[Technic of lumbosacral graft in the surgical treatment of dysplastic spondylolisthesis].

The authors describe a method of treatment for spondylolisthesis using a posterior approach, reduction and fixation with Harrington rods and cortical fibular graft driven through the sacrum, across the lumbosacral intervertebral disc into the body of the 5th lumbar vertebra. The technique is completed by a postero-lateral graft. This technique avoids the need for an anterior approach.

A novel multifunctional PEEK internal fixation plate regulated by Gentamicin/chitosan coating.

Polyetheretherketone (PEEK) has been broadly used in orthopedic implant devices. Nevertheless, the bioinert tended to cause implant loosening and bacterial infection in orthopedic and trauma surgery. In this study, a drug-laden chitosan coating (CS) was constructed and deposited on the porous surface of PEEK (CG-SPEEK) internal fixation plate for multi-functionalization. The physical characterizations of CG-SPEEK were further investigated in the morphology, hydrophilicity, surface energy, roughness, drug release and mechanical properties. CG-SPEEK exhibited excellent antibacterial capabilities

An Innovative Technique for Intraoperative Radiological Assessment of Pelvic and Spine Fractures in Austere Environment.

INTRODUCTION: Managing pelvic and spine fractures in the austere environment can be challenging even for skilled orthopedic surgeons, largely due to the difficulty of radiological visualization of bone structures. We present a simple alternative to the metallic operating table by placing the patient on a spinal board that will allow for a better radiological assessment of these fractures. MATERIAL AND METHODS: A cross-sectional, descriptive, and retrospective study was carried out in the period between 2015 and 2020. The study population was all patients with pelvic o spine fractures, managed

Optimizing strength-ductility, biotribological, angiogenic, and antibacterial properties of hot-rolled and hot-extruded Zn-4Ag-0.4Sr alloys for biodegradable orthopedic fixation.

Biodegradable zinc (Zn) alloys are promising candidates for bone implants due to their suitable degradation rate and inherent bioactivity. However, current Zn alloys require enhancement of their mechanical properties and biological functions through alloying and thermomechanical processing. Herein, we report the outstanding mechanical properties, corrosion, biocompatibility, and biofunctionalities of hot-rolled (HR) and hot-extruded (HE) Zn-4Ag-0.4Sr (ZAS) alloy in comparison with those of Zn-4Ag (ZA) for biodegradable orthopedic fixation. The HE ZAS sample exhibited the optimal balance of mec

Periprosthetic fractures: an unrecognized osteoporosis crisis.

Total joint replacement is common and increasing. Many of these patients have low bone mineral density preoperatively, and arthroplasty leads to bone loss. As falls are common before and after arthroplasty, it is unsurprising that periprosthetic fractures, defined as those associated with an orthopedic device, whether a joint replacement or other internal fixation devices, are not rare. These fractures engender morbidity and mortality comparable to osteoporosis-related hip fractures but remain largely unrecognized and untreated by osteoporosis/metabolic bone disease clinicians. Indeed, recent

Maintenance of hardware after early postoperative infection following fracture internal fixation.

BACKGROUND: The development of a deep wound infection in the presence of hardware after open reduction and internal fixation presents a clinical dilemma, and there is scant literature to aid in decision-making. The purpose of the present study was to determine the prevalence of osseous union with maintenance of hardware after the development of postoperative infection within six weeks after internal fixation of a fracture. METHODS: The present study included 121 patients from three level-I trauma centers, retrospectively identified from billing and trauma registries, in whom 123 postoperative

Damage control orthopaedics in the context of battlefield injuries: the use of temporary external fixation on combat trauma soldiers.

INTRODUCTION: The purpose of this study was to analyze the use of temporary external fixation within the perspective of a damage control orthopedics attitude in the management of battlefield extremity injuries. HYPOTHESIS: Damage control orthopaedics (DCO) is a valuable treatment option for easy and rapid limb fracture stabilization in an unfavorable surgical environment. METHODS: Sixteen French soldiers wounded on the battlefield between 2004 and 2009 and managed by damage control orthopedics were included in this retrospective series. All patients were males with a mean age of 30 years (20

Ankle fracture syndesmosis fixation and management: the current practice of orthopedic surgeons.

