Surgical site infection after osteotomy of the adult spine: does type of osteotomy matter?

BACKGROUND CONTEXT: Surgical site infection after spinal surgery is frequently seen. It occurs between 0.7% and 12% of patients, leading to higher morbidity, mortality, and health-care costs. Osteotomy procedures are known to have increased blood losses and surgical times when compared with other spinal surgeries. Both of these factors have previously been identified as significant risk factors for SSI. We performed a cohort study of this high-risk population to identify risk factors and rates of SSI after spinal osteotomy surgery and identify difference in risk between different types of oste

Comparative evaluation of vertical body osteotomy and sagittal split osteotomy for mandibular corpus distraction.

PURPOSE: Mandibular corpus distraction is conventionally carried out by fixing miniaturized intraoral distractors to the body of the mandible after vertical osteotomy posterior to the last tooth. This procedure is not only technically difficult but also has high chances of damaging the inferior alveolar neurovascular bundle. The third molars are also required to be extracted. To overcome these problems, bilateral sagittal split ramus osteotomy was used and the distractor was fixed. The study was carried out to compare the results of the 2 osteotomies for mandibular corpus distraction. PATIENT

Comparison of Ponte Osteotomies and 3-Column Osteotomies in the Treatment of Congenital Spinal Deformity.

BACKGROUND: Congenital spinal deformity (CSD) has traditionally been treated with 3-column osteotomies [hemivertebrectomy (HV) or vertebral column resection (VCR)] to address rigid deformities. Alternatively, multiple Ponte osteotomies (PO) may provide correction while minimizing risk. The purpose of this study was to compare safety and outcomes of patients undergoing surgical treatment for CSD with these 3 procedures. METHODS: Retrospective review of CSD patients treated with posterior spinal fusion between 1996 to 2013. Patients treated with multiple Ponte osteotomies (PO group) were compar

Computer-assisted osteotomies for genu varum deformity: which osteotomy for which varus?

High tibial osteotomy (HTO) is commonly used for genu varum deformity in young and active patients. Corrective valgus osteotomy may however lead to an oblique joint line in cases of associated femur varum or absence of tibia vara. The over-correction, needed to obtain good long-term clinical results, may increase the obliquity even more. To avoid this drawback, the authors suggest use of an accurate and reproducible radiological protocol including at least a standing AP long-leg X-ray to measure not only the hip-knee-ankle (HKA) angle but also the medial distal femoral mechanical angle (MDFMA)

Peroneal nerve injury as a complication of pediatric tibial osteotomies: a review of 255 osteotomies.

A retrospective study of 255 consecutive tibial osteotomies performed for correction of frontal, sagittal, and rotational deformities in children is presented. Eleven (4.3%) peroneal neurapraxias were identified; seven were motor and sensory (2.7%), and four were sensory only (1.6%). In all cases, traction on the peroneal nerve, either by intraoperative retraction, or by anatomic displacement of the osteotomy fragments, was felt to produce the neurapraxia. There were no vascular injuries or compartment syndromes. Increased patient age, estimated blood loss and tourniquet time, difficulty in ex

Mandibular ramus anatomy as it relates to the medial osteotomy of the sagittal split ramus osteotomy.

The sagittal split ramus osteotomy is probably the most frequently used procedure for correction of mandibular skeletal dentofacial deformities. Despite numerous improvements in the technique in the 30 years since the procedure was introduced, a number of troublesome complications still occur. These include unfavorable fracture during surgery, paresthesia, and relapse. The purpose of this study was to determine where fusion of the buccal and lingual cortical plates occurs in the upper mandibular ramus, as it is thought that placement of the horizontal medial osteotomy above the point of fusion

Functional and Radiological Benefits of Adding Fibular Osteotomy to Supramalleolar Osteotomy in Ankle Osteoarthritis.

BACKGROUND Ankle arthritis is a chronic degenerative disease; its typical symptom is pain in the ankle joint. This retrospective study of 65 patients with mid-stage ankle osteoarthritis aimed to compare clinical outcomes from supramalleolar osteotomy (SMOT) and supramalleolar osteotomy combined with fibular osteotomy (SMOT+FO). MATERIAL AND METHODS Based on whether fibular osteotomy was performed during the procedure, the patients were categorized into the SMOT group (n=34) or the SMOT+FO group (n=31). Surgical outcomes were systematically assessed using the Visual Analogue Scale (VAS) for pai

The new "dual osteotomy": combined open wedge and tibial tuberosity anteriorisation osteotomies.

