Comparison of the Efficacy of Midazolam and Dexmedetomidine As Sedative Agents in Third Molar Surgery.

Introduction Minor dental and oral surgical procedures have been made comfortable with the rise in the use of daycare sedatives. Of these sedatives, midazolam is deemed a common sedative used for minor oral surgical procedures. Newer and safer sedatives such as dexmedetomidine have certain properties that may prove more efficient in oral surgical procedures. Third molar surgery is one of the most common minor oral surgical procedures performed in dentistry. Thus, this study aims to compare the efficacy of midazolam and dexmedetomidine as sedative agents in third molar surgery. Materials and me

A technique of a one-stage repair of soft tissues in severe bilateral cleft lip without presurgical orthodontics or lip adhesion.

The repair of severe bilateral cleft lip is usually done electively at three months after orthodontic appliances have pushed back the protuberant premaxilla to its expected position in the dental arch shelves. Historically the premise that the repair will break down or that the premaxilla will not recede has been challenged. This paper describes a technique that the author has used as a one-stage 'putting it all together' repair without presurgical orthodontics or lip adhesion. The results have been favourable and may bring about a rethink on the tissues' inherent plasticity, the need for earl

[Bilateral congenital macrostomia].

INTRODUCTION: Congenital macrostomia, a transverse facial cleft, is a rare deformity of the mouth, which can occur alone or in association with other deformities. We report a case of congenital bilateral macrostomia. We discuss the difficulties of plastic surgery in this pathology. OBSERVATION: Congenital macrostomia in a one-year-old girl compromised feeding. The patient presented an isolated bilateral transverse facial cleft. Surgery associated a suture of the orbicularis oris muscle and a cutaneous W plasty. At twelve months, follow-up has been uneventful. DISCUSSION: Congenital macrostom

Mandibular angle resection using cervicofacial lifting surgical approach: Technical note.

Facial feminization surgery brings a set of surgical procedures that address the three thirds of the face and aims to give feminine characteristics to a masculine face. The characteristics of the lower third of a male face are a long, broad chin, a prominent mandible with sharp angles (in connection with the insertions of the masseter muscles). Mandibular angle reduction is an important procedure of the male-to-female transformation. It allows to modify the appearance of the lower third of the face, reducing the bi-mandibular distance in the coronal plane and opening the mandibular angle in pr

Evolution of my philosophy in the treatment of unilateral cleft lip and palate.

At the end of 50-year-long clinical activity, the evolution of my approach to the treatment of unilateral cleft of the lip and palate is discussed. I had several teachers in this field (Rusconi, Reherman, Perko, Delaire, Talmant, Sommerlad and others) and I introduced in my approach what I considered to be improvements from all of them. My current protocol is related to the anatomy of the cleft: for wide clefts a two-stage protocol is applied (1° step: soft palate and lip and nose repair; 2° step: hard palate repair with gingivoalveoloplasty); for narrow cleft (less than 1 cm at the posterior

Medication Management of Jaw Lesions for Dental Patients.

Most pathologic lesions of the jaws or of oral mucosa are treated successfully by surgical interventions. For treatment of the central giant cell lesion, aneurysmal bone cysts, histiocytosis of the mandible, hemangioma, odontogenic keratocyst, Paget disease, oral submucous fibrosis, and oral lichen planus, medical management consisting of intralesional injections, sclerosing agents, and systemic bisphosphonates is as successful as surgical procedures with fewer complications. Pharmacology of agents used and protocols are presented.

Surgical considerations in 22Q11.2 deletion syndrome.

The 22q11.2 deletion syndrome (22q11DS) may be associated with several palatal abnormalities, including overt cleft palate, submucosal cleft palate, palatopharyngeal disproportion, and velar hypotonia. The syndrome is the genetic disorder most commonly associated with velopharyngeal dysfunction (VPD). The complex causes of VPD in affected patients combine with the complexity of associated medical disorders to render surgical management of the velopharynx particularly challenging. Optimization of surgical outcomes requires precision in diagnosis, surgical management, and multidisciplinary care.

[Choice of surgical suture material used in oral cavity-clinical study].