There is a wide variety of treatments for disruption of the syndesmosis. There is also controversy as to which device should be used for fixation of the syndesmosis, how many devices should be used, how many cortices the screws should engage, and whether, when, and where the screws should be removed. We conducted a study to determine how orthopedic surgeons manage these injuries. In a survey, we asked orthopedic trauma and foot and ankle fellowship directors and members of the Orthopaedic Trauma Association and the American Orthopaedic Foot and Ankle Society how they routinely treated the synd

Complications and strategies for salvage of intervertebral fixation devices.

The recent evolution of interbody fixation devices, known as threaded interbody fusion cages, is a significant new form of segmental fixation for application to the lumbar and lumbosacral spine. This article discusses the construction, biomechanics, and use of these devices, which serve as carriers for autogenous graft material while restoring segmental stability to the involved motion segment. The biomechanics of these devices greatly surpasses those of the interbody spacers used historically. Interbody spacers have traditionally been utilized as a component of an overall construct that also

Harmful lifestyles on orthopedic implantation surgery: a descriptive review on alcohol and tobacco use.

Alcohol abuse and smoking habits have adverse effects on bone health and are a risk factor for osteoporosis, fractures and impaired fracture repair. Osteointegration processes around implanted biomaterials involve a coordinated cascade of complex events that are very similar to those occurring during fracture repair and require a suitable microenvironment and the coordinated action of cells and signal molecules. Therefore, diseases and harmful lifestyles that impair the normal bone healing process can reduce the success of implant surgery and may negatively influence the osteointegration of pr

Management of adult diaphyseal both-bone forearm fractures.

Simultaneous diaphyseal fractures of the radius and ulna, often referred to as both-bone forearm fractures, are frequently encountered by orthopaedic surgeons. Adults with this injury are typically treated with open reduction and internal fixation because of the propensity for malunion of the radius and ulna and the resulting loss of forearm rotation. Large case series support the use of plate and screw fixation for simple fractures. More complex fractures are managed according to strain theory, with the intention of controlling rather than eliminating motion at the fracture site. This can be

The biomechanics of fixation techniques for hand fractures.

Multiple options are available for the fixation of metacarpal and phalangeal fractures; treatment options depend largely on the fracture characteristics, concomitant injuries, and surgeon preference. With an understanding of the options available and the biomechanics involved, appropriate treatment options may be chosen. Although plate and screw constructs provide the strongest fixation for these fractures, many other techniques provide adequate fixation and may be favored for reasons other than biomechanics.

Three-dimensional analysis of 2 fusionless scoliosis treatments: a flexible ligament tether versus a rigid-shape memory alloy staple.

STUDY DESIGN: Experimental scoliosis was created and subsequently corrected in goats. The 3-dimensional (3-D) effects of the treatments were analyzed. OBJECTIVE: To analyze the 3-D effect of 2 different fusionless scoliosis treatment techniques on an experimental idiopathic-type scoliosis using plain radiographs and computerized tomography. SUMMARY OF BACKGROUND DATA: Scoliosis is a complex 3-D spinal deformity with limited treatment options. By preserving growth, motion, and function of the spine, fusionless scoliosis surgery provides theoretical advantages over current forms of treatment.

Experimental evaluation of Harrington rod fixation supplemented with sublaminar wires in stabilizing thoracolumbar fracture-dislocations.

Segments of the spinal column of calves were instrumented with Harrington rods with and without sublaminar wires. Grossly unstable lesions were created by resecting a 2-cm segment from the central spinal segment. Testing on an Instron tester documented the improvement in stability provided by the sublaminar wires in this grossly unstable experimental fracture. Improved stability was seen in resisting axial loading, lateral bending, and forward flexion loading. Rotational stability was particularly improved.