The high frequency with which medial compartment osteoarthritis is associated with patellofemoral osteoarthritis makes the addition of tibial tuberosity anteriorisation to high tibial osteotomy an appealing solution, despite the discouraging previously reported long-term results when tubercle anteriorisation was combined with a Coventry closed wedge technique. We conducted a prospective study of a new osteotomy combination: "the dual osteotomy". An open wedge high tibial osteotomy was combined with 1- to 1.5-cm Maquet-like tibial tuberosity anteriorisation. Thirty-four knees in 30 patients und

Evaluation of surgical experience and the use of an osteotomy guide on the apical angle of an Austin osteotomy.

BACKGROUND: Distal osteotomies of the first metatarsal are commonly used to correct hallux valgus deformities. Of the distal osteotomies, the Austin osteotomy is popular among foot surgeons on an international level. The precision of the osteotomy is important to achieve a congruous osteotomy. OBJECTIVES: The purpose of this study was to examine the effects of experience and technique on creating a precise Austin osteotomy. METHOD: Three individuals with varying levels of experience (student, resident and podiatric physician) created Austin osteotomies in metatarsal sawbones, using three dif

Triple innominate osteotomy and rotational acetabular osteotomy in the treatment of congenital hip dysplasia.

Congenital hip dysplasia is a developmental disease which predisposes patients to osteoarthritis of the hip. We compare the results and complications of triple innominate osteotomy and rotational acetabular osteotomy in the treatment of this disease. Between 1984 and 1992, 27 patients with 30 dysplastic hips received reconstructive procedures at National Taiwan University Hospital. Fourteen patients (16 hips) with a median age of 18 years were treated by triple innominate osteotomy. Nine of 16 hips had concomitant femoral osteotomy. Another 13 patients (14 hips) with a median age of 24 years w

Tibial rotational osteotomy for idiopathic torsion. A comparison of the proximal and distal osteotomy levels.

A retrospective analysis was done of 52 rotational tibial osteotomies (RTOs) performed on 35 patients with severe idiopathic tibial torsion. Thirty-nine osteotomies were performed at the proximal or midtibial level. Thirteen were performed at the distal tibial level with a technique previously described by one of the authors. Serious complications occurred in five (13%) of the proximal and in none of the distal RTOs. For severe and persisting idiopathic tibial torsion, the authors recommend correction by RTO at the distal level. Proximal level osteotomy is indicated only when a varus or valgus

[Pelvic osteotomies: Salter, triple osteotomy, Chiari. Philosophy, technic, choice, indications].

Pelvic osteotomies were proposed in order to preserve as much as possible of the acetabular cartilage. The aims were twofold: centering of the head of the femur in the acetabulum, to improve its stability; correction of the delay in growth of the acetabulum: there is a potential correction, allowing for the femoral growth, or a passive correction by widening a deep acetabulum. Two types of osteotomy are proposed: reorientation osteotomies (Salter, triple osteotomy) which have both a mechanical and biological effect and widening osteotomies (Chiari), which are in fact extracapsular arthroplasti

Deep learning-driven analysis of osteotomy gap healing following mandibular bilateral sagittal split osteotomy.

Bone healing across the osteotomy gap after bilateral sagittal split osteotomy (BSSO) determines whether osteosynthesis material can be safely removed, but is traditionally assessed by subjective visual inspection of cone-beam computed tomography (CBCT) images. This study aimed to develop and validate a deep learning-based model for automated segmentation of BSSO osteotomy gaps on postoperative CBCT scans and to quantify the effect of training dataset size on segmentation performance. Seventy-two osteotomy gaps from 36 patients were manually segmented. Six patients (12 osteotomy sites) were ra

Intraoperative Challenges of Patient-Specific Osteotomy Guides in Anterior Maxillary Osteotomy: A Case Report.

Orthognathic surgery has increasingly incorporated digital workflows and patient-specific surgical guides to enhance accuracy and predictability. While these technologies offer improved planning and execution, their intraoperative application in segmented maxillary osteotomies remains technically demanding. Limited literature exists describing the practical challenges encountered during guide-assisted osteotomy, particularly in anatomically constrained regions. This case report describes the use of a patient-specific cutting guide in a 24-year-old male patient undergoing anterior two-piece max

Corrective osteotomy for malunited distal radius fracture using an augmented reality (AR) osteotomy guide.

Distal radius fracture malunion can result in functional limitations, and corrective osteotomy has been used to treat symptomatic cases. This study aimed to evaluate the precision of corrective osteotomy for distal radius malunion by projecting a virtual osteotomy guide onto the surgical field using augmented reality (AR). Six patients with a mean age of 48 years underwent corrective osteotomy using an AR osteotomy guide. Preoperative planning involved creating 3D bone models from computed tomography (CT) data and simulating osteotomy using medical computer-aided design software. The 3D data w
