[Outpatient diagnoses in primary care: Adding transparency in the field of outpatient diagnoses].

INTRODUCTION: In Switzerland, there is no obligation to record outpatient diagnoses. PraxisGruppe Schweiz (PraxiS), a network of about 30 GP and specialist practices in predominantly rural German-speaking regions in Switzerland, assign diagnosis codes per patient and consultation using the ICD-10 classification system. We compare these diagnoses with approximate ones, the so-called Pharmaceutical Cost Groups (PCGs), based on claims data from SWICA health insurance. We suspected that the distributions of diagnoses differed significantly between the two samples. METHODS: The PraxiS database was

Outpatient shoulder arthroplasty: outcomes, complications, and readmissions in 2 outpatient settings.

BACKGROUND: There is currently a paucity of non-database studies looking at safety and outcomes after outpatient shoulder arthroplasty. The purpose of this study was to report our initial safety experience with outpatient shoulder arthroplasty including 90-day complications and readmissions. Our hypothesis was that the rate of early complications after outpatient shoulder arthroplasty would be low and similar to that of inpatient procedures regardless of outpatient setting. METHODS: We analyzed 50 consecutive patients who underwent outpatient shoulder arthroplasties (44 anatomic total shoulde

Epistaxis: Outpatient Management.

Epistaxis is a common emergency encountered by primary care physicians. Up to 60% of the general population experience epistaxis, and 6% seek medical attention for it. More than 90% of cases arise from the anterior nasal circulation, and most treatments can be easily performed in the outpatient setting. Evaluation of a patient presenting with epistaxis should begin with assessment of vital signs, mental status, and airway patency. When examining the nose, a nasal speculum and a good light source, such as a headlamp, can be useful. Compressive therapy is the first step to controlling anterior e

Outpatient endometrial ablation.

The simplicity of use and short treatment times of second-generation ablation techniques have increased the likelihood of acceptable treatment under local anaesthetic. Once treatment under local anaesthesia +/- conscious sedation has become accepted, the next step is to explore the viability of moving ablation out of theatre. Whilst there are many series reports of treatment under local anaesthetic, few well-constructed trials have evaluated acceptability and success rates. A reliable anaesthetic regime is available with low failure rates, although problems exist with unpredictable post-operat

[Outpatient alcohol detoxification].

Increasing significance is being attributed to outpatient detoxification treatment of alcohol-dependent patients. Many patients want to undergo outpatient detoxification or even carry it out without professional supervision. In some Scandinavian countries and in the USA, outpatient detoxification under medical supervision has increased substantially. In these countries, numerous studies describe outpatient alcohol detoxification as a safe and cost-reducing method. These studies vary in many aspects, especially regarding the nature and dose of the withdrawal medication, but also concerning the i

Outpatient hysteroscopy--problems.

Outpatient hysteroscopy is now a routine procedure, but difficulties may be encountered. It was decided to assess the problems found in one practice over a period of time. Between August 1, 1989 and July 31, 1996 there were 1,080 examinations performed by the author; pain was sometimes significant, but already has been well covered in the literature. The other important problems were vasovagal reactions and failure to complete an adequate examination. This report emphasizes these problems. Being aware of potential problems allows one to anticipate them and often allows the procedure to be carr

Outpatient cervical ripening.

A limited number of studies have provided preliminary data on the efficacy and safety of PGE2 used as an outpatient cervical ripening agent. Multiple inpatient studies have confirmed the effect of PGE2 in favorably changing the cervical score, reducing the incidence of failed inductions and instrumental deliveries, shortening the induction-to-delivery interval, and reducing oxytocin use with few maternal side effects and no known adverse effects on neonatal outcome. These studies, however, are difficult to compare because of differences in study design, PGE2 formulation, patient selection (inc

Outpatient Laparoscopic Cholecystectomy was attempted in 98 patients selected from 266 patients presenting for elective cholecystectomy (37%). Two patients required admission following conversion to "open" Cholecystectomy, one patient was admitted for observation because of a technically difficult Laparoscopic Cholecystectomy and 16 patients were admitted because of refractory nausea and vomiting in the early post-operative period. Seventy-nine patients (81%) were able to be discharged home within 4 to 6 hours of surgery, with only one patient requiring readmission to hospital because of the o

Characteristics of outpatient suicides.

OBJECTIVE: To help identify outpatients who may be at risk for suicide, the authors used data from psychological autopsies of completed suicides to develop a profile of the clinical and demographic characteristics of suicidal patients. METHODS: Ninety-three outpatient suicides committed between April 1, 1988, and August 31, 1991, by patients in programs operated by the New York State Office of Mental Health were analyzed. RESULTS: Persons who committed suicide were younger than other outpatients and more likely to be male, to have a history of drug or alcohol abuse, and to have a primary dia

Safety of outpatient cerclage.

One hundred one women who underwent scheduled McDonald cerclage were studied. Fifty outpatients were reviewed prospectively, and 51 inpatients were studied retrospectively and concurrently. Forty-five outpatients and 48 inpatients completed pregnancy. The fetal outcomes were similar for both groups, with no significant differences in the number of complications for newborn infants. The average cost of the procedure (exclusive of physician fees) for outpatient cerclage was $546 and for inpatient, $1,174. The difference clearly shows the cost-effectiveness of outpatient cerclage.

Outpatient penetrating keratoplasty.

Elective penetrating keratoplasty has been performed on an outpatient basis on 37 patients since July 1987. The commonest indications have been herpetic keratitis, keratoconus, and pseudophakic or aphakic corneal oedema. Local anaesthesia, with little or no sedation, was used in most cases, and only three patients had a general anaesthetic. Accompanying procedures have included anterior vitrectomy, extracapsular cataract extraction with posterior chamber lens implantation and IOL exchange. The results demonstrate that outpatient keratoplasty is a safe, effective alternative to hospitalisation,

Outpatient percutaneous nephrostolithotomy.

Technical advances and operator experience have resulted in a rapid and marked streamlining of the percutaneous approach to renal calculi. The development of nephrostomy tract balloon dilators, improved grasping instruments and the use of assisted local anesthesia have been integral in reducing the morbidity and cost of the procedure. We report our initial favorable experience in the use of percutaneous stone removal on an outpatient basis. All 5 patients underwent an uncomplicated 1-stage stone removal. Cost for outpatient percutaneous stone removal was substantially less than for surgery or

Outpatient phototherapy for psoriasis.

Using a new protocol with varied exposure increments, we found that, in 18 of 20 patients with psoriasis vulgaris who were given ultraviolet (UV)-B phototherapy three times a week, the disease completely cleared. One patient's condition substantially improved, and one patient's condition failed to respond. Ten to 38 treatments were required during a three- to 13-week period. The only topical treatment agent used was white petrolatum. The results are compared with a schedule using five treatments a week and fixed UV-B--exposure increments. Infrequent (three times weekly) treatments, avoidance o

Outpatient phototherapy of psoriasis.

Twenty-six outpatients with chronic plaque-type psoriasis vulgaris were treated five times weekly with topically applied white petrolatum and subsequent exposure to erythemogenic doses of ultraviolet radiation from fluorescent bulbs with a wavelength of 280 to 400 nm. All patients were cleared of psoriasis in ultraviolet-exposed sites in an average of 27 treatments. The combination of topically applied white petrolatum and ultraviolet radiation provides a convenient, cosmetically acceptable outpatient therapy for psoriasis.
