Mondini dysplasia with recurrent bacterial meningitis caused by three different pathogens.

Mondini dysplasia is rare, but has an important association with recurrent bacterial meningitis. We herein describe the case of a 3-year-old girl with unilateral sensorineural hearing loss who presented with three independent episodes of bacterial meningitis within 8 months. Temporal bone computed tomography indicated the characteristic features of Mondini dysplasia in the right inner ear. This was treated by surgical closure of the inner ear defect via oval window and additional vaccination was administered. Appropriate vaccination might prevent the recurrent bacterial meningitis associated w

Low-frequency sound pressure and transtympanic endoscopy of the middle ear in assessment of "spontaneous" perilymphatic fistula.

This study was designed to verify an eventual perilymphatic fistula (PLF) in 264 patients with sensorineural hearing loss (SNHL) and/or vertigo. The patients were exposed to a low-frequency sound stimulation (LFS) on posturography to objectively test Tullio's phenomenon and Hennebert's sign. Endoscopes with 5 degree and 25 degree of visual angle and an outer diameter of 1.7 mm were used. The round window niche, with its foldings, oval window with stapes superstructure, a part of the facial recess and the area in the fissula ante fenestram were examined and video recorded. In one patient, we en

Anomalies of the auditory organ in Potter's syndrome. Histopathological findings in the temporal bone.

Histopathological findings in the temporal bone are described in a newborn infant, diagnosed as having Potter's syndrome. The infant has severely malformed low-set ears bilaterally and a small lower jaw; autopsy findings showed bilateral renal agenesis and pulmonary hypoplasia. The temporal bone indicated the deformities of the inner ear, classified as Mondini-type, complicated by extensive deformities to the external ear and middle ear, including absence of auditory ossicles, atresia of the oval window, abnormal course of the facial nerve, and hypoplastic external auditory canal. The cochlear

CT and MRI for the diagnosis of perilymphatic fistula: a study of 17 surgically confirmed patients.

BACKGROUND: We evaluated the usefulness of CT and MRI for the diagnosis of perilymphatic fistula (PLF) of the round (RW) and/or oval (OW) windows, with surgery as gold standard. METHODS: We retrospectively enrolled 17 patients who presented a surgically confirmed PLF of the round (RW) or oval (OW) windows. All patients were imaged by CT + MRI (T2W SSFP without contrast) prior to surgery (= gold standard). Two radiologists, analyzed the RW and OW on the side of the clinical symptoms and sensitivity (Se) + Specificity (Sp) were calculated. RESULTS: Round window fistula was the most frequent (7

Third window vibroplasty: an alternative in surgical treatment of tympanosclerotic obliteration of the oval and round window niche.

OBJECTIVE: To describe a new technique in surgical treatment of obliterative tympanosclerosis by applying a floating mass transducer (FMT) to a third window. PATIENT: A 64-year-old woman with a severe combined hearing loss due to tympanosclerosis received a third window vibroplasty. INTERVENTION: A mastoidectomy and a posterior tympanotomy via the large facial recess were performed. The promontory was exposed by a transcanal approach. The third window was performed anterior inferior to the round window. The membranous cochlea was left intact. The FMT was gently pushed into the perichondrium-

Histologic variants in otosclerosis.

A study of 164 temporal bones with otosclerosis revealed stapes fixation in 123. Large lesions (4 mm or larger) were characterized by involvement of the cochlear endosteum, increased cellular activity, and bone resorption. Anatomical sites of predilection were: 1. anterior to the oval window, 2. margins of the round window, and 3. apical-medial wall of the cochlea. Two of the five ears with cochlear otosclerosis were from a patient with severe bilateral hearing loss. The size, cellular activity, and location of otosclerotic lesions showed no correlation with the magnitude of sensorineural hear

Use of CT in stapedial otosclerosis.

Otosclerosis (otospongiosis) is a primary focal disease of the labyrinthine capsule. The stapes footplate is fixed when the spongiotic focus expands and invades the oval window. Persons with stapedial otosclerosis experience a progressive conductive hearing loss. In many cases, cochlear degeneration is observed, in which a mixed hearing loss occurs. Using computed tomography (CT), we studied the ears of 45 selected patients with conductive or mixed hearing loss. CT proved valuable in determining otosclerotic changes of the oval window and otic capsule. Spongiotic changes of the otic capsule ar

Results of surgical repair of inapparent perilymph fistulas.

In patients who are thought to have a perilymph (PL) fistula, careful inspection of the round and oval windows during exploratory tympanotomy may be normal. The decision must then be made either to terminate the procedure--knowing that the patient's symptoms will probably continue or deteriorate--or to repair both windows as if PL fistulas were present, risking further damage to the inner ear. From a series of 14 patients explored for possible PL fistulas, we report on 6 patients with preoperative diagnoses of PL fistula, based on history, physical examination, and audiometry. Symptoms resulti

Investigation of the relationship between structural damage and hearing impairment based on the whole ear model.

BACKGROUND: With the existing experimental techniques, it is unable to measure the overall structural vibration of the human ear, which is difficult to reflect the relationship between hearing and ear structures. AIMS/OBJECTIVES: The overall biomechanical behaviour of the whole ear structure during sound perception is described. METHODS: Based on CT scanning data and knowledge of ear physiology, a 3D whole ear numerical model was developed that conforms to the actual physiological environment of the human body. RESULTS: The outer ear canal (EC) has an amplifying effect on sound conduction.

Preoperative evaluation and intraoperative protection of the facial nerve in congenital aural atresia.

We conducted a prospective study to assess the value of high-resolution computed tomography (HRCT) in identifying facial nerve variations in patients with congenital aural atresia and to determine how they affect otologic reconstruction surgery. Our study population was made up of 65 patients (69 ears) aged 6 to 22 years (mean: 13.7) without regard to sex. They were classified into three groups according to their scores on the Jahrsdoerfer grading scale: 46 ears scored 8 or more, 18 had a score of 6 or 7, and 5 scored 5 or less. The course of each facial nerve as determined intraoperatively wa

Anatomical and Surgical Evaluation of the Common Marmoset as an Animal Model in Hearing Research.

Recent studies have indicated that direct administration of viral vectors or small compounds to the inner ear may aid in the treatment of Sensorineural hearing loss (SNHL). However, due to species differences between humans and rodents, translating experimental results into clinical applications remains challenging. The common marmoset (Callithrix jacchus), a New World monkey, is considered a pre-clinical animal model. In the present study, we describe morphometric data acquired from the temporal bone of the common marmoset in order to define the routes of topical drug administration to the in

Role of transtympanic endoscopy of the middle ear in the diagnosis of perilymphatic fistula in patients with sensorineural hearing loss or vertigo.

BACKGROUND: The diagnosis of perilymphatic fistula (PLF) is often difficult, and therefore the condition can be overlooked. Tympanoscopy presents an alternative procedure for visualising the middle ear anatomy, and it may help to diagnose PLF. AIM: The aim of this study was to evaluate the use of middle ear endoscopy in establishing the diagnosis of PLF and in defining its incidence in patients with sensorineural hearing loss and/or vertigo and tinnitus. SUBJECTS AND METHODS: Two hundred and sixty-five patients (22-80 years of age, mean 48 years) were prospectively and consecutively referred
