The Le Fort I osteotomy is a versatile operation for correction of developmental, congenital, and posttraumatic deformities of the lower midface. OBJECTIVES: Osseous dysplasias are the most common subtype of fibro-osseous lesions of the maxillofacial bones. They are benign and often present as incidental asymptomatic lesions. Diagnosis can be made with clinical and radiographic examination.
CASE REPORT: This article reports the case of a 47-year-old man with a huge cementifying osseous dysplasia of the maxilla that presented with secondary infection after teeth extraction and repair of oroantral fistula. Some defects related to untreated palatal or residual clefts represent major deformities. A review of 177 cases, untreated or operated on according to Schwekkendiek's (1958) chronology, was carried out. The advantage of simultaneous operative techniques is discussed, specifically pharyngoplasties, vestibular flap rotation, and osteoplastic closure. When all other surgical possibilities have failed, some flaps have been used to restore mucosal continuity. A multidisciplinary evaluation of each individual case is emphasized.

The pedicled buccal fat flap (PBFF) is a versatile and robust regional flap used for a variety of maxillofacial reconstructions. However, recent literature has questioned the popularity of this flap. The goal of this single-instruction, retrospective study is to review the outcomes and complications of the PBFF.
