PCSK9 Inhibitor Alirocumab Improves Diabetic Cardiomyopathy Through the ERK/p38 MAPK Signaling Pathway.

PCSK9 is a gene associated with familial hypercholesterolemia and is involved in other biological processes such as apoptosis, autophagy, and inflammatory responses. This study aims to further validate whether PCSK9 inhibitors can improve diabetic cardiomyopathy and elucidate their mechanisms of action. This study utilized H9c2 cells and C57BL/6J mice to validate the efficacy of the PCSK9 inhibitor alirocumab through in vivo and in vitro experiments. In vitro, alirocumab was shown to enhance cell viability and reduce oxidative stress in H9c2 cells under high glucose stress. It can also decreas

PCSK9 inhibitors in the management of hypercholesterolaemia after heart transplantation in the UK, a single centre observational study.

We looked to establish if Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor therapy can be safely initiated in heart transplant recipients and effectively reduce target low density lipoprotein (LDL). This prospective audit reviewed heart transplant recipients between 1st June 2019 and 1st November 2022 at Harefield Hospital in London. At baseline all patients must have attempted statin and ezetimibe therapy. All patients who remained with an LDL > 3.5 with very high cardiovascular risk or LDL > 4.0mmol/L with high risk were initiated on alirocumab injection every 2 weeks. Monitor

Case Report: Neurological adverse events in subject with myasthenia gravis after PCSK9 inhibitor administration.

BACKGROUND: Myasthenia gravis is a rare chronic autoimmune neuromuscular disorder mainly caused by autoantibodies to the nicotinic acetylcholine receptor. Cholesterol is an essential molecule that affects the distribution and proper functioning of this receptor. Several reports have described the potential worsening of myasthenia gravis in patients treated with statins. CASE PRESENTATION: The patient was an obese 72 years old man, past smoker, diagnosed with ischaemic heart disease, type 2 diabetes mellitus and lipid metabolism disorder. Statin treatment was not implemented because of chronic

Causal relationship between PCSK9 inhibitor and autoimmune diseases: a drug target Mendelian randomization study.

BACKGROUND: In addition to decreasing the level of cholesterol, proprotein convertase subtilis kexin 9 (PCSK9) inhibitor has pleiotropic effects, including immune regulation. However, the impact of PCSK9 on autoimmune diseases is controversial. Therefore, we used drug target Mendelian randomization (MR) analysis to investigate the effect of PCSK9 inhibitor on different autoimmune diseases. METHODS: We collected single nucleotide polymorphisms (SNPs) of PCSK9 from published genome-wide association studies statistics and conducted drug target MR analysis to detect the causal relationship betwee

PCSK9 inhibitors for treating dyslipidemia in patients at different cardiovascular risk: a systematic review and a meta-analysis.

Statin-induced lowering of low-density lipoprotein cholesterol (LDL-C) reduces cardiovascular morbidity and mortality, but many patients do not adequately reduce their LDL-C levels. Monoclonal antibodies targeting PCKS9 are currently in the advanced phase of development. We aimed to investigate the efficacy and safety of PCSK9 inhibitors in patients at different cardiovascular risk in a systematic review. Studies were searched on MEDLINE and EMBASE until January 2016. Differences in the outcomes among groups were expressed as mean differences, or pooled odds ratio (OR) and corresponding 95% co

Discovery, synthesis, and biological evaluation of N,N'-diacyl-m-phenylenediamine derivatives as novel PCSK9 small-molecule inhibitors.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a validated therapeutic target for regulating low-density lipoprotein cholesterol (LDL-C) levels and treating dyslipidemia-related cardiovascular diseases. In this study, pharmacophore-based virtual screening identified 4 (ZINC4681905) as a lead compound, which was subsequently optimized into a series of N,N'-diacyl-m-phenylenediamine derivatives. A total of fourteen compounds were synthesized and evaluated using a fluorescence polarization-based assay. Among them, compound 4a exhibits the highest potency with an IC50 value of 1.07 μM, s

Signal Detection of Adverse Events Associated with PCSK9 Inhibitors: A Nationwide Sequence Symmetry and Combined Worldwide Disproportionality Analysis.

BACKGROUND AND AIM: Despite the increasing use of proprotein convertase subtilisin/kexin type 9-inhibitors (PCSK9i), their safety assessment from large real-world databases remains limited. We aimed to perform a large-scale safety signals detection study regarding PCSK9i. METHODS: A sequence symmetry analysis (SSA) was conducted using the French nationwide health system databases (SNDS); PCSK9i initiators (2018-2023 time-period) were matched to ezetimibe ones, to control for temporal trends and potential indication bias. Outcomes of interest included all designated medical events (DMEs) and a

Inhibitors of Protein Convertase Subtilisin/Kexin 9 (PCSK9) and Acute Coronary Syndrome (ACS): The State-of-the-Art.

Acute Coronary Syndrome (ACS) remains one of the most frequent causes of morbidity and mortality in the world. Although the age- and gender-adjusted incidence of ACS is decreasing, the mortality associated with this condition remains high, especially 1-year after the acute event. Several studies demonstrated that PCSK9 inhibitors therapy determine a significant reduction of major adverse cardiovascular events (MACE) in post-ACS patients, through a process of plaque modification, by intervening in lipid metabolism and platelet aggregation and finally determining an improvement in endothelial fu

The possible role of PCSK9 inhibitors in cardiac syndrome X: Bridging microvascular dysfunction, endothelial pathobiology, and molecular pharmacotherapy.

Cardiac syndrome X (CSX), historically used to describe angina with normal coronary angiography and now largely encompassed within microvascular angina and INOCA, is an ischemic disorder characterized by angina pectoris without narrowing of coronary arteries. CSX, which primarily affects postmenopausal women, is driven by coronary micro-vascular dysfunction, endothelial impairment, oxidative stress, and chronic low-grade inflammation. Notably, the Conventional anti-anginal therapies often yield limited symptom relief, underscoring the need for novel mechanism-based approaches. Proprotein Conve

LDL-cholesterol lowering agents (statins and PCSK9 inhibitors) and the risk of intracerebral hemorrhage: A network meta-analysis.

BACKGROUND AND PURPOSE: Statin therapy reduces the risk of ischemic stroke; however, certain studies have observed an increased incidence of intracerebral hemorrhage (ICH). Moreover, proprotein convertase subtilisin/kexin type 9(PCSK-9) inhibitors have emerged as a powerful class of lipid-lowering medications, potentially with a lower propensity for causing hemorrhagic events. To investigate this matter further, we conducted a network meta-analysis of randomized controlled trials (RCTs) involving statins and PCSK-9 inhibitors that reported occurrences of ICH. METHODS: We performed a literatur

Efficacy and safety of PCSK9 inhibitors combined with statins in acute ischemic stroke: a systematic review and meta-analysis.

BACKGROUND: Acute ischemic stroke (AIS) imposes a substantial global burden of mortality and long-term disability. As novel lipid-modulating agents, proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) have demonstrated therapeutic benefits across a wide range of cardiovascular patient groups, but dedicated evidence in AIS cohorts has not been quantitatively synthesized. We therefore conducted this meta-analysis to compare the efficacy and safety of PCSK9i combined with statin therapy versus statin monotherapy among patients with AIS. METHODS: We searched seven databases from inc