INTRODUCTION: Historical data on closing and suturing of surgical wound describe a wide range of various suture materials. The choice of the surgical catgut, i.e. type and diameter, depends on the localization, characteristics and condition of the tissue to be treated. From the standpoint of oral-surgical practice the following clinical parameters are of the outstanding importance regarding the choice of suture material: accumulation of soft deposits on the sutures, decubitus of the adjacent soft tissues and dehiscence of the operative wound. AIM: The aim of this research was to determine the

[Cleft palate repair with bilateral buccal musculomucosal flaps].

OBJECTIVE: A method for cleft palate repair was used for the purpose of diminishing surgical interference with maxillary growth. METHOD: Sixteen cases of cleft palate were repaired with bilateral buccal musculomucosal flaps. RESULT: The operation results in all cases were satisfactory. CONCLUSION: It is concluded: 1. The method does not traumatize the periosteum of the hard palate, thus alleviating postoperative maxillary deformity; 2. The buccal musculomucosal flap is generally long enough to reach across the incisive foramen and maintain good blood circulation; 3. The method benefits reco

Rational utilization of antibiotic prophylaxis in oral and maxillofacial surgery: a large scope review of recent data.

BACKGROUND: Surgical site infections raise the risk of serious health issues following oral and maxillofacial surgeries. There is full consensus that using systemic antibiotics as a prophylactic measure can reduce infection risk in oral and maxillofacial surgical procedures, but the optimal duration and effective usage of these antibiotics remains debated. In light of growing antibiotic resistance, prophylactic use of antibiotics should be evaluated carefully and limited to certain scenarios. Thus, the aim of this scoping review was to summarize the available data regarding the prudent and evi

The impact of practice recommendations for inserting and counting throat packs: a 12-year analysis of never-events data.

Throat packs are frequently used in oral and maxillofacial surgery, but can be fatal if not removed at the end of surgery. We analysed the impact of recent guidelines on reported trends in retained throat packs and found a successful reduction. We briefly discuss what else can be done to completely eliminate these events.

Comparative Clinical Evaluation of Semilunar Coronally Positioned Flap Alone and Semilunar Coronally Positioned Flap in Conjunction with Free Gingival Graft for Root Coverage.

Gingival recession accounts for apical migration of the gingival margin, resulting in exposure of the cementoenamel junction and root surface, with exposure of the root surface linked to deteriorated esthetic appearance and increased dentinal hypersensitivity. Various surgical techniques have been used to correct labial gingival recession defects. The present study evaluated and compared the results of semilunar coronally positioned flap (SCPF) alone and in conjunction with free gingival graft (FGG) for the treatment of Miller Class I and II gingival recession defects in maxillary anterior tee

[Laser navigation guided cleft lip repair].

A new method using the ideal mid-facial line as the navigating reference was introduced to improve the outcome of cleft lip repair. Using the verticle coordinate crossing the middle point of the intercanthus line, surgeons could observe and correct the distortion of the fine structures in labial-nasal area. This laser projecting mid-facial-line navigation was repeatable, while not interfere the operating. In conclusion, generalizing laser navigation is a valuable supplementary for cleft lip repair.

Vessel Sealing Systems in Veterinary Oral and Maxillofacial Surgery: Current Evidence and Clinical Applications.

Vessel sealing systems (VSSs) are medical devices that use bipolar electrothermal energy to seal blood vessels by denaturing collagen and elastin in the vessel wall, successfully occluding the vessel and reducing blood loss during surgical procedures. These devices eliminate the need for conventional ligatures or surgical clips in many situations and have become widely adopted in both human and veterinary surgery. Reported advantages include reduced operative time, improved hemostasis, decreased blood loss, and the ability to perform dissection and vessel sealing with a single instrument. The

Comprehensive Approach to Reduce Surgical Site Infections in Patients Undergoing Neurosurgical Procedures.

Background: Surgical site infections (SSIs) are recognized complications of surgical procedures. Methicillin-resistant Staphylococcus aureus (MRSA) colonization increases the likelihood of developing SSIs. Decolonization of MRSA has been shown to reduce post-operative SSIs, therefore, the aim of this project was to identify and decolonize MRSA carriers and to tailor perioperative antibiotic prophylaxis to protect those at high risk for SSIs better. Methods: In September 2013, a quality improvement process initiative was implemented for pre-operative screening of MRSA nasal carriage for patient
