatric eye care providers were invited, via email, to participate in a survey to evaluate current age-based refractive error prescribing practices. Questions were designed to determine which factors may influence the survey participant's prescribing pattern (e.g., patient's age, magnitude of hyperopia, patient's symptoms, heterophoria and stereopsis) and if the providers were to prescribe, how much hyperopic correction would they prescribe (e.g., full or partial prescriptio

Referrals from community optometrists in England and their replies: A mixed methods study.

PURPOSE: Community optometrists, through routine eye examinations, identify patients with disease or ocular abnormalities requiring referral to the Hospital Eye Service. In many cases no reply to the referral letter is received, resulting in some patients being re-referred unnecessarily, potentially increasing the number of other patients who lose sight whilst on hospital waiting lists. This study investigated, qualitatively and quantitatively, factors influencing optometric referrals and replies. METHODS: The three-phase, sequential mixed methods study started with a literature review and qu

Distribution and Predictors of Initial Glaucoma Care Among Ophthalmologists and Optometrists: A Population-based Study.

PURPOSE: To evaluate evolution in the distribution of new glaucoma patients between ophthalmologists and optometrists, and to examine factors predicting provider type, in the context of expansion in the scope of optometry practice. PATIENTS AND METHODS: A population-based study was undertaken using validated datasets in Ontario, Canada from 2007 to 2018, encompassing time before and after optometry practice scope expansion in 2011. All patients aged 66 and older receiving a glaucoma suspect diagnosis or first-line therapy for glaucoma from ophthalmologists or optometrists were enrolled. Predi

Refractive errors among children, adolescents and adults attending eye clinics in Mexico.

AIM: To assess the proportion of refractive errors in the Mexican population that visited primary care optometry clinics in fourteen states of Mexico. METHODS: Refractive data from 676 856 patients aged 6 to 90y were collected from optometry clinics in fourteen states of Mexico between 2014 and 2015. The refractive errors were classified by the spherical equivalent (SE), as follows: sphere+½ cylinder. Myopia (SE>-0.50 D), hyperopia (SE>+0.50 D), emmetropia (-0.50≤SE≤+0.50), and astigmatism alone (cylinder≥-0.25 D). A negative cylinder was selected as a notation. RESULTS: The proportion (95%

Ocular manifestations of AIDS.

The acquired immunodeficiency syndrome (AIDS) is a devastating disease which continues to claim the lives of most of its victims. Despite the media's silence, AIDS has in no way disappeared. Over 4,400 cases have been reported thus far and many more new cases are reported each week nationwide. The ocular manifestations of AIDS should alert the doctor of optometry to the possibility of an AIDS suspect, especially if the patient is in one of the major risk categories. Early detection of this disease may permit more prompt treatment and thus a better prognosis for the AIDS patient.

Application of biofeedback and behavior modification techniques in visual training.

A review of the biofeedback literature was made and proposals for use in optometry advanced. To this end electromyograph units can be connected to an apparatus which gives a reward when a patient performs proper eye movements. By this means, patients who undergo visual training become conscious of their eye movements. As a reinforcement, they are rewarded by music or the motion of toys when they give the right answer. Application of the system in cases of strabismus, limitation of gaze, convergence insufficiency or excess, ptosis, and other cases are discussed. Methodological and financial adv
